The employee picked up one end of a small piece of steel dredge pipe located toward the front of their pickup bed and slid it toward the rear of the truck near the tailgate so it could be removed by a forklift. After setting the pipe down near the tailgate, the employee experienced back pain and was determined to have sustained a back strain.

While installing a section of catwalk grating on a dredge, the employee pinched their left pinky finger between the end of the grating and a handrail while sliding the grating into place. The injury resulted in a cut that required three stitches.

The employee was exiting the loader and, while climbing down the loader steps in normal fashion, they felt a pop in their right shoulder.

The employee was lifting a full box of copy paper from on top of a desk in order to move it to the floor. When they went to set the box of paper down on the floor, they felt a pop in their left shoulder.

EE finished making a weld on the excavator bucket, EE lifted welding helmet to chip and clean up what had just been welded and a slag fragment got in eye.

A small piece of metal left over from cutting a pipe with a bandsaw was left deposited on my sweatshirt. When I took off the shirt, a piece of metal got in my eye. An eye doctor removed the piece on 5/1/2021 and I am back to normal.

Installing a heavy drive shaft on dredge. Employee set drive shaft down to reposition employee's body and the U-joint pivoted and pinched the tip of employee's finger. Fracture to the tip of left hand ring finger and contusion under fingernail.

Employee was performing plant maintenance activities, which included removing a loose piece of steel that had just been cut loose from the plant. When the employee tried to remove the loose piece of steel, it dropped suddenly and pinched employee's right hand.

Miner was greasing a dewatering screw when elbow locked up & miner felt a pop in the joint. Miner did not report incident to management until 6/3/16 and did not seek medical treatment until 6/7/16. Miner was referred to another doctor on 6/21/16. We currently do not know the extent or cause of the injury but the miner is still working under light restrictions.

Buffing crusher parts before reinstalling back together.

Employee suffered a back strain while removing an 8' fiberglass ladder from a wall mounted storage rack. Employee was taken to a clinic for evaluation and was given a prescription for a muscle relaxer. It was also advised that he be restricted to Light / Medium lifting duties.

Moving a large tool chest and needed to slide it over, pushed against it with the right shoulder and arm and strained a muscle on the right side of the body.

EE was moving a metal component approx. 30 pounds when he lost his grip and dropped the object. The metal component bounced off of the concrete slab and hit his left shin, which caused a laceration.

Employee had respiratory allergic reaction to fumes in the shop caused by heating of a urethane liner attached to a steel chute being worked on. The allergic reaction caused asthma like symptoms and required medication to counter the reaction.

Employee was lifting a metal wear shoe out of a steel 55 gallon drum when he cut his hand on a sharp piece of metal on the inside of the steel drum. The laceration of the hand required eleven stitches.

EE said he was pulling and lifting on screen retainer to slide retainer out of the way so they could replace screen.

Just moving a battery from one spot to another & felt a pop in left shoulder.

Was dismounting from loader. Hanging onto handrail with right hand, EE stepped off the last step and his right shoulder dislocated.

CUT LITTLE FINGER ON LEFT HAND WHILE REPLACING A SCREEN ON SHAKER. HAND SLIPPED & CUT FINGER ON SHARP EDGE. REQUIRED TWO STITCHES.

STRAINED BACK (LOWER)WHILE LIFTING RAILROAD TIES

REMOVING A BEARING CAP FROM BOOSTER STATION. THE CAP WAS SLICK DUE TO BEING LUBRICATED. THE CAP SLIPPED FROM HAND & HE ATTEMPTED TO CATCH IT. FINGER CAUGH BETWEEN BOTTOM OF CAP & HOUSING. CUT SMALL FINGER ON LEFT HAND FROM UPWARD KNUCKLE OF FINGER TO THE BONE. ALSO CRUSHED BONE.

AN ELECTRICAL CABINET UNLOADED OFF OF A FLATBED TRAILER TIPPED AND FELL OFF CAUSING EMPLOYEE TO FALL CABINET LANDED SCRAPING EMPLOYEES LEFT FOOT-PEELED SKIN OFF FROM TOP OF FOOT CRUSHED AND MA NGLED ALL 5 TOES TOES HAD TO BE SURGICALLY REMOVED.

EMPLOYEE WAS OPENING A WINDOW, SLIPPED AND BROKE WINDOW RESULTING IN A GLASS PUNCTURE TO HIS LOWER RIGHT ARM.

EMPLOYEE WAS PICKING UP A PIECE OF 12" X 3' PIPE TO PUT IN A PICKUP TRUCK.

EMPLOYEE WAS LIFTING TIRES TO HOLD TARP DOWN COVERING BINDER PILE

EMPLOYEE WAS REMOVING TIRES FROM THE ROTARY TUB ON THE PLANT TWISTED & HURT HIS BACK RESULTING IN LOWER BACK PAIN & MUSCLE SPASMS

EMPLOYEE WAS TAKING THE BOLTS OFF OF A CHUTE ON A SHAKER SCREEN ASSEMBLY, THE SCREEN WAS SITTING ONA SAWHORSE, THE SCREEN SLIPPED OFF OF THE SAWHORSE AND IN FALLING RESULTING IN AN INJURY TO HIS FINGER.

EMPLOYEE WAS GE7TING DOWN FROM LOADER AND BUMPED HIS RT KNEE AT FIRST IT ONLY WAS SORE BUT LATER IT SWELLED BECAME VERY TENDER AND REQUIRED SURGERY ON DEC 20 TO REPAIR FLUID SAC UNDER KNEE CAP

EMPLOYEE WAS STEADYING PONTOON SO PIPELINE SECTION COULD BE SET ON IT HIS LEFT THUMB WAS CAUGHT BETWEEN PIPE & ANGLE IRON BRACKET
