The employee picked up one end of a small piece of steel dredge pipe located toward the front of their pickup bed and slid it toward the rear of the truck near the tailgate so it could be removed by a forklift. After setting the pipe down near the tailgate, the employee experienced back pain and was determined to have sustained a back strain.

Employee was working in the shop on lining a chute with polyurethane sheet material. While fitting a piece of the polyurethane into the chute, the employee was using a utility knife to trim one of the edges of sheet when the utility knife slipped and cut the employee's right index finger and part of EE's palm.
