An operator in a skid steer with an enclosed cab could not get the bucket lowered. To get the bucket lowered another employee was standing on the bucket and jumping on it. While jumping on the bucket EE felt a pop in EE's left achilles.

An operator was driving a haul truck down a heading when the haul truck started to slide on a downhill grade. The truck slid for roughly three headings then overturned in the travel way. The operator was wearing a seat belt and removed from the cab. EE was diagnosed with a hairline fracture on EE's left clavicle

Employee was assisting shoveling a pile - EE fell into the pile and got lime in eyes. EE went to local medical center to get eyes flushed. Treating physician prescribed EE eye drops.

Employee got dust in their eye, scratched an eye and sought medical attention. Went to the eye doctor and was diagnosed with a conjunctival abrasion. There was nothing in the eye but was prescribed antibiotic eye drops.

An employee was observing a drag chain conveyor. EE's left hand was holding a drill with chisel attachment, the chisel slipped, and EE's finger contacted the corner casing of the elevator that carries coal to the conveyor. The emergency department classified this injury as a laceration/open fracture to EE's left-hand ring finger, requiring surgery to close the wound.

Our driver was attempting to unhook hoses from the tractor to the trailer so they could disconnect the tractor from the trailer. Their foot got tangled with the step on the back deck of truck. Although they were holding on to the safety bar, they slipped and fell. They bruised their left side.

Our loader was at the Carbis rack at the mine's surface location attempting to open the lid on a pneumatic tank trailer. When they pulled on our latch, they struggled with getting the latch to move. They pulled too hard causing a strain to their groin area. This caused a hernia to their groin area. They will need to have surgery.

Employee was welding a patch on the enhanced hydrate blow down line. EE stated EE got something in EE's right eye, and thought it was slag from chipping EE's weld. EE washed out EE's eyes and used eye drops on site before going to a local medical center. At the med center the doctor used a Q-tip to remove a spec from EE's eye and prescribed EE eye drops for a scratch on EE's cornea

Employee was getting onto the forklift when they felt a pull in left hip/groin area.

Employee was holding onto railcar gate opener "rail wrench", while it was closing the railcar gate. The "rail wrench" vibrates when closes the lids, during this they felt a pop in their left bicep.

Employee was replacing a take up on a conveyor belt when, while doing so EE slipped resulting in a loss of balance and EE's arm impacted a guard adjacent to the belt. Went to doctor and was released to full duty that day. Followed up with doctor and fluid containing blood was drained from hematoma. Received doctors' notes on 12/4/23.

Employee was moving railcars and was very hot due to the high temperatures outside (90 F+). EE realized EE stopped sweating, which EE recognized as a sign of potential heat stroke and told EE's coworker EE needed to get out of the sun. Coworker helped get EE inside to air-conditioned break room. EE was taken to Medical Center via ambulance where EE received multiple IV's.

Employee had just got done fueling the bolter when they heard crumbles hitting their hard hat, they then went to exit the area when a rock fell an estimated 22 feet from roof of the mine hitting their arm. Was taken by ambulance to hospital where x-rays were taken and revealed a broken left forearm.

Employee was getting on the bus to ride underground and tripped on the first step. When EE caught self, EE hit EE's finger on the step.

EE was trouble shooting high differential pressure on a dust collector. When EE entered the collector EE found a blank with a punch plate instead of a bag. EE used a pair of plyers to try and free the punch plate from the blank, while doing so EE's plyers slipped and hit EE in the mouth breaking a tooth and chipping another

Employee was using a bar working on unplugging a pipe. While working, employee's hand struck employee's helmet-mounted earmuffs and knocked hard hat and safety glasses partially off face. At the same time, an employee one floor above had opened an inspection door, and vibration caused ground lime to fall through grate flooring and into the lower employee's eye.

The employee was loading trailers with another employee. EE told co-worker that didn't feel well. When EE was in the passenger seat of the tractor, EE passed out and fell over. EE's co-worker sought help from lime mine employees because EE couldn't move EE. EE was in cardiac arrest.

Employee was replacing guard on return roller of 196 belt. The right hand side of the guard fell off the pin and the guard swung to the left, pinching EE's left hand between guard and frame. Laceration to left index finger which required 5 stitches.

Employee was attempting to knock debris loose from from a dump chute located above a vibrating feeder on the no.8 kiln. The debris inside broke loose creating a large cloud of lime dust that surrounded the employee. This got into both eyes. Employee was wearing safety glasses with dust seal and respirator which were current requirement.

Employee reportedly lost consciousness while driving work truck and crashed into a concrete barrier. Employee wasn't injured from the wreck. A non-emergency ambulance was called to come to the site and check the employee out. The ambulance arrived at 1:30 pm and conducted some tests on the employee, employee's family took them to the hospital for further evaluation.

The employee was cutting angle iron with a 4.5" grinder. Employee shut the grinder off but, with the wheel decelerating they set it down and it made contact with the back of their hand causing a laceration wound.

At approximately 3:40 PM, the employee contacted the foreman to tell the foreman the employee wasn't feeling well and had chest pains. The foreman brought the employee out of the mine and the Mine Superintendent transported the employee to the hospital. An examination and tests were performed at the hospital and the employee was released.

Hose on tire machine disconnected suddenly when employee was pulling; struck back of left hand on frame of machine. First aid applied to middle knuckle laceration initially; one week later noted limited finger movement, physician diagnosed tendon injury requiring surgery, making injury reportable 5/5/22.

Employee entered area where blasting occurred <15 minutes prior to inspect shot. A piece of rock fell from 25' roof, striking the employee's hardhat and left shoulder, fracturing clavicle and scapula. Trying to flee the area, employee likely twisted an ankle and fell on large rocks, fracturing fibula above right ankle and seven ribs left side. Employee was hospitalized.

Scale ring in Kiln 7 broke apart and created ash dust, which entered the cooler. The hot dust flowed through the cooler belt feeder and onto the cooler belt, which ignited due to the high temperature. Damage was limited to the cooler belt, head pulley, some rollers, and nearby electrical wires/cables.

Employee pulled stomach muscle pushing/sliding the rack to load a trailer. Possible hernia.

Tramming Fletcher Scaler and the tram motor went out causing it to free wheel downhill. Miner applied manual break. Scaler traveled 10-20 ft. and hit rib. On impact, miners leg slipped off break and hit the bracket holding the fire suppression system in cab causing a laceration.

Roof fall in North East Section of the mine. Nobody was injured. Damage to brattice did occur. MSHA and DEP are notified. Orders issues by MSHA and State PADEP.

Drive belts on Metso vibratory screen were improperly tensioned, which either caused or was caused by the screen being overloaded. This caused the belts to slip and catch on fire, which ignited the screen dust cover. Damage was limited to drive belts and dust cover.

Miner complained of chest pains while driving truck through the portal on way to surface office. Forman contacted and ambulance called for care. Miner was transported to hospital. Returned to work next day OK, pains believed to be caused by acid reflux, pollen allergies and high blood pressure.

Discovered 7:47 am, intersection of 35 E 42 S.

Employee was washing a small glass cup used in an instrument to test lime quality. The bottom of the cup cracked and fell out, employee cut hand on sharp edge.

4:30am flames observed on kiln 6 guard. Employees used garden type hose used for wash down located near site of the fire, reduced it to point where the guard could be opened. Once opened, fire extinguisher used to extinguish existing flames by 5:00 am. Oil level was above operating level for the reservoir causing it to drip off gear and onto hot kiln shell where it ignited.

Roof fall in inactive area of mine.

11/9, EE was opening a dribble chute on K6 preheater. Chute opened more quickly than expected. Momentum of the handle caused EE's hand to be caught between the handle and a dumpster. It did not hurt much initially but the next day it was worse, EE went to the ER and was diagnosed with a fractured second right metacarpal. Reported to company on 11/12.

Employee unloading 50-lb bags of ANFO. After unloading 3-4 bags, employee felt a "pop" in EE's shoulder then unloaded the rest of the bags (47). Reported to supervisor 30 min later. Orthopedic evaluation and X-ray next day confirmed nothing broken or dislocated. Employee given restriction, no pushing/pulling/lifting greater than 5 lbs with left arm. Follow up after MRI.

Employee was steam cleaning haul truck (H-14) at the steam jenny. Climbing down off of truck and foot slipped. Was holding wand in one hand. Fell from 2nd rung from the top landing on the ground hitting the left side of EE's body. ER visit confirmed nothing broken. Put on work restriction due to soreness and limited motion.

At approximately 510 PM there was a radio call that employee was having seizure. It was break time, employee was at the UG office. 911 was called. Foreman responded with the emergency truck and employee was brought out of the mine to surface mine office. The ambulance was directed to surface mine office. Employee was transported to hospital by ambulance.

Employee cleaning floor hole at the face. Small (size unknown) rock fell from roof or face (location unknown), hit EE in the shoulder. Initial x-rays (12/17) at ER did not show fractures. Employee requested follow up apt. due to reoccurring pain. Apt. 12/31 with specialist found possible fracture, employee given restriction no use of right arm with a CT scan and follow up apt.

Employee heard on radio gasping for air. Radios have name indictor on them. Employee found in seat of fuel truck struggling to breath and then became unresponsive. First response team found EE had no pulse. CPR and AED was used. Employee taken by ambulance to ER with cardiac arrest. After investigating this appears to be a medical emergency due to natural causes.

Employee was plasma cutting on a loader bucket. EE then was needle scaling the slag off when a piece of slag shot off and got under EE's safety glasses.

Employee was shutting a sliding guard. As EE shut the guard EE pinched wrist between the handles of the guards. The handle of the guard hit into EE's right arm resulting is a fracture in the right wrist.

Oil from the Kiln 6 drive gear sump contacted the kiln shell, exceeding the oil's flashpoint and started a fire. The kiln was shut down for 36 hours to replace the guard, which had buckled due to the heat of the fire.

Employee was using a wrench to remove a swivel fitting. The head of the wrench broke and the wrench came back and hit the employee in the face, cutting EE's nose. EE received 5 stitches.

Employee was hand scaling from JLG basket. A rock came off rib and pinched finger between rock and hand railing of JLG basket.

Employee cleaned out air hood, put it on, piece of lime fell out got into their eye, behind prescription safety glasses. Employee flushed eye, no pain, saw lime in eye. On 1/8/19 they observed lime was still in eye, wanted it removed. 1/9/19 - Went to doc., lime fell out. Prescribed drops to prevent infection.

While inspecting the area of a pipe for a hole that required to be repaired lime fell on the employee as EE was looking up. The lime fell directly on the employees face, getting behind EE's foam lined safety glasses.

Employee was using a long pipe to tap a hopper of a dust collector to determine how full it was based on sound from tapping. On the first tap, the employee felt a pain in wrist.

Repeated use of a water hose. Holding the hose caused soreness in hand, diagnosed as "bil hand pain, trigger finger". Given work restriction no use of hose.

Employee unplugging Hydrator, plugged due to power outage. Using pipe wrench to move the first stage of the hydrator, wrench broke. Employee caught self to regain balance, pulling shoulder. Employee was initially seen by doc., returned to work no restrictions. Follow up visit identified weakness, determined to be from tear in rotator cuff. Employee had surgery on 1/9/2019.

Employee removing sensor from transmission using prybar. Prybar slipped, employee hit hand, resulting in laceration on finger.

Employee was welding grating down and was bent over when employee twisted employee's back causing pain in lower back

Employee was using 2 hammers to drive rail anchors into railroad track. When employee struck the hammers together a shard of steel came off and pierced right leg below the knee.

Employee working in 24E @ 37S, tripped over rock and hit shin on another rock. Employee worked next two days without issue. On Monday 5/14 employee asked for doc. apt to have evaluated. Injury evaluated 5/15, diagnosed with severe contusion employee put on limited duty 5/16 to 5/18.

Shot crosscut into intersection at 10:00 pm 4/13 at shift end. Fall happened sometime between 10:00 pm on 4/13 and 8:50 am on 4/15 when first signs of a possible fall were discovered, confirmed by foreman at 9:05 am. No damage or impairment. No production crews were underground on 4/14 or 4/15. First evidence of a fall discovered by crew checking water pumps on 4/15.

Kiln 7 was starting up and just began to produce lime. The hot lime coming out of cooler ignited belt. Belt was burned, wire were burned, and rollers were destroyed.

The employee was driving haul truck in underground mine and ran over rock on road. Reported tightness in upper back but thought EE was OK. EE later requested a medical evaluation. EE was see on 2/9, and was put on restricted duty. An investigation was completed and it was determined that hitting the rock may have aggravated an exiting condition.

Employee bent over to tighten hose clamp. When ee stood up ee felt pain in back.

Occurred on 3rd shift (10:00pm-6:00am). Employee changing screens in the METSO screen, pulling on a screen, felt a pain in shoulder. Diagnosed with tendinitis on 12/8, permitted to return to work, and could continue job as normal. The pain continued, follow up MRI found tear in the rotator cuff which was diagnosed on 1/16. This will require surgery and time off.

Employee was raking coating from Kiln #6. EE used a long metal rake. EE shoulder felt sore, ee reported it to supervisor on 10/24/2017, but did not want to seek medical care. EE thought it was just sore, but wanted to let them know. On 10/25, ee requested to see a medical professional for evaluation. EE was seen on 10/25/2017, and was diagnosed with Rotator cuff tendonitis.

Cleaning out dust clogged dust collector, air fired on collector and pushed air out cleaning port, hit employee in face. Lime burns in eye.

Employee on bed of pick up truck picking up empty paint cans. When employee attempted to exit bed of truck, EE elected to step off on the tire side, as opposed to back. The back had a step provided by bumper. As EE exited the side, right foot on tire, left foot caught the wooden bed rail, as EE unhooked the foot, EE pulled it back, landed on left foot awkwardly, twisting knee.

Inserting cable bolts into drill holes on day shift working from a JLG man basket. This required the employee to reach away from EE's body while pushing the bolt into the drill hole. After installing 15 bolts, employee back felt sore. EE continued to install bolts with support of other employee. At lunch felt pain in back, was seen by doc, diagnosed with lumbar sprain/strain.

Employee retrieving sample bucket from K8 rail car load out. Lime collected in corrugated tube. Employee pulled bucket off stand, the tube flew up in the air, lime in the bend of the tube, in corrugated sections flew out spraying employee chest and face. Lime went under safety glasses and got in both eyes. Eyes were flushed onsite, then in ER. Follow-up at eye care provider.

Employee was loading a pneumatic tank in the pebble loading area. EE pinched right thumb with the bar that is used to close lids. EE was given 4 stitches.

Temperatures were very high and the humidity was very high. The employee was loading trailers and was overcome by the heat. EE suffered from heat exhaustion.

Installing the a burner pipe on Kiln 7, a tab needed cut off the pipe seal ring. Employee used 4 1/2 in grinder with a .042 cutting wheel to cut tab off. Grinder started to bounce, employee stopped. EE then when to cut the tab again and the wheel shattered. A broken part of the wheel flew back and cut employee left hand, middle knuckle.

Loading hydrate for transfer. Loaded 32000 pounds. Vibrated the shoot several times to remove lime from shoot. Lifted the shoot out of the trailer and walked back along the trailer to close the lids and lime came from the shoot and down over contractor employee. It got in face and some got in eye.

Employee loading drilled holes in the face. This involves using a hose to inject emulsion into the holes. The employee was pulling the hose from one hole to the next. It was caught on a rock on the mine floor. Employee pulled the hose to get it to the next hole and felt a pop in R bicep. Diagnosis was minor tear in bicep, limited duty, restricted use of R arm.

Operating mechanical scaling equipment, an enclosed can. Mechanical scaler, scaling the wall, pulled rock off the wall, shook the cab of equipment. Employee was wearing seatbelt, jerked EE's body and experienced lumbar strain. Employee was seen by doc., lumbar strain. Follow up 1 week later, prescribed physical therapy. Employee working, no work restriction.

Employee was grinding and welding all day and noticed EE had something in EE's eye when EE got home. EE tried flushing the eye at home. Reported injury the next day. Tried flushing on site, went to local clinic where referred to ER.

Employee over stepped the first step and fell down four steps.

It was raining. The employee slipped on wet lime. This caused him to fall to the ground. He was checked out at the hospital. He has an inflammation in his back.

Employee was walking to office and slipped on wet pavement which had lime dust spillage and injured his knee.

Employee felt sharp pain in right shoulder when his scaling bar slipped.

Employee had just come down from the scaffolding and was standing on the 5th floor of the kiln when he turned to stop and twisted his ankle.

Employee was drilling a flange when the drill twisted his finger causing a fracture.

Employee alleged a back injury on 9/28/15. Injury was not reported until 10/1/15 when the EE requested medical treatment. EE was diagnosed with a back strain and placed on restrictions on 10/1/15.

Employee was tightening bolts overhead with an impact wrench. Debris fell behind his safety glasses and entered his eye causing an irritation.

After cutting a groove in a yellow mine pipe, the employee wiped his eye with his hand and felt something in his left eye.

Individual was bending wires and sprained his right shoulder. The individual was placing 500 mcm wires into a disconnect and bending them to fit into the disconnect. They were working with a partner at the time of the injury as well.

Removing wood block from metal stand used to hold new burner pipe. Moved block, stand fell, impact foot behind steel toe. RT Foot Contusion, work with minimal ambulation. Required several moves to clear power lines, increased employee interaction. Lack of prep how stand was set up, the wood block was placed on stand and was not needed.

Employee was getting off truck and walking to shanty when he noticed a pain in his right knee

Employee struck on left leg by falling steel. employee received contusion to left calf. employee taken off work until follow-up with doctor.

Employee removing sheet metal from shipping container when he cut his finger on sharp edge. Employee was wearing gloves.

Employee loosened bolt two turns when he was told to wait to do clean up, the metal clamp on the guard broke causing the guard to pin his leg against the skid steer. Bolt punctured skin on right calf.

While doing maintenance work the employee accidentally hit the dust collection pipe above where he was working, which caused dust to fall between his safety glasses and face, into his eye.

Possibly due to fact employee had stretched prescription safety glasses, which slid down on his nose, not covering top of eyes. (issue not reported until employee left work site)

While employee was hand scaling and baring the loose roof he felt something give in his side.

Employee was moving an I-Beam with another employee when he felt a strain in his back. He immediately reported incident and continued to work. employee reported back to work on 12/15 and stated his back had not gotten any better and was sent to the clinic. employee was given light duty for 5 days and prescription meds.

Injured back pulling up broken shock on catwalk at mine. On 12/8/14, he made a delivery. Reported the injury from lifting a hose to his supervisor on 12/9/14. He did not indicate that he initially injured himself at the mine on 11/25/14 until he came into fill out injury report. No witnesses. Told loader it needed fixed. Told him he hurt his back.

Employee cut his little finger on a sharp metal edge of a light fixture.

Parked lift, stepped out onto rock and rolled left angle.

Employee was climbing down from a piece of equipment and his foot missed a step and as he fell he strained his right shoulder.

The employee slipped on ice and fell twisting his leg.

Employee bent over to pick up a switch and could not stand back up.

The haul truck the employee was driving overturned and came to a rest on the passenger side

Employee was decending from ladder and his foot slid down bottom two rungs and twisted his knee when his foot hit the floor

Was cutting zip tie from hose reel to be installed and cutting blade slipped and cut right index finger requiring stitches.

Employee was tightening a fitting on water pump of roof bolter when wrench slipped hitting left thumb

Employee was operating a haul truck traveling down rough roadway in mine. The back end of the haul truck bounced a couple times from hitting uneven road which caused injury to the lower back.

Employee was attempting to open a lid to our pneumatic tank. The lid was frozen shut. The employee pulled the lid to open it. This caused a hernia to the right side groin area.

Employee started noticing symptoms of carpal tunnel and scheduled an appointment with physician on 1/31/13 to get it checked. Symptoms started about 3 months prior to accident date above. Physician scheduled surgery for 2/19/13.

Employee entered kiln to help scale during kiln shutdown. A large piece of coating came loose and rolled down pile and hit his foot.

This was reported as a first aid on 11/12/12 but employee did not seek medical attention until 12/5/12. Employee was pulling electrical cable in a tray when his retractable engaged causing him to twist his back.

The Employee, was working at height and fell approximately 9 feet to the surface below when the weld of tie off point failed. Employee sustained broken ribs and bruising.

Employee was cutting with a torch and when completed he flipped up his shield and dust rolled off of shield behind his safety glasses into his right eye.

Employee was grinding the rough edges off of a 4" x 6" x 1/8 plate using a bench grinder. He tilted the plate at an angle and the wheel sucked the plate into the gap between the guard and grinding wheel, lodging his finger between the wheel and plate.

Employee was standing on catwalk of the Gradall, mechanical scaler doing his pre shift, checking the oil. Employee was bent over closing the bottom engine cover latch and when he went to stand back up he lost his balance and fell off catwalk.

While operating a D-5 Oldenburg dual boom drill the drill slipped and lost traction and the rear of drill impacted a pillar. Employee was thrown into dash on impact.

Our Employee was moving an small extension ladder with both hands, trying not to hit another Employee. The Employee stepped back and his left foot went over the kick plate and into a guarded hole in the floor of the Turbine/Generator operating deck. The employee did a split and strained his groin.

Employee was pulling a 3" hose to hook up to pump. When pulling hose he felt a strain to lower back and left arm. Employee did not seek medical attention until 6/29/11.

A fine mist of chemical released from bleeder port of pump at boiler system got carried in the air from the wind going through the building and went up into employees face and into his right eye.

Employee could see the steam coming off of the diesel pump and knew it was not pumping. Cold water running into the hot pump case created steam pressure that exceeded the hose clamps capability and it blew off the suction hose. Hot water then spilled out of the pump case and splashed onto the employees legs.

Employee was loading and unloading roof bolts from the bed of a pick-up truck, he then went to pick up a wheel block from behind the tire and felt a strain to his lower back.

Employee was handscaling using a scaling bar to the the rock to insure it was not loose. While prying up on the bar the piece of rock at the bars tip broke loose and allowed the bar to travel up in a snapping motion. Employee did not seek medical attention until 7/7/10.

Laceration to left hand removing a bolt on a drive shaft assembly of a CAT 962 Loader with an air impact gun.

Employee stepped out of the equipment (Gradall Scaler) he was doing a pre-shift on, missed the step and jumped to the ground.

Employee was found to have occupational induced hearing loss based on audiogram.

Employee felt pain in left forearm and wrist after loading bolts into the magazine of roof bolter. Employee did not seek medical attention until 5/25/2010.

Employee was handscaling in a scaling basket. He was leaning back to scale a piece off the face and roof area and felt strain to his back. Employee did not seek medical attention at this time. On March 7, 2010 he did seek medical attention but did not receive medical treatment until 4/5/10.

At 12:50 the Site Supervisor called main office and advised that he had been told by his contact at the plant that an employee from night shift had went to the hospital as a result of lime burns around his neck. Employee had work his shift from Monday, 02/01/10 at 6:00pm to 4:00am Tuesday, 02/02/10.

Employee strained right shoulder clearing a jam in the underneath side of the crusher. Employee did seek medical attention and needed surgery which was scheduled for 3/9/10. Employee worked up to the day of his surgery.

Employee was servicing a dust truck, reaching between front tire and frame rail, trying to loosen fuel filter to remove.

Employee was walking from hydrate to coal hopper, watching for plug ups.

On 9/22/09 at the end of the employee's shift, the employee was working on the primary feeder pulling a gear off of the primary stone feeder assembly, when the gear disengaged from the shaft. The employee tried to catch it to keep the gear from falling into the lower gear and felt a strain in his lower back. Employee seeked medical attention on 10/6/09.

Employee was walking by burner pipe of Kiln #7 when limestone dust particulate blew out hitting him in face and eyes.

East side of 23W at 4N intersection roof fell above the anchor point of the bolts. Mechanical scaler was working on northeast corner of intersection at the time of the fall.

Employees right foot slipped on a rock and he fell, hitting his right arm just below the elbow.

None: Suspected roof control problem area. Area was bermed off prior to fall on 5/19/09. Roof fell on 5/22/09.

Employee was found to have occupational induced hearing loss based on audiogram.

Employee was shoveling a pile of stone on 4/27/09, he felt a strain but did not seek medical attention, on 5/5/09 employee went for a regular routine physical where his family physician found a hernia.

Employee was standing on a pallet containing metal covers for screws placing tools in the bed of his service truck before moving it, when he slipped and fell, cutting his right lower leg.

Area of roof at intersection of 1N and 6W fell, impeding travel along the primary escapeway. Fall of roof occurred at start of 3rd shif and no miners were in the mine at the time. This area of the mine was developed prior to 1953 and was never bolted. Area has been scaled and ground support added at edge of the fall zone.

An unexpected roof fall was discovered during a pre-shift inspection of the underground mine on the 2-10pm shift. The roof fall is above our normal bolting horizon of 8 ft. The roof fall did not cause any personal injuries, entrapment of an individual, impair ventilation or evacuation escapeways. No mining equip., infrastructure or mining utilities were effected by the roof fall.

Wind, loose dirt.

Employee was using an air impact wrench to loosen bolt on a piece of mobile equipment. Employee was holding onto socket so it would not come off and had hand in contact with rotating part of wrench.

Employee was moving from bay to bay in the #6 kiln preheater when a piece of refractory fell onto his right hand.

Intersection of 15E and 28S roof fell above the anchorage of the bolts. Intersection is in an area that was bermed to prevent entry.

Employee was wearing rubber bibs and jacket, rubber gloves, face shield and saftey glasses to peform a line break on a 93% sulfuric acid line. Employee stood to the right of the nut he was turning. Sulfuric acid misted out of the line into the air when he turned the nut. The mist got on the right side of face and neck.

Employee was found to have occupational induced hearing loss based on audiogram.

Employee was taking bolts out of a guard when the guard slipped off falling down striking his right hand and cutting it.

Employee was climbing down a ladder at old water tank, when he slipped.

Employee was attempting to start a pump, when a falling pipe fell toward him, knocking him down.

Employee was shoveling lime under cooler belt, when a gust of wind blew, and he got lime in his eye.

Employee was raking coating off of the cooler grates of #6 Kiln. While raking, the kiln puffed back from positive pressure inside the kiln, causing dust to fly out of the door, where he was raking from.

Employee was cleaning plugged collectors, by repetitvely hammering on side with a rubber mallet.

Employee was operating a Komatsu hydraulic hammer scaler. While scaling the face, the motion of the Komatsu scaler lunged forward.

Employee was shutting the tail gate of a triaxle mack dust truck. When tailgate went shut the employee had his right hand index finger in pinch point.

Employee was reaching down in a chute to clean and overstretched.

Employee was walking fron one area to another in the plant and stepped into a frozen rut, twisting his right knee. He did not seek medical attention until Sept. 29, 2008, resulting in surgery scheduled on 10/22/08.

Employee was trying to unplug a chute that takes lime away from the hydrator. While trying to unplug the chute, the access door came open causing lime to cover employee's face.

Emp. was walking from one conveyor to another, stepped on stone, twisting his knee.

While loading hydrated lime, employee turned the system off. However, the lime continued to flow after it was turned off. The control room became very dusty so he went outside the control room where conditions were very windy as he tried to get out of the dust. Employee had lime blow into his face receiving chemical burn to eyes and skin.

Roof fall in an abandoned area of the mine tore down ventilation brattice. No other impact to mining operations, travelways or active work areas. No injuries related to the roof fall occured.

Bottom step of Gradall broke when employee was getting in to it, causing him to come down and hit his abdomen, strain his back and right knee. The accident happened on 10/10/07, he went out on lost time 5/7/08.

Employee cut finger on piece of pipe while cutting with torch.

Employee was tightening bolts on the gear of the kiln when he felt a pop & pain in his lower back. He had been pushing up on the wrench when he should have been pulling down. Employee went to clinic on 9/10/07, taken off work for 2 days, released to modified duty until released to regular duty on 9/18/07.

Emp. was walking up steps, going to old kiln floor, his foot slipped off of step and he hyperexteded his right knee when his right foot landed on the floor. Employee had an MRI done on 9/20/07 and was given a shot of cortisone in his knee. The doctor put him off work until 9/25/07.

Emp. had pressure applied to pipe that he was holding in place to be welded. Emp. sneezed a few times and felt a pop in his mid stomach area. He later noticed a bump. Emp. has not had anything done to repair the hernia at this time.

Was grinding overhead with face shield and safety glasses on when a piece of metal fell in left eye. Employee received prescription medications for eye injury.

Crew was restriging cable after maintenance, running the cable through an unsecured becket when the cable came loose & struck employee in the face. He sustained a broken nose & cheekbone & returned to restricted duty. EE quit after returning to work for 2 restricted days.

Employee was guiding in an equipment block that was hung up on a bolt. He put his hands on it to get it loose when it fell on his right little finger resulting in a fracture and restricted duty.

Employee was lifting a steel kiln plate when it slipped & fell onto his (R) thumb fracturing the tip & avulsing the nailbed. He continued to work, but 2 weeks later asked to see a doctor who gave the above diagnosis & allowed employee to return to work without limitations.

Employee was found to have occupational induced hearing loss based on audiogram.

Employee was stepping down off of a scaler when he stepped on a rock and twisted his right knee.

Employee was pulling a collector pipe over to put in place, as he was pulling it over he felt a pull in his shoulder. Employee was not able to get an appointment to see the doctor till 3/9/07.

Employee strained lower back while hand scaling at face while pulling bar.

Using hand tools (carpal tunnel).

Employee was inspecting hinge point on a gradall, when he slipped and twisted his back.

Employee was hooking up a welding lead to thaw a water line when he slipped on the ice. *Note* Employee returned to work on 2/5/07. He could not get a doctors app. till 2/22/07. On 2/22/07 the doctor put him on leave.

Employee was scaling at kiln when dust from the ash ring came down and fell into eyes.

Employee pulled too hard and rapidly on prybar

Cleaning lime buildup from the cyclone at the hydrate plant. Loose wet material came down and landed in face, affecting the right eye. Was wearing protective equipment (safety glasses, hard hat, gloves, etc).

Burned hand while grabbing hot piece of metal.

Employee slipped while walking on catwalk of the Gradall. He struck his jaw on the handrail of the Gradall. He damaged a tooth.

Rock struck windshield causing windshield to break. Piece of foreign material in eye.

Employee bent over to pick up a piece of hose and felt something pull in his lower back/hip area.

Employee strained right shoulder when lifting pipe.

Employee was walking up stairs to hydrate building and strained right knee.

The roof fall was in an active area of the mine where benching was taking place. This fall occurred over a long holiday weekend, there were no personal injuries or equipment damage. The fall did not impede ventilation or passage. The report time was within the required 15 minutes. Location-16 east x 33 south. Size-aprox. 50' x 50' x 11'. It was above the roof bolt anchorage zone.

Employee stepped on a rock and twisted right ankle.

The fall occurred in the 17 East X 33 South intersection. The size was aprox. 50' X 75' x !2' deep. This was a known problem area and was blocked off Friday night prior to the fall. The fall occurred on an off shift and was reported as soon as it was determined to be reportable. The fall did not impede passage, effect ventilation, cause personel injury or damage equipment.

Employee strained his left wrist and arm while climbing up onto equipment to clean it.

A shot was put off at aprox 2:00PM Friday that conected 27 South with 27 East going West. This area has been a known problem and the shot was allowed to set over the weekend. Size of fall: 50'x70'x10' It did not affect ventilation of passage however it was above the 8' & 9' bolts. The day of the fall was not a scheduled work day. Pumping only.

Employee strained back & hip while driving a truck with a broken seat.

Roof Fall in an area of the mine that had been inactive since 5-31-05. The location is the intersection (old four way) of 2 east & 31 north (aprox. 300' x 50')It fell above the 8' Jenmar bolts . It shows shearing, calcite jointing and dampness. No personal injuries, no equipment damage, no issue for ventilation or escapeways. The area was baracaded.

He was cleaning off lime belt opened door to let light in on tail wheel and lime came out and into his eyes.

THE WORKER WAS MOVING SCRAP METAL AROUND AS HE WAS MOVING A PIECE AND A PIECE BESIDE IT FELL OVER AND PINNED HIS LEFT HAND BETWEEN THE TWO PIECES OF SCRAP STEEL. HIS 2nd & 3rd DIGIT FINGERS WERE BROKEN.

Slipped on tracks of scaler and hurt lower back

Slipped and fell on left leg, twisting hip and ankle. Employee did not seek medical attention until 11/03/05

Oct 13,2005 employee was walking across shop floor and felt something snap in his left knee. He was not able to see a doctor unitl 10/17/2005.

Their was not any injury(s) or equipment damaged nor did it impede ventilation. The roof broke above the bolting horizion. Location 29 South @ 17 East. Size of fall is approximately 50ft x 50ft x 14ft. This area was blocked off to all mine personnel since 5/24/04 anticipating this fall.

Was coming down a ladder and slipped and fell on left side.

While scaling the face of 14E - 31N, a piece of rock came off of the rib and hit the employee on the right ankle.

Employee was taking a drill steel apart with a pipe wrench and the wrench slipped off striking the employee in the face.

EMPLOYEE LOSS HIS GRIP ON GRINDING TOOL AND WHEEL HIT HIS GLOVE AND CUT HIS THUMB

Employee was pushing a wheelbarrow and his foot slipped causing him to hit his chin on the wheelbarrow and cutting his chin.

Oxygen hose failed below the fitting near the torch-end thus causing a fire and igniting the employee's glove on fire. The burning glove caused second and third degree burns on the right hand of the employee.
