Mostrando 50 de 384Employee attempting to loosen the chuck on a cordless drill that had a hole saw bit in the drill & as EE was twisting the chuck their left hand slipped causing #2 & 3 fingers to contact the teeth of the bit resulting in lacerations. Seen by site medic, referred to the ER, 2 sutures were required, Full work release was issued making this a medical reportable.
On 2/19 employee was helping partner pull a jackleg out of the backfilled floor when EE felt a burning sensation in the right shoulder. When EE reported for the next shift on 2/20 EE reported the incident to supervisor and was then seen by the site medic & referred to the ER. X-rays were negative, restrictions were issued for muscle strain to right shoulder area.
Employee was conducting Shelby tube sampling on tailings using hydraulic ram set. While taping cap to sample tube, the sampler slipped from chucks lacerating and fracturing the tip of the right number 4 finger. Full release, no days lost. Employee is from Canada and has valid TN-1 status. (no social security number)
Employee was attempting to enter the back of a vehicle when EE felt a pop in EE's left calf muscle on EE's last shift. EE was seen by the site medic. The following day the employee went to a doctor and was diagnosed with a muscle tear and restrictions were issued.
Employee finished bolting & while wiping face with a rag, EE felt something in left eye & EE stated EE got it out. At 4:00 am it started bothering EE again, EE reported it, was seen by site medic, referred to the ER, then onto a opthamologist where a rust ring was removed & restrictions were given.
Employee was bolting with a jackleg drill and while was bent down picking up the drill a 6-foot drill steel fell and struck the employee across the back of the head. The employee called off on last shift of the rotation. Upon further investigation when the employee returned on the next rotation (4 on 4 off rotation) it was determined that EE called off for a headache.
Employee was carrying a Gardner Denver drill to the face and while carrying the cradled drill EE felt pain in the center of EE's back. EE was diagnosed with a back strain.
Employee was carrying a BO soot Trap Filter back to the service vehicle when EE stated, that EE stepped wrong and felt a pop followed by discomfort to right knee. EE reported to the supervisor and was seen by the site medic and referred to the ER where EE was issued restrictions.
Employee got a glass sliver in right index finger cleaning up a broken window 7/12/16. Sliver was removed, seen by medic & released back to work. 7/21/16 went to physician as felt something was still in finger, examined, given full work release. 9/11/16 seen again as it seemed to be infected, injury location was opened & cleaned requiring 1 suture to close becoming reportable.
An employee and a partner were lifting a valve onto a flatbed pickup, when the valve was on the vehicle they went to lay it down flat, the employee pinched their left little finger between the flatbed and the valve causing a laceration. Employee was seen by the medic and referred to the ER, sutures were required, no restrictions were issued, no days lost.
Employee went to close the man gate of a John Deere tractor, upon doing so pinched the tip of right pinky finger between the gate and man basket frame. Employee was seen by the site medic and referred to the ER, X-rays were positive for a hairline vertical non-displaced fracture of the pinky finger tuft, full release was issued and no days lost
Employee was performing a sand pipe swap and while attempting to line the 4" pipe up to connect it felt a pull in right bicep area. The employee was evaluated on March 3rd and received restrictions making this incident reportable.
After employee finished drilling a round with a jumbo drill, was changing the bit on the jumbo and was struck on the ring finger resulting in a finger fracture and laceration.
Employee stated lifted a core barrel tube and twisted and felt pain in lower back. Employee ended up receiving medical treatment and received a full work release to return to work on 2/22/2016.
After unloading a rail flat car, employee & 2 others were pushing it back by hand, at which time the employee noticed a pair of rail tongs laying in the way. The employee reached down to move them, in the process pinched left pinky finger between the tongs & the rail flat car causing a laceration. Employee was sent to E.R. and received sutures.
Employee was opening a drawer in a tool cabinet and a port-a-power ram rolled out of the tool cabinet landing on the top of the employee's right foot. Employee was seen by E.R. x-rays negative but restrictions were issued.
Employee picked up a metal ladder and turned to move it. On the next day, 11/14/15, he reported that he has experienced some discomfort & pain in the left leg, associated with moving the ladder on the previous day.
Employee finished loading a round & while attempting to remove the air hose from the anfo pot that was in the bucket of a 4 yard mucker, a rock dislodged from under the bucket causing it to contact the top of the employee's foot, resulting in a metatarsal fracture.
Employee bent over to remove a rock from the roadway. He moved the rock & felt his left shoulder "pop". He went to medic, then to ER, X-Rays were negative, but restrictions were issued.

Employee has been experiencing some elbow discomfort for some time now. On July 20, 2015 he went to Doctor. Employee had some fluid drained off & had received a full release - no restrictions. He was diagnosed with Bursitis.
Pulling drill pipe swivel hose, caught on machine head. End of rod swung up hitting the driller in the eyebrow
Employee stated while pulling core tube out of pipe with both hands, twisting & pulling - he stated he felt a pinch in his neck, causing discomfort/soreness. Went to medic, then to ER. Doctor issued restrictions.
Employee was bending down to pick up the wheel chock blocks from the parked vehicle on the surface & felt a "POP" in his left knee. This was a REPORT ONLY at the time. Employee has missed NO days, until 7/13/2015~ when he had surgery on his left knee(7/13/2015 = 1st day missed)
Employee was preparing to put a drifter on jumbo feed beam, R. hand (#4 finger) got pinched between the drifter & bumper of the service vehicle while attempting to unload it. X-rays: positive for finger tip fracture. Sutures required. Restrictions were issued.
Employee was setting up to bolt, when he picked up jackleg drill out of bolt deck. He lifted & twisted improperly. He experienced lower back pain & discomfort. Employee went to medic, then down to ER. X-rays were negative. Doctor issued restrictions.

Employee was working with 36" hard vent line in the back of a lift truck when a section of the vent line came down & contacted his left hand, which then got pinched between vent line, back & bed, causing a contusion. Restrictions issued.

On 7/17/2014 employee was loading jumbo steel onto rack in laydown when he experienced discomfort & some pain in his left shoulder. He went to medic on site -It was a report only. On 7/29/2014, he stated the he still had some pain in his left shoulder, went to doctor - restrictions were then issued. (first shift missed, 8/2/2014).
Employee was lifting axle to set onto cart, his left index finger got pinched between cart & axle. Sutures required. no days lost. full release issued
Employee was assisting with loading a fan onto a flatbed and load shifted, pinching the tip of his right ring finger.
Employee stated that while hanging screen in the back, his elbow contacted the screen (while wearing Kevlar sleeves) causing laceration ~ which required sutures. No restrictions issued - No days lost - full release.

Employee was removing a 6" bit and 4' drill rod from the drill feed on an up hole. When the employee lifted the bit and rod from the feed, he lost grip of the rod and forced his hand into the side of the drill feed causing a boxer fracture on his right hand.
Employee reported to us on 2/23/2014 ----- he stated that on 2/18/2014 he was pulling a bull hose & felt discomfort between his shoulder blades

Employee was hanging screen when a small rock contacted tooth & chipped it. Employee went to dentist & tooth was repaired. (Note: this happened on employee's Friday on his rotation)
