Employee pinched/smashed two finger tips on EE's right hand when caught between two steel shim plates while in the process of adjusting a Jaw Crusher. The pinching incident resulted in lacerations and contusions to the middle and ring finger on the right hand needing First Aid and stitches.

EE was pushing a rollable chute closed and pulled a muscle in their back. The chute track was clear of any obstacles. The EE went to Medic Express for the pain. They gave EE a prescription for pain and placed EE on restricted duty until EE's next visit on 2/22/24.

Employee was changing engine oil in a triaxle. EE was working in the lube pit when the accident occurred. A drop of oil fell between the employee's safety glasses and eye. Hence, employee got a drop of oil in EE's eye.

After removing a tire from the lift axel of a triaxel on GOH equipment number 7-194, the employee felt a pain in lower left back as EE stood back up.

Employee was performing preshift inspection on the secondary plant screen deck. Employee used left hand and arm to raise a rubber curtain on the feed end of the polydeck. At this time, the employee felt a sharp pain in the left shoulder and down to the left elbow.

Some paper fell behind a filing cabinet drawer. While employee was retrieving the papers, they placed their left hand on the cabinet drawer slide. A piece of sharp metal lacerated the left ring finger.

EE was recharging the Cooltech HVAC machine. EE closed the R134A refrigerant tank valve and then proceeded to remove the line set. There was a refrigerant discharge when the line set was removed because the R134A refrigerant tank valve was not completely closed all the way. The EE received a chemical burn on their left hand as they attempted to reconnect the line set.

The employee was helping a mechanic change the cutting edge on Cat 950H Front End Loader 17-77 when the right corner bit fell to the ground and rolled over onto the top of the employee's right foot. The employee saw a doctor 11/8/12. The employee has 4 broken toes and localized swelling. The employee is allowed to continue to work on light duty by the attending physician.

Employee injured lower back while attempting to lift/slide a 100 lb. wear plate approximately 3 feet to a chute opening. Employee was being assisted carrying the plate by another employee when the injury happened.

Three employees were pushing a frozen piece of material off of a conveyor. One employee was using a bar the other two were supporting the frozen piece while baring it off the conveyor. Once the piece was off the conveyor one of the employees supporting the frozen pieced noticed his arm was hurting.

EE was checking tire pressures on Tri axel # 7-124. EE was spinning the lift axel tire to locate the valve stem to check the tire pressure. The lift axel was in the up position. During the spinning process, EE's hand became pinched between the tire and the metal fender. The metal fender cut the back side of his right hand at the base of the thumb.

Employee was struck by a reducer/motor assembly he was helping install when it moved and hit him on top of the left shoulder.

He was rolling a flat tire 11R22.5 up a ramp onto the back of an F350 service truck to take it back to the shop and when he got the tire to the top of the ramp and felt a pop in his back. He reported in the morning that he was having trouble getting moving and came to work late. his supervisor saw him later showing pain when moving and he went to get checked.

Employee tried to turn the belt clamp attached to the conveyor belt to be able to get at the bolts holding clamp. When employee tried turning the clamp up, employee felt a pull in his back.

Employee was climbing into water truck cab, stepping up with left leg and right foot on ground, heard a pop & felt a pain in right knee. Upon moving water truck to fill station, when employee got out of truck, then realized could not put any weight on right knee.

Alex was in the process of loading a new screen cloth from the storage rack into a utility truck. The schroud (crimped edge of the screen) had a razor like edge caused by a bad shear in the manufacturing process. Alex cut his left thumb on the razor like schroud which resulted in 5 stiches.

Employee was standing near his handle that operates the truck bed tarping system. The tarp was jammed. A mechanic from the shop helped free the tarp system and pushed it with his hands. The handle came around and struck employee in the lower lip.

Injury was lifting a 5 gallon jug of antifreeze out of the bed of his pick up truck to restock the lubrication cabinet in the quarry. As He lifted the jug with his right arm over the side of his truck bed, he felt a sharp pain in his right shoulder and right elbow.

Empployee was rolling up a pressure washer hose back onto the hanger in the shed behind the Shop when he felt a pop in his lower back. He then had pain in his lower back and right testical.

Shoveling material for approx. 45 min. twisted left to right and felt pain in lower back.

Setting a top deck screen panel and while moving it around his little finger was pinched.

Carrying empty totes in loader down hill for disposal when one fell out of bucket and went under it. He backed up and got out to put it back in. He lifted it up and went to push it in when his hand slid cutting his arm.

EE was cleaning oil from the bottom of a tank. Cut right forearm on sharp edges on side of tank.

He was reaching into the framework on concrete forms to make sure the block out was flush in the form when he scraped his hand across rebar and cut his finger. Required 2 stitches.

Driver was rolling back tarp on his truck preparing to be loaded using the provided hand crank. While performing this task something popped in his right elbow. Possible snow on tarp made tarp movement more difficult.

EMPLOYEE NOTICED A SLIGHT PAIN IN HIS GROIN AREA AND POSSIBLY A SMALL LUMP IN THIS AREA ALSO. HE WAS DOING NON-PHYSICAL WORK AT THE TIME. (HERNIA WAS SURGICALLY REPAIRED.)

Employee was trying to move a 55 gallon barrel out of the oil building. His figer was pinched between the barrel and the buildings door frame. He received seven sutures. There was a management change at this site on 10-11-04. In preparing the 4th quarterly mine report for 2004 this injury report was discovered.

EMPLOYEE WAS HELPING CHANGE SCREEN ON SECONDARY SCREW DECK. HE WAS OUTSIDE ON A SHORT LADDER TIGHTENING BOLTS WITH HIS RIGHT HAND. HE PUT HIS LEFT ARM UP OVER HIS HEAD TO HOLD ONTO THE CATWALK AND WHEN HE DID THIS HIS SHOULDER CAME OUT OF PLACE.

THE EMPLOYEE WAS ASSISTING IN THE REPAIR OF A 48" CONVEYOR BELT FOR THE PRIMARY CRUSHER. AS HE WAS PULLING THE BLET INTO PLACE, HE FELT A DISCOMFORT IN HIS LOWER BACK WHICH GOT PROGRESSIVELY WORSE AS THE SHIFT WENT ON.

EE WAS OPERATING A FRONT ENDLOADER TO LOAD TRUCKS OUT OF A STOCKPILE. AT SHIFT END, HE PARKED THE LOADER AND STOOD UP TO EXIT THE MACHINE. WHEN HE STOOD UP HE NOTICED A SHARP PAIN IN HIS BACK.

EMPLOYEE WAS HELPING TO CHANGE A BUCKET ON A 330EXCAVATOR HE STEPPED OVER TO HELP THE OPERATOR PUT THE PIN IN AND IT FELL OUT OF OPERATOR HANDS AND THE BUCKET THEN LANDED ON HIS FOOT. DR TOOK X-RAY AND SAID THE BONE THAT WORKS THE 4TH TOE ON RIGHT FOOT IS CRACKED.

EE WAS CLEANING TRACK ON CAT 376 HOE & STRAINED LEFT CHEST MUSCLE.

TRYING TO REMOVE A ROCK THAT WAS STUCK ON CONVEYOR. CONVEYOR WAS STOPPED, THE ROCK FELL ON EMP RIGHT THUMB THAT WAS PUSHED AGAINST A PIECE OF PERMANENT ANGLE IRON. DR STITCHED THUMB UP AND EMP WILL HAVE FULL USE WHEN HEALED.

CHANGING A WEAR PLATE ON A CAT 980 BUCKET. THE BELT HEADS WERE WORN DOWN AND HAD SHARP EDGES ON THEM. THE EMPLOYEE TOOK HIS GLOVES OFF AND CUT HIS RIGHT WRIST. HE HAD MINER BLEEDING, BUT RECEI VED SUTURES.

EE WAS LIFTING A ROCK, THE ROCK SLIPPED & FELL ON HIS HAND CUTTING HIS MIDDLE FINGER

WHILE EE WAS LIFTING THE DISCHARGE HOSE TO HOOK IT ONTO THE PUMP HE FELT A PULL IN HIS GROIN AREA. DIDN'T SEEK MEDICAL ATTENTION UNTIL 3/23/98.

THE EE FELT PAIN IN HIS LOWER BACK WHEN HE WAS LIFTING THE 6" SUCKER HOSE ONTO THE DIESEL PUMP. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 3/5/98.

THE EE WAS USING A 5 GALLON BUCKET TO SUPPLY PRIME WATER FOR A DIESEL PUMP. WHEN HE WAS LIFTING THE BUCKET OUT OF THE WATER HE FELT PAIN IN HIS LEFT SHOULDER.

THE EE WAS CLEANING WITH FUEL OIL WHEN HE GOT HIS FEET SOAKED WITH IT. HE ALREADY HAD A CUT IN HIS TOE, WHICH GOT INFECTED AFTER HIS FEET WERE SOAKED WITH FUEL OIL..

EE WAS KEYING UP A BRICK JOB IN #3 KILN WHEN A BRICK FELL FROM ABOVE THE ONE HE WAS PUTTING IN, SMASHING HIS FINGER BETWEEN THEM.

EE WAS WORKING WITH COWORKER SCALING IN 10E ROADWAY. THEY WERE USING HAMMER AND WEDGES TO BRING DOWN A SLAB OF ROOF. WHILE LOOKING FOR THE WEDGE AFTER THE ROOF FELL, THE EE ROLLED OVER A PIECE OF ROOF AND FELT A PAIN IN HIS BACK.

EE WAS PULLING A COUPLER OUT OF THE ROADWAY WHEN HE PULLED HIS LOWER BACK.

WHEN PLACING FORKS ON TOWMOTOR, FORK SLIPPED OFF OF MACHINE, INJURING THIRD AND FOURTH FINGERS ON RIGHT HAND.

AFTER INSTALLING NEW FEED PIPE THE EE WAS GOING TO THROW THE OLD PIPE UP ONTO THE PIER. THERE IS A STEEL GRAM BY THE WALL THE EE S HAND WAS ON THE END OF THE OLD PIPE WHICH GOT SMASHED AGAINS T THE BEAM.

TRYING TO CLAIM A REOCCURRING INJURY & CONNECT BACK TO 3/23/95.

EE WAS HELPING TO LOAD SOME SCREEN ON THE BACK OF A PICK UP TK. THE RIGHT INDEX FINGER GOT CAUGHT AND WAS CUT.

EE WAS CLEANING SNOW FROM THE TOP OF RAIL CARS. AFTER CLEANING, HE OPENED THE TOP HATCH COVER AND FEL A SHARP PAIN IN HIS LOWER BACK.

EMPLOYEE WAS THROWING BAGS ON A PALLET. HE WAS STACKING BAGS OFF A CONVEYOR WHEN HE FELT A PAIN IN HIS BACK RESULTING IN A BACK SPRAIN.

INJURED WAS WORKING IN CLOSE QUARTERS WHILE INSTALLING A CRUSHER DOOR ON THE LIME CRUSHER. HE FELT PAIN IN HIS BACK.

INJURED WAS UNLOADING STEEL BEAM FROM SHOP TRUCK WHEN HE CUT HIS PINKY FINGER (LEFT HAND). REQUIRED 4 OR 5 STITCHES.
