Night shift employee was lifting a metal plate out of the third level dump hopper. As EE was attempting to pick the plate up EE felt a sharp pain in the lower back. EE then reported the incident to the supervisor. EE was taken to a nearby medical center where EE was treated for lower back muscle strain.

EMPLOYEE WAS MARKING OFF FACE HEADING N-16 FOR DRILL HOLES TO BE DRILLED. ROCK FELL FROM FACE AREA AND STRUCK HIM IN THE RIGHT SIDE OF HEAD.
