Mill operator was atempting to dump a 5 gallon bucket of garbage into a large dumpster, while lifting up to place in the dumpster some of the contents fell out of the bucket and struck the top of the operators hand resulting in a laceration of approx. 1.5 inches in length requiring 3 stitches.

Mostrando los 22EE was lifting around 20 lbs tray for a salad bar. While lifting, she twisted her lower back, lower back muscle stain. She was distibuted with 2 doses of 30 mg of Toradol on site. Rotational s chedule, RTW date is 8/6/08.
While operating was moving liner into place, EE pushed liner to rotate in position. The liner bumped the wall pinching his finger between wall and liner, Resulting in a fractured finger on his left hand
While transferring oil from 55 gal. metal drum to the waste oil tank,the vent was overflowing and employee quickly shut off the valve and by do so caught his right middle finger at the tail end of a double hose clamp resulting in a laceration to that finger
Employee was moving a portable bandsaw when the base came in contact with the track rails inlayed in the shop floor. Employee tried to force the unit over the rails causing the saw to fall on his leg. Employee received a 3 inch laceration. Employee received 15 stitches.
While pushing material down in a dumpster a piece of wire cable sprung out and contacted the employee's chin causing a small laceration. Five stitches were required.
Employee was pulling on a hose when he felt a sharp pain. Later diagnosed as a broken rib. No blunt injury occurred. Employee is unsure of how it happened.
While employee was climbing down ladder he turned to get off and struck his right elbow on the aluminum door frame. Incident was not reported to his supervisor.
Employee was lifting when he lost his footing and felt pain in mid back region.
Employee was in the process of lifting wire mesh panels into a pickup bed for transport the wire mesh panels are 4ft by 8ft and weigh approx. 10 lbs each.
Employee was hanging poly pipe and was trying to secure a piece of loose mesh when he walked into the wire knocking his safety glasses off and resulting in a small laceration just under where his glasses normlly rest. 7 stiches were used to close the wound.
While unloading 2 inch metal pipe from the scissor lift, 2 pieces started to slip off and employee tried to catch them. The weight of the pipe hyperextended his left wrist.
Employee was hanging a length of 4" pipe on the rib. The pipe slipped and smashed the employee's fingers between the pipe and the safety rail on the lifting device. The pipe was wet and slippery. The tips of three fingers were caught. The employee was wearing gloves. The accident resulted in bones in two fingers being cracked.
Employee was moving table and had palm of right hand between table and wall causing the right palm to be pinched and resulting in a 1 inch laceration requiring more than 1 stitch. He was treated and released back to work with no work restrictions. Medical recordable based on treatment.
Employee was installing bolts and lacerated the right index finger on the bolt.
As the employee was lowering a roller into the manbasket, the roller struck the outside of the employee's right ankle.
Employee's hand was inbetween an air mover and dunnage as the air mover was being set.
Employee was pulling new 350 MCM cables from the power car to the new camp. He stood up from a crouching position and felt a belt of pain go across his lower back, above his tail bone. He went to the PA's at 11pm who gave him med's for the pain and sent him to the medical clinic at 930am on 5/11/05. He was given a 7 day light duty release, "no lifting or pulling".
Employee was loading containers on to a trailer. While trying to loosen the safety catch on the container door latch, it suddenly broke free pinching his finger between the handle and the frame.
EMPLOYEE WAS RELOCATING JACKLEG ON SCISSORS DECK & KICKED THE HOSES TO MOVE THEM & FELT A "POP" & THEN PAIN.
EMPLOYEE WAS ASSISTING ON A CONCRETE POUR & WAS PART OF THE CREW HANDLING THE PUMPED CONCRETE LINE HOSE. HE FELT SOMETHING "POP" IN HIS ABDOMEN & PROBABLY WILL BE TREATED FOR A HERNIA.
JOB TITLE: COOK. EE LIFTED TOO HEAVY OF A POT WITH WATER AND BEANS ONTO A STOVE TOP WITHOUT ASKING FOR HELP OR FILLING THE COOKING POT WHEN ON THE STOVE TOP AND STRAINED HIS STOMACH. DIAGNOSED AS HERNIA. HERNIA SURGICALLY REPAIRED 1/19/05.
