Employee was adding hydraulic fluid to the loader when they fell, striking the tire with their right arm.

Employee was assisting in dropping a transmission into a 993K loader. They bent over to move some hydraulic hoses out of the way and felt a sharp pain in the left arm and shoulder. However, the employee stated they had recurring pain in this area for over a week and could not pinpoint this injury to this particular instance. This is still under investigation.

Employee was moving scrap metal, one end slipped pinching thumb between 2 pieces of metal.

Pulling miner cable slack to be hung & strained lower back.

Employee struck his head against a water valve while shutting off the drain water valve on water truck. Employee stated his hard hat was knocked off. Wounded required staples for closure.

EE picked up one side of the top roller while another ee was lifting the other side and when they turned to install the top roller in the rail, ee felt a sharp pain in his left calf muscle. EE continued to work in his normal occupation until 6/15/2015 and then started missing time.

While moving the section the employee bent over to pull on the miner cable and felt a sharp pain in his lower back. He went to the doctor and found out that he had a pulled muscle.

Employee was working on the Gizmo engine in the refuse area. The wind was blowing which caused dirt/dust to enter into his left eye. He was wearing Rx safety glasses. The employee sought medical treatment at SRMC ER for irritation. The eye was irrigated and the employee was given an antibiotic ointment.

The employee was mounting the 16G motor grader, equipment #301. When the employee go to the 4th step from the ground, he experienced pain in his lower back. The employee finished this shift, but was unable to work his next 2 scheduled shifts.

EE & co-worker was installing a new head shaft in the #1 buggy in the last open break of the power entry. While splining of the head shaft, the item fell out of place and dropped approximately 2 1/2 feet striking EE in the top portion of his left foot.

Employee was spot bolting 10 foot cable bolts. One piece of steel was in the hole when he was pushing a piece of steel up to connect it together he got his middle finger between the steel and the top cutting the end of his finger off.

While gripping oil filter to spin loose he felt hand pop. then a few minutes after as he climbed up to clean mirrors it popped again.

An employee attempted to lift a full sample bag of coal into the basket of the electric hoist with his left hand. He felt a burning sensation and pain betweeen his shoulder and neck. On Friday, June 21st; this said employee received medical treatment as he was given prescriptions for pain medication and a muscle relaxer for a strained trapezoid muscle.

Employee was assisting in the transportation of an injured co-worker. The responders had positioned the injured co-worker on a back board and placed the back board upon a battery powered mantrip. During transportation the back board shifted and pinched the assisting employees hand between the mantrip and the back board.

EE was lifting a piece of belt structure and felt pain in the left bicep. EE was diagnosed with a torn bicep ligament.

Employee was helping pull miner cable, felt something "pull" in his groin area. *Employee did not report injury to Employer until 4/2/13.

EE was taking down hung miner cable when cable came loose it bent EE's back over suddenly causing a pull in lower back.

Installing final drive gear, gear slipped pinning (r) hand against wheel assembly.

Two employees were replacing bucket on a 488 scoop. Injured employee stated that he was having trouble lining up the hinge pin in the bucket hinge and was using bare finger to find out why there was misalignment. The bucket shifted causing the fingertip to be pinched between the hinges

Employee and co-workers were changing an impeller on a pump. The employee was Pulling on the motor belt when the ring finger left hand was pinched in the motors pulley. Power was locked and tagged out to the pump.

Employee was hanging curtain in a high area when he caught a finger on left hand in a line curtain hanger. Resulting injuries was a laceration which required sutures for closure

The employee and a helper were cutting down wire roof mesh when his are slipped and became caught by a piece of wire resulting in a laceration to his wrist.

Employee was helping to set a beam on cribs when the beam fell off the crib and struck him on the hand.

Employee Strained back while lifting a steel roller

Operator of left bolt machine was cleaning out dumper on dust box on off side when the bolter helper dropped the slab jack causing the flapper on the dumper to close smashing his finger.

Employee was pulling slack in a roof bolting machine cable when a hanger broke loose causing him to lose his balance and fall to the mine floor. He struck his hand against the floor.

Employee was pulling a rock off the side of the scoop caught his finger between the rock and roof.

Employee was in the process of tightening some bolts under cab of machine, bumped head & knocked off hard hat, struck head on a piece of sheet metal causing a laceration to left side of head.

Employee was standing near the waterline when an outby valve was opened allowing flow in the line. The line jumped and the manifold struck him on the leg.

Employee was hanging miner cable with a coworker when he felt a sharp pain in his lower back.

Employee was taking out belt roller and felt something pop in lower back.

Employee was helping miner operator lift miner cable when he felt pain in his low back.

Employee was assisting miner operator hang miner cable when the cable rolled back and hyper-extended the thumb of left hand.

Employee was riding in mantrip trailer. Another employee activated the automatic ventilation doors and then let go of the control switch box. The box swung back slightly and hit the injured employee on the cheek.

Employee inserted a roof bolt into the drill hole. He then pulled the bolt with his left hand to guide it into the drill pot chuck and felt pain in his left side and back. The employee continued to work in his regular capacity until 8/26/2010 at which time he saw a physician and began restricted duty.

Employee had surgery to repair a hernia on 6/18/10. This condition is related to lifting and pulling in low coal. He sought medical attention after moving a piece of coal in the face and experiencing pain.

Employee was treated for torn cartlidge in the right knee 7/16/10. On 6/7/10 while performing normal duties and stepping up on a rail car approximately 8" from the mine floor the employee felt moderate pain in the knee No EVENT to cause damage. Employee suffered from torn knee cartlidge right knee in 2004. Determined during investigation repeated trauma to the knee.

Employee stated he felt pain and a pop in shoulder when he grabbed the bolt with one hand and attempted to twist it.

Employee was helping load a stablizer bar for a 1500 rock truck onto the bumper of the truck, employee got his thumb between the bumper and the bar, resulting in a laceration to the thumb on right hand. Wound required sutures.

EE was loading cinder blocks in scoop and felt a pulled muscle.

EE was plastering stopping and did not use gloves and burned his right wrist.

Managment was notified by the employee on 12/29/09 that he sustained an injury to the lower back on 12/13/09 as he was shoveling frozen dirt to allow for additional room to install a belly pan on a dozer.

Employee was lifting roller from frame of a D-10 dozer when metal from frame cut his middle finger on right hand.

Employee and two coworkers were maneuvering a 4'x8'x1/4" piece of sheet metal into place to form the back of a chute at a belt head. Employee slipped on a turnbuckle, anchored to the tailpiece and lost control of the metal, allowing it to strike his right forearm.

Employee indicated she was on her knees when she threw a crib block and felt pain in her lower back.

Employee was going to pull the miner cable. When he reached down to get hold of the cable, his left thumb was cut on a discarded razor knife blade. Cut required 3 stiches.

The employee complained of back pain after lifting and turning and doing normal work of a scoop operator on March 3. He continued to work through April 27th without incident. He began missing work on April 28th due to back pain. He has a history of back problems. This is an occupational illness from repetitive motion trauma.

The employee received a laceration to the right hand requiring sutures for closure while removing ice from return belt rollers on the raw coal belt. Employee returned to normal duties on his next scheduled shift.

Employee felt pain in his back after bending and pulling on a small loop of miner cable, a normal work duty. He began working in a restricted duty capacity 01/10/09 and is working in a restricted duty capacity.
