Was loading a face round and stumbled over a rock and caught himself with his hand and cut himself.
Was checking on a pressure tap seal in the Roaster. While stepping onto a grating to turn ball valve the grating bent down at a 45 degree angle and his foot slipped into the hole and fell to his knee. He caught himself against a hot cyclone and burned his forearm and hands.
Prying on grating when he hyperextended his left knee. Employee was placed on restricted duty.
Employee was moving a pipe in a vertical position under the mill when the flange on the bottom of pipe caught employee's right hand and fingers. Employee was placed on restricted duty for 3 days.
Employee was punching holes in a rubber belt with a hole punch and a hammer when the hammer slipped striking the employee on left hand thumb area. Employee was placed on light duty for 3 days.
EE was working on the stand for the ball mill, when a cloud of dust fell from above, striking EE on the head, resulting in a strain to his neck and left shoulder. EE was taken off work on 6/17/07 and returned to light duty on 6/18/07.
EE setting 4" pipe on the pipe stand, he lifted the pipe the wrong way, which caused a strain on his back. EE was placed on restricted duty for 3 days.
Employee was working on Sissor Deck lift ajusting the Vent bag and turning on the air and water, when either he was knocked off or fell off of the sissor deck approximately 10-15 feet and landed on his head. He was not wearing fall protection.
Was working on the boot wash with a screw driver and his screw driver slipped and he poked himself in the left hand.
Was walking down the stairs and caught his foot on something and rolled his foot.

Climbing down off of a haul truck his foot slipped and he caught himself with his left arm and pulled his shoulder out.

Employee was taking a shower and became light-headed and pulse rate was high. Employee thought he might be having a heart attack and went to ER by himself. Doctor could not find anything wrong with heart and asked him if there anything out of the ordinary that happen that day and employee said that the chlorine in the shower water was strong.
Was walking and using a pry bar as a walking stick in his right hand and carrying tools in his left hand. The bar stuck in some wire. He jerked on it and pulled his shoulder injuring it.
Employee was helping to install a shock absorber, while holding the nut with his hand, there was an impact wrench on the other side tighting the bolt.
Was making his rounds walking through the Mill slipped on some spilled flocculent that was wet and fell on his right hand trying to catch himself.
SSX Mine Z1-6945-XC3. Operator was bolting up the area when he noticed the ground starting to work. He called his supervisor and told him that he was going to pull out because of the ground working and they need to shotcrete this area before they could bolt it. Operator was pulling out of area and stopped to roll water hose up, when the back came in on the Boom of the bolter.
Was changing the oil on a Kubota tractor. While taking the oil plug off the oil pan his wrench slipped off of the oil plug and he struck the frame of the Kubota, causing numness to his left wrist and hand.
Employee was getting out Van and put her hand in the frame of the sliding side door and another person slamed the sliding door on her finger.
Had stepped off of the drill to take a sample and heard a noise and looked up while a drill steel came down and hit on top of instep of his right foot and broke it.
EE was stepping off of a drill and stepped on to a rock and rolled his right ankle and broke it.

The employee was taking his chalk block off of the lube truck when he started to loose his grip and tried to catch it and twisted his right wrist. He was taken in for X-rays and released for full duty with a brace. He went back in on the to see the doctor 07/23/2006 because his hand was not getting any better. He then was put on light duty, which turn this into a reportable.
Was putting in a pin to hook up the Cubex Drill and a Teledyne Boom truck and the Teledyne moved and pinched his fingers on his right hand and had 8-10 stiches put in it.
Walking and ankle rolled on him and he heard a snap. He hurt his ankle about 5 years ago. This was a re-injury to his injury from before.
Person was turning screw bolt for the back door of an underground cement carrier truck when his wrench slipped off of the bolt and he hit his back of hand above the thumb on a steel guardrail post.
Employee was hanging ventilation tube; he was about to install a bolt in the support chain. When he pulled on the chain for more slack his hand caught on the expanded metal used for ground support cutting his left ring finger. He was taken to surface where his cut was cleaned then he was transported to the hospital where he received 9 stitches.
Employee was removing skid plate from the bottom of the loader to access a leaking hose. The plate fell after bolts were removed, hyper extending his wrist by pushing his hand down against his forearm. X-rays showed no fracture, diagnosed as strain.
Employee covering mill gear with plastic and he was tied off, he slipped on plastic when the harness pulled him back against a bolt on the mill door. Employee was released on restricted duty. Project was complete and employee was layed off on 4/22/06.
Employee was attempting to move a plate of steel, when he lost his footing on loose rock and fell to the ground with the plate landing on his left forearm causing the bone to fracture in two places. Employee did not work the second day of injury. He was placed on restricted duty. Project was complete on April 20, and employee was laid off.
Had escape raise hoist's squirrel cage go through the limit switch on top and break off both limit switches brackets and shut the escape hoist down. We had to send a person up on a snow mobile to check it out. Hoist was down, until we could open the road up to the top of raise and get a boom truck up to the top raise and repair it.
The injured miner was struck by falling rock while he was bolting and screening in a heading. The loose rock came through /around previously screened and scaled ground. The previous screen had collected some loose rock and appears to have given way.
Employee slipped while pulling water hose from bolter and landed on left side, causing neck pain. Saw doctor on 3/8/06 where physical therapy was prescribed, making this MSHA reportable.
While tracing piping at cooling tower, slipped on snow and hit knee on hugger bolt. Caused small laceration on knee, requiring 4 stitches.

The prell hose failed and blew prell into eye, causing minor burns to skin around eye and small laceration on eye.
Manbasket employee was in stuck on rib while lowering to floor. When basket came undone, it fell approximately 2 feet, jarring his knee.
While drilling conventional uphole, pulling tube to add rods, he felt a pain in his back.
While working on drill, poked finger on frayed boom cable. By the following day, finger showed signs of infection.
While operating underground haultruck, hit hole in road and hit head on top of truck.
Employee was attempting to lift the seat on the blasting truck to get to the batting. As he pulled very hard his shoulder became very sore. He had pulled his rt shoulder joint out 7 months ago working for another company.
Employee had removed a metal guard and was attempting another task. He stepped onto the guard, tested it and assumed it was stable. as he put his full weight on it it folded in half and mashed his foot.
Employee states he was reaching into the passenger compartment to retrieve a lunch box as he was pulling on the box a large piece of pipe fell on his right hand smashing his right ring finger.
