Mostrando los 8Employee was doing normal work on the packaging line. Employee reached for a 50 lb. bag, and as employee pulled it forward, felt a sharp pain in upper L. arm.
Tailgate on 10 yd dumtruck was out of adjustment. Employee tried to make adjustment using bare hands, rather than a bar. L middle finger was grazed, crushing and tearing nail and tip of finger. L ring finger bumped, but not cut.
Backache was reported on Sep. 28, 2015. Employee suspected strain happened on his own time, but reported "just in case." Employee treated backache with first aid/exercises. On Oct 07, 2015, employee requested to file a workman's comp. claim, which we can not deny. We have requested an investigation into the validity of this claim as a workman's comp claim.
A rock was lodged between the tailgate and the bed of the truck. Pulled the locking pin, the bolder dislodged and tailgate dropped. He thought his hand was clear of the latch, but it was not. Skin on R finger was torn, and required stitches to repair. No days lost. Came right back to work the same day after repair.
Employee was cleaning overhead door. It made a "clicking" noise so he went to turn it off. The door fell and folded in half. As it fell, it grazed his face, cutting and scraping it.
EE bent down to pick up a pallet. His back hurt and as he stood up, he felt a sharp pain. Sent him to the chiropractor.
Worker was replacing a bearing. The bolt he was loosening broke and his hand slipped down onto a sharp piece of metal. The metal cut his right thumb.
