Upon standing and moving away from the desk, employee's foot got caught on the computer mouse cord and EE fell, hitting their right cheek on a desk, causing a laceration.

Worker attempted to adjust a conveyor belt while in motion, instead of stopping it and adjusting it. EE's Right ring finger was caught by a roller, and EE's finger nail was pulled off the nail bed.

Employee was doing normal work on the packaging line. Employee reached for a 50 lb. bag, and as employee pulled it forward, felt a sharp pain in upper L. arm.
Employee slipped and fell in a muddy patch on the concrete. A wide walking path was cleared after a rainstorm. Employee chose to walk over the 8 inch muddy area, rather than use the 4 foot walking path that had been created. Employee strained right knee.
Employee was bit by a spider on the calf. The bite would not heal, and required intravenous antibiotics to promote healing.

Tailgate on 10 yd dumtruck was out of adjustment. Employee tried to make adjustment using bare hands, rather than a bar. L middle finger was grazed, crushing and tearing nail and tip of finger. L ring finger bumped, but not cut.

Miner was using sledge hammer to break apart a large rock that was stuck in the jaw of the crusher. The hammer slipped off the rock, and EE banging EE's R. index finger on the side of the crusher, cutting EE's knuckle.

Backache was reported on Sep. 28, 2015. Employee suspected strain happened on his own time, but reported "just in case." Employee treated backache with first aid/exercises. On Oct 07, 2015, employee requested to file a workman's comp. claim, which we can not deny. We have requested an investigation into the validity of this claim as a workman's comp claim.
A rock was lodged between the tailgate and the bed of the truck. Pulled the locking pin, the bolder dislodged and tailgate dropped. He thought his hand was clear of the latch, but it was not. Skin on R finger was torn, and required stitches to repair. No days lost. Came right back to work the same day after repair.
Employee was cleaning overhead door. It made a "clicking" noise so he went to turn it off. The door fell and folded in half. As it fell, it grazed his face, cutting and scraping it.
EE bent down to pick up a pallet. His back hurt and as he stood up, he felt a sharp pain. Sent him to the chiropractor.

EE was standing on the track of the D9G, removing a snow tarp. EE slipped and fell off the track landing on his right side onto the ground. EE felt pain in his right arm. EE went to the ER at Holy Rosary. X-rays were taken and determined he had a crack in the bone of his upper arm.
Employee stepped upon a bin lid before ensuring lid was properly seated on bin. The lid shifted, turned up and hit him in the groin. He lost his balance and fell scraping his outer thigh.

EE was burning weeds with a portable propane weed burner, he turned off flame and laid the nozzle down to do another task. EE reached down to pick up the burner and was distracted by another EE and picked up the hot nozzle with his bare hand.

Worker was replacing a bearing. The bolt he was loosening broke and his hand slipped down onto a sharp piece of metal. The metal cut his right thumb.

HE WAS TRIMMING A BELT, THE KNIFE SLIPPED AND CUT HIS LEFT ARM. THE BELT IS USED ON THE CRUSHER TO KEEP ROCKS FROM LEAKING OUT WHERE THEY AREN'TSUPPOSED TO. IT WAS BELTING MATERIAL, NOT AN ACT UAL BELT THAT RUNS A MOTOR.
