Employee was moving a pipe in a vertical position under the mill when the flange on the bottom of pipe caught employee's right hand and fingers. Employee was placed on restricted duty for 3 days.

EE was working on the stand for the ball mill, when a cloud of dust fell from above, striking EE on the head, resulting in a strain to his neck and left shoulder. EE was taken off work on 6/17/07 and returned to light duty on 6/18/07.

EE setting 4" pipe on the pipe stand, he lifted the pipe the wrong way, which caused a strain on his back. EE was placed on restricted duty for 3 days.

Working on a Long hole drill. Was changing drill rods above his shoulders and pulled his left lower groin area. Has a Hernia. Didn't have a good platform to work from.

Was walking and using a pry bar as a walking stick in his right hand and carrying tools in his left hand. The bar stuck in some wire. He jerked on it and pulled his shoulder injuring it.

Employee was getting out Van and put her hand in the frame of the sliding side door and another person slamed the sliding door on her finger.

The employee was taking his chalk block off of the lube truck when he started to loose his grip and tried to catch it and twisted his right wrist. He was taken in for X-rays and released for full duty with a brace. He went back in on the to see the doctor 07/23/2006 because his hand was not getting any better. He then was put on light duty, which turn this into a reportable.

Employee was hanging ventilation tube; he was about to install a bolt in the support chain. When he pulled on the chain for more slack his hand caught on the expanded metal used for ground support cutting his left ring finger. He was taken to surface where his cut was cleaned then he was transported to the hospital where he received 9 stitches.

Employee was removing skid plate from the bottom of the loader to access a leaking hose. The plate fell after bolts were removed, hyper extending his wrist by pushing his hand down against his forearm. X-rays showed no fracture, diagnosed as strain.

Caught left pinky finger between power box and rib. Bent finger back, dislocating it and chipping the bone.

While drilling conventional uphole, pulling tube to add rods, he felt a pain in his back.

While working on drill, poked finger on frayed boom cable. By the following day, finger showed signs of infection.

Employee was attempting to lift the seat on the blasting truck to get to the batting. As he pulled very hard his shoulder became very sore. He had pulled his rt shoulder joint out 7 months ago working for another company.

Employee had removed a metal guard and was attempting another task. He stepped onto the guard, tested it and assumed it was stable. as he put his full weight on it it folded in half and mashed his foot.

Employee states he was reaching into the passenger compartment to retrieve a lunch box as he was pulling on the box a large piece of pipe fell on his right hand smashing his right ring finger.

While bleeding air out of hydraulic system, the fitting was loosened too much causing high pressure fluid to be released, peeling back the skin on the palm of my left hand.

Employee leaned a large sheet of wire up against the rib, it bowed out toward him and struck him in the face.

Miner was mucking a raise bore vent raise and mucked below the brow. A piece of loose rock rolled down the muck pile and struck the miner on his hard hat. The hat caused a laceration on the miner's forehead.

EE WAS ASSISTING THE MAINTENANCE GROUP WORKING ON A CONE CRUSHER. ONE OF THE HYDRAULIC ARMS FELL. HE TRIED TO CATCH THE ARM WITH HIS RIGHT HAND AND WAS STRUCK BY THE ARM AND FINGER WAS PINCHED AGAINST THE END OF THE SHAFT.

Had new rod trialer supended from hoist installing axles. The trialer slid and hit him, cutting his head. 10 stiches.

Ee was getting on & off the Kabota tractor & loading equipment onto it. His back began to hurt to the point that he could not stand up.

Scored a rock bolt with a hacksaw and was bending bolt to break bolt when it flip and hit arm.

Ee was taking down larger power cable off of the back when he felt pain in his elbow and forearm he tried to wait and see if it was going to be ok, but decided to go to a dr on the 17 ft of aug 2004.

Employee was putting wheel chocks down on a vehicle. When employee stood up he hit the door of the vehicle with his head.

EE WAS PICKING UP A SHEET OF EXPANDED METAL. ONE CORNER OF THE SHEET SNAGGED THE GROUND AND FORCED THE SHEET AGAINST HIS ELBOW.

EE WAS LIFTYING SUPPLIES FOR HIS JOB WHEN HE FELT A SHARP PAIN IN HIS GROIN

EE WAS REMOVING RICKID VENT TUBE THAT WAS SUPPORT BY A CHAIN AND BOLT ONCE HE REMOVED THE BOLT THE TUBE WAS TO HEAVY AND FELL DOWN ON HIM AND STRUCK HIS KNEE.

PUT TUBE IN RODS CAUGHT BOUNCED BACK HITTING HAND.

EMPLOYEE WAS ATTEMPTING TO PLACE A NEW 50 LB. FILTER INTO A SEPARATOR AT THE COMPRESSOR BUILDING. HE LOST CONTROL OF THE FILTER AND IT CAME DOWN ON TOP OF HIS RIGHT THUMB.

EE WAS WORKING ON ONE SIDE OF A SMALL METAL SHED AN OPERATION IN A TRUCK ON THE OTHER SIDE OF THE SHED WAS AT TO BACK UP INSTEAD HE WENT FORWARD AND PUSHED THE SHED INTO HIM PINCHING HIM UP AG AINST A LOADER.

EE WAS ATTEMPTING TO CLIMB UP ONTO A UNDERGROUNDTRUCK WHICH WAS ON GUIDE, HE WAS UNABLE TO PUT HIS FOOT IN THE TOE HOLE SO HE PUT HIS KNEE IN AND PULLING HIMSELF UP WHICH CAUSED PAIN TO HIS RIGHT SHIN.

EE WAS CLOSING THE DOOR ON A DRILL WHEN THE HANDLE CAME OFF AND CRUSHED HIS THUMB.

THE EE STRAINED HIS KNEE (LEFT) WHILE ATTEMPTING TO CLIMB ONTO A HAUL TRUCK (CAT 777C) THE EE REPORTED THE INJURY TO HIS SUPERVISOR ON 1/4/99

REMOVING A BOLT FROM A BOLTER, BOLT BROKE & HANDSLIPPED & CUT FINGER

EE WAS REMOVING UTILITY LINES WHEN THEY REACHED THE NEXT SECTION. THEY LOCKED OUT THE VALVE ABOVE THEM & THEN RETURNED TO DEPRESSURIZE THE LINE. THE VALVE WAS STICKING & THE EE TRIED TO FO RCE IT OPEN. HIS GLOVES WERE WET & HIS HAND SLIPPED OFF THE VALVE. AIR PRESSURE THEN CONTACTED HIS ARM

EE WAS UP ON A POLE PUTTING ALUMINUM WIRE INTO TRAVELERS, WHE HE REACHED OUT TO GRAB WIRE, EE SUFFERED A STRAIN TO RIGHT SHOULDER. EE WAS PLACED ON RESTRICTED DUTY FOR 3 DAYS.

MOVING TIMBERS. TIMBER ROLLED OVER ON WRIST.

EE WAS RESUPPLYING HIS GROUND SUPPORT WIERE. HE LEANED A PIECE UP AGAINST THE RIB AND TURNED TO WALK AWAY WHEN THE PIECE FLL AND STRUCK HIM ON THE NECK.

ADDING A ROD & TWISTED WRONG, WHEN TIGHTING IT.

EMPLOYEE WAS POURING #2 FUSION FURNANCE WHEN A CRUCIBLKE FELL AND STRUCK HER LEFT FOOT ABOVE THE STEEL TOE. EMPLOYEE WAS SENT TO THE DOCTOR ON 11/09/97. I DID NOT RECEIVE THE C-4 REPORT UNTIL 11/24/97 STATING SHE HAS A HAIRLINE FRACTURE. THIS REPORT WAS DEVELOPED THE SAME DAY.

EE WA SUNLOADING 4" PIPE FROM A SUPPLY TRUCK. HE HAD ONE END OFF WHEN THE OTHER END SLIPPED DOWN. THIS CAUSED HIS END TO POP UP AND COME DOWN ON HIS LEFT HAND.

EMPLOYEE WAS WORKING ON A SOLENOID VALVE OF A HANKINSON AIR DRYER. THE DRYER WAS SHUT DOWN AND ISOLATED. WHEN HE UNSCREWED THE SOLENOID VALVE, BACK PRESSURE FROM A LEAKING ISOLATION VALVE BLEW THE PISTON OUT OF THE VALVE STRIKING HIM ON THE HAND, CHEEK AND NOSE.

EMPTYING A TUBE. PULLING TUBES OUT OF RODS.

PULLING A PIECE OF DRISCOLL PIPE AND PULLED A MUSCLE IN HIS BACK.

WHILE DELIVERING SUPPLIES TO UNDERGROUND STORAGEAREA, EMPLOYEE CUT BANDS ON A PALLET OF 18" BOLTS. TWO OF THE BANDS HAD BEEN CUT AND THE LAST BAND BROKE ALOOWING THE THE BOLTS ON THE TOP [ALLE T TO FALL ON HIE RIGHT HAND CAUSING A LACERATIONTO THE WRIST.

EMP WAS MOVING STEEL POUE TABLE IN FIRE ASSAY ROOM IN PREPARATION OF POURING SLAG. TABLE FELL OVER AND STRUCK EMP ON LEFT FOOT CAUSING FRACTURE.

EMP WAS PLACING AN EMPTY OXYGEN BOTTLE ONTO A RACK WHEN HE EXPERIENCED PAIN IN AND AROUND THE MID SECTION OF HIS BODY (LOWER ABDOMEN). THIS HAS BEEN DIAGNOSED AS A HERNIA. SURGERY HAS BEEN SCH EDULED FOR 4-24-97.

EMP WAS CLIMBING UP THE LADDER OF A DRESSER 150 TON HAUL TRUCK AT THE BEGINNING OF HIS SHIFT ANDFELT SOMETHING POP IN RIGHT SHOULDER. AFTER SEEING DR THAT DAY, SHOULDER CONTINUED TO HURT. EMP REFERRED TO DR WHERE HE WAS JUDGED TO REQUIRE SUGERY WHICH WAS PERFORMED ON 4-4-97.

EE WAS PUTTING IN A PIECE OF SPILING BETWEEN LAGING IN STEEL SETS WHEN THE UNSECURED LAGING FELL OUT AND LANDED ON HIS LEFT THUMB CAUSING A LACERATION AND FRACTURE.
