Mostrando los 12Night shift employee was lifting a metal plate out of the third level dump hopper. As EE was attempting to pick the plate up EE felt a sharp pain in the lower back. EE then reported the incident to the supervisor. EE was taken to a nearby medical center where EE was treated for lower back muscle strain.
Employee was shoveling rock and felt a pain in the abdomen. Incident reported 02/04/2019.
Employee stated got thumb nail caught on a piece of the structure.

Employee was parked on a small grade. The drill was raised on jacks. He exited the drill and had his arm resting on the door frame of the drill. The door closed, striking the employee on the arm.
The employee bent to pick up the guard to replace on tail pulley after cleaning. When he lifted the guard from the ground he strained his back.
Employee states that while lifting hydralic motor from a tool box, he experienced pain in right side. Has not seen the doctor yet, so severity of injury is unknown at this point.

EE WAS ATTEMPTING TO REMOVE A 16' DRILL STEEL WHICH WAS STUCK IN THE HEADING WHICH RESULTED IN LOWER BACK LUMBAR STRAIN. ORIG DR REPORT STATED EE WAS TO RETURN TO WORK 8-13-01 ON 8-15-01. HE R ETURNED TO DR AND WAS TOLD TO BE OFF WORK UNTIL 8-22-01.
EMPLOYEE STRAINED RIGHT ELBOW WHILE LIFTING 50LBBAGS OF NITRATE ON POWDER MONKEY. EMPLOYEE WILL BE OFF WORK UNTIL 11/30/00.
EMPLOYEE WAS REMOVING ROCKS FROM ROADWAY BY HAND, EMPLOYEE STATED THAT HE FELT A POP COMING FROMHIS SHOULDER AREA, EMPLOYEE WAS COMPLAINING OF INJURY, WAS SENT TO CARROL COUNTY HOSPITAL FOR TREATMENT. DOCTOR REQUESTED LIGHT DUTY FOR 1 WEEK.
