Employee was pulling & hooking up the dogs & twisted wrong. Employee did not go to doctor until 12/20/2018 due to the fact that they were hoping their back would get to feeling better, but it stayed a constant pain. Hence, the reason for the delay in seeking medical attention.

EMPLOYEE JUMPED DOWN FROM THE CRANE, HURTING HIS LEFT SIDE WHEN HE LANDED. EMPLOYEE CONTINUED TOWORK UNTIL JULY 26 WHEN HE UNDERWENT SURGERY FORTHE LEFT INGUINAL HERNIA. HE CAME BACK TO WORK ON 9-21-94 AFTER RECOVERING FROM THIS OPERATION.
EMPLOYEE TRIED TO UNHOOK CHAIN FROM BLOCK BEFORE BLOCK WAS LOWERED COMPLETELY TO THE GROUND THE FINGER NEXT TO INDEX FINGER ON LEFT HAND WAS SMASHED BETWEEN THE CHAIN & THE BLOCK OF GRANITE
