EE was cleaning up sand buildup under a conveyor. When EE leaned over to get inside the conveyor frame, EE turned EE's head and bumped the frame and some loose sand got in EE's right eye. Was wearing hard hat and safety glasses. Turning EE's head sideways allowed sand to enter eye.

Loading a flat tire into a pick up and EE felt discomfort in their left arm. EE said it was not bothering self and EE would wait until Monday to see how it felt. It worsened so EE went to a Dr on 8/7/23 who diagnosed a strain - follow-up on 8/14/23 same, then another on 9/7/23 for MRI which showed a torn tendon. Has worked regular job with no restrictions all along.

Employee was positioning a shim beneath a toggle plate on a feeder and pulled a muscle while positioning it. Employee's initial diagnosis was a pulled muscle and on employee's return visit to the clinic the doctor gave employee a steroid injection. The employee has not missed any work and is still performing regular job.

Employee was changing the skirting on the 53 x conveyor when EE's hand slipped and hit the tail pulley guard causing a 1" cut to EE's right palmar surface. Fabricated tail pulley guard.

Employee was shoveling spilled crushed white rock from beneath a belt at the main plant (build up on concrete surface) and when they forcefully pushed the shovel into the rock they "tweaked" the left shoulder.

Pulled on a hose and it cut the arm had to get 5 stitches.

Operator was climbing up the stairs of the loader to check out some recent damage to the machine and when the employee reached up to feel the metal on the hood, there was a sharp piece that had resulted from a recent fender bender that cut employee's right hand on the dorsal surface. (The machine was a Komatsu a WD-500.

EE was assisting another ee in standing up a 85 gallon drum of transmission fluid (drum was on its side) when ee felt a pain in lower back. EE had left hand on the top rim of the drum and right hand on the bottom lip. The other ee was reversed.

Employee was pulling on a cable (drag cable) trying to situate it in a draw of the NW 95 drag line and subsequent to these actions complained of a burning sensation in right forearm. (Replacing the dump cable on the dragline)

Employee reached down to pick up a hose and said they tweaked their back. Employee was going to place the hose nozzle in a position to wash sand from beneath a tail pulley.

Employee was exiting the water truck after watering the work areas. Employee said when exiting the vehicle their elbow struck the door of the H2O truck causing a bruise to elbow. Employee did not slip or fall to cause the contact with the door.

Employee was welding and when replacing the argon tank, it tipped and began to fall. The employee reached for the tank at arms length and strained back muscles.

Employee was shoveling loose rock from beneath belt when he felt his shoulder pop. Was in the act of forcing the shovel into the loose rock. (Shift end maintenance).

Injured employee was assisting the superintendent adjust the lagging on the pea gravel belt. The guard that had been removed to make the adjustments slid down and pinched the employee's hand where he had hold of a piece of bracing. (Pinched palm surface).

Employee was removing rocks that had become wedged on the feeder, plate when the removal of one, rock allowed another to roll onto his finger and pinching his right middle finger. Fracture & nail bed evulsions.

Employee was holding a 25# steel wear plate while another employee was lining up the bolt holes. The other employee miscommunication with the injured employee and pulled the trigger on the pneumatic air wrench pinching injury's finger.

Employee was removing rocks from a plugged cone crusher when a rock became dislodged and struck his finger. Employee suffered a fractured distal phalange on his left ring finger.

EE was replacing a u-belt onto a pulley and when he was trying to slip the belt over the pulley his fingers (index & middle) got caught between the pulley and the v-belt pinching ee's fingers.

EE was changing the air filter on a 345B Excavator and the air filter canister cut his thumb while he was placing the new filter in the canister. 3 stitch laceration.

Employee was changing a rip-rap screen and when finished he wiped his face with his glove and a foreign object entered his right eye. The eye became irritated over the weekend. Diagnosis was a bruised eye but the Dr. put antibiotic drops in the eye making injury recordable.

Employee was cleaning rock spillage in the screen room with another employee (shoveling rocks back onto the screen surface). The other employee threw a rock toward the screen and struck the employee on top of the head. Employee states hard hat had fallen off as he was coninually reaching down.

EE was assiting other EE's change a screen, when he handed the screen to the other EE's, he strained his groin area.

Employee was lifting a fork to place in the mast of CAT TH63 forklift when his helper dropped his end. This caused the employee's right thumb to get pinched between the fork and the mast.

EE was digging soil from a footing he was forming. When he rotated to throw the dirt from his shovel, his foot did not pivot causing him to twist his knee. The injury did not bother him for over a week before the pain began to intensify. Became reportable today 11/2/06.

Employee was winding a garden hose up and when swinging his hand back to retreive more hose it struck the corner of an electrical box that had a bent area on it. The bent corner cut his hand.

Employee was removing a drawer from a Gilson mechanical shaker (machine off) and cut her left pinky finger (tip) on a lip of the drawer.

EE DISCOVERED A KNOT IN HIS LOWER ABDOMEN AFTER WORK ON THE EVENING OF 7-6-04. WHEN ASKED TO LIST AN EVENT THAT COULD HAVE LED TO THIS HE COULD NOT PROVIDE ANYTHING. HE SAID HIS JOB ALWAYS HAS PHYSICAL LABOR PRESENT BUT HE COULD NOT ATTRIBUTE ANY SINGLE EVENT TO THE OCCURRANCE OF THE KNOT.

EMPLOYEE WAS PUSHING BUTTONS IN THE CONTROL BOOTH AND HIS BACK BEGAN TO ACHE. THE PAIN GOT WORSE DURING THE DAY AND HE HAD TO LEAVE WORK THE NEXT DAY. EMPLOYEE SAYS HE CAN THINK OF NOTHING THA T HE DID TO CAUSE THE PAIN.

EE HAD PLACED A PIECE OF AR PLATE STEEL ONTO THETIPPLE TO BE WELDED IN AND HAD PLACED ANOTHER PLATE ON IT TO HOLD IT IN PLACE AND THE PIECE FELL OFF STRIKING THE EE ON THE FOOT, BEHINE THE STEEL TOE PROTECTION. THE BLOW SHATTERED THE LITTLE TOE ON HIS LT. FOOT.

EMPLOYEE WAS ASSISTING ANOTHER PUT A BIT IN A HYDRO HAMMER & WHILE HE WAS LIFTING THE BIT HE TURNED STRAINING HIS BACK.

EE WAS ATTEMPTING TO PULL THE LEVER ON HIS DUMP BED AND IT STUCK. HE THEN PUT A 2" CHEATER PIPE TO GAIN LEVERAGE AND WHEN THE LEVER WENT OVER CENTER THE PIPE CAME DOWN AND CUT HIS LEFT SHIN.

EE WAS LIFTING A SPRING ROLLER INTO HIS LOADER BUCKET TO TAKE IT BACK TO THE SHOP FOR SOME WELDING AND WHEN HE TURNED, AFTER LOADING THE SPRING, HE TWISTED HIS BACK.

EE WAS UNPLUGGING A ROCK CHUTE WHEN A ROCK SHIFTED PINCHING HIS FINGER AGAINST ANOTHER ROCK

EE WAS CHANGING A SCREEN IN THE WASH PLANT WITH ANOTHER EE. EE WAS HOLDING A CORNER OF THE SCREEN UP WITH ONE HAND WHILE TRYING TO DISLODGE THE OTHER CORNER WITH HIS OTHER. WHILE INTHIS POSTUR E THE OTHER EE PUSHED THE SCREEN EE S WAY CAUSING THE SCREEN TO DISLODGE PUSHING HIS HAND INTO A SPRAY BAR WHICH CAUSED THE LACERATION.

EMPLOYEE WAS UNLOADING SOME STEEL PLATS FROM A TRUCK AND WHEN SETTING TH PLATES DOWN THE WIGHT OF THE STEEL RIPPED THE SKIN FROM HIS FINGER. (HE ACTUALLY SET THE PLATES BACK ONTO THE TRUCK WH EN HE FELT THE EXTRA WEIGHT).

WHILE TURNING OFF A WATER DRAIN ON THE SAND SCREW (EMPLOYEE WAS BENT WAY OVER TO KEEP AWAY FROM THE WATER COMING FROM THE VALVE) THE EMPLOYEE STRUCK THE SIDE OF THE SUPERSTRUCTURE OF THE SAND SCREW WHICH KNOCKED HIS HARD HAD OFF THEN CAUSED THE LACERATION.

EMPLOYEE HAD LAID A PIECE OF CHANNEL STEEL (6') ON THE TOP OF A VIBRATING SCREEN (WASH PLANT WAS SHUT DOWN FOR REPAIRS) AND WAS KNEELING BENEATH IT WORKING ON LOOSENING SOME BOLTS. EMPLOYEE L EANED WAY OVER CAUSING HIS HARD HAT TO FALL (WORKING IN 90 DEGREE HEAT CAUSED THE BAND TO BE SLICK) & CAUSED HIM TO STRIKE CHANNEL WITH HIS BODY THUS CAUSING THE STEEL TO FALL ONTO HIS HEAD.

AFTER CLEANING THE WINDSHIELD OF THE TRUCK THE EMPLOYEE JUMPED DOWN FROM THE VEHICLE AND HIS FINGER GOT CAUGHT IN A BUMPER GUARD. CLEANING THE WINDSHIELD FROM THE FRONT OF THE TRUCK.

EE WAS WORKING ON A STARTER MOTOR WHEN IT FELL ON HIS LEG CAUSING A LACERATION.

EMPLOYEE WAS ASSISTING A MECHANIC LIFT A GATE ONTO THE DRAGLINE AND STRAINED HIS BACK.

EMPLOYEE WAS PECKING A WEDGE INTO A DRAG CABLE AND LEANED OVER TOO FAR. THE CABLE SLIPPED OUT ALONG WITH THE WEDGE AND STRUCK THE EMPLOYEE IN THE MOUTH. THE IMPACT CAUSED THE CABLE TO CUASE A 6 STITCH LACERATION.

EMPLOYEE WAS ASSISTING ANOTHER EMPLOYEE IN REMOVING A DRIVE SHAFT FROM THE BACK OF A PICKUP TRUCK WHEN THE SHAFT SLIPPED & CUT EMPLOYEES LEFT FOREARM END OF THE SHAFT CUT THE UNDERSIDE OF EMPL OYEES ARM A LENGTH OF ABOUT 3IN
