Poplar Ridge No 1 Deep Mine Coal

Had a roof fall in the old works on 2nd West Mains in the # 1 Entry at spad # 71542, the fall was approximately 8 ft. high 25 ft. wide, and 20 ft. in length. The area was dangered off and used timbers, prop setters for additional support, and moved stopping line over to make travelway.

Employee was getting out of coal truck to change a flat tire and as he was exiting the truck he slipped & fell on his left side catching himself with his left arm & dislocating his left elbow & fracturing his left wrist.

Employee was driving a mantrip from the outside to the section, when a rock fell from the top and struck in the head / shoulder area knocking his hard hat and glasses off of him. He was sent by Ambulance to SMH and was granted a full release. He informed Mine Management that due to the injury he received on 10/21/14 that he would be going off work on 3/16/15.

Employee was in the process of making a belt splice when the utility knife he was using slipped and lacerated the index and middle finger on his left hand this causing him to receive stitches on both fingers.

RBO was bolting in the face when he went outby to retrieve some additional curtain, while in the process of pulling up the curtain he stepped on a small rock and twisted his Left Ankle.

Employee was assisting in the transportation of an injured co-worker. The responders had positioned the injured co-worker on a back board and placed the back board upon a battery powered mantrip. During transportation the back board shifted and pinched the assisting employees hand between the mantrip and the back board.

Employee was tightening a bolt on the rear end of a scoop using a 3/4 inch drive socket when he felt a "pop" in his back.

Employee ran his hand across a belt splice he and co-worker was working on. Employees was checking condition of splice when co-worker hammered again striking the employees middle finger left hand. Mines were idle from Nov 18 - 25.

Employee was operating a roofbolter and slipped on a lump of coal tired to catch himself grabbing the roof bolter tray causing him to strain his shoulder. Employee sought medical attention and was released with no days lost or restricted. On 11/27/12 employee sought additional medical treatment.

Wrench slipped while tightening wheel. Driver fell and twisted his ankle fracturing it.

Employee was using a pick hammer to remove a bolt for the cable stand off on the continuous miner when he struck his left hand with the pick hammer. On Oct 3, 2012 Management learned that the employee sought medical treatment and received sutures.

Employee was hanging curtain in a high area when he caught a finger on left hand in a line curtain hanger. Resulting injuries was a laceration which required sutures for closure

Operator of left bolt machine was cleaning out dumper on dust box on off side when the bolter helper dropped the slab jack causing the flapper on the dumper to close smashing his finger.

Roof falls were discovered in the 8 R panels SE Mains 10,000' from the working section. Failure occurred due to laminations above the anchorage zone of 5' torque tension roof bolts. The are was developed 3/11/11. ventilation and the transportation of miners' were not affected. No injuries occurred as a result of the fall. #8 Entry 20 Break 2 breaks outby 66423, 50'LX19'WX8'H.

A roof fall 7'L x 19'W x 8'H was discovered outby spad 66570 #4 entry crosswise to the #6 entry. The fall was caused by failure above the anchorage zone of the 5' torque tension roof bolts. The area was developed 4/6/2011 and was 10,000 feet from the face area. Ventilation and the transportation of miners' were not affected. No injuries occurred.

Employee was crossing belt when he slipped and fell on the belt. Employee was transported on moving belt 1500' when he stopped the belt. Received a fracture to the L2 lumbar vertebra. Mine was idle from 12/22/11- 01/03/12

IEE was standing outside his truck talking to other drivers. He was waiting on coal. A car driven by another EE came around the small curb said sun was in his eyes and hit my driver causing injuries to his front teeth, lower lip, his right hand and knees.

Employee was operating a scoop in an entry in close proximity of line curtain. Attempting not to tear the curtain, the Employee reached out to push the curtain back. While doing so the employee's right hand was pinched between an off set in the rib and the scoop canopy. Resulted in crushing injuries to the employee's fingers. The middle finger right hand was later amputated.

A roof fall measuring 30 long, 18 wide, 6 thick was discovered on the SE Mains 1 crosscut inby spad 66710 #5 entry due to coal and rock streaks above the anchorage zone. The fall was 4,400 from the section, mine operations, ventilation, and the passage of men was not affected. The area was developed with 5 torque tension roof bolts was the primary support.

Employee was helping miner operator lift miner cable when he felt pain in his low back.

A roof fall measuring 70'L x 19'W x 8'thick was discovered during an exam in the SE Mains along #5 belt, No. 3 entry, inby survey spad 66639. The area was developed 4/5/2011 with 60" torque tension roof bolts as the primary support. The fall occurred due to laminated coal and rock in the roof. Ventilation and the movement of machines were not affected. No injuries occurred.

Employee was assisting miner operator hang miner cable when the cable rolled back and hyper-extended the thumb of left hand.

A roof fall was found during a weekly exam in the 1st Left panel off south east mains 3200' outby the 001 section. The fall measured 420' wide, 19' wide, and 8' thick. The fall occurred above 6' torque tension roof bolts due to coal and shale laminations. The area was developed 10/31/09. No injuries occurred. Ventilation, passage of men, and use of equipment was not impeded.

A roof fall measuring 70' Long, 18' wide, and 9' thick was discovered in the #2 entry 7 right off SE Mains at spad 66193. The fall was 500' from the working face, ventilation or the passage of men was not affected. The fall occurred due to laminated rock above the 60" torque tension roof bolts. The area was developed Feb 2011 no injuries resulted from the fall.

Was injured when the railroad jack he was using to place mantrip back on the rail skipped gears causing the hand to drop quickly. Employee suffered a fracture to the thumb right hand.

Was lapping belt when EE stepped into a hole twisting his right knee.

Employee was treated for torn cartlidge in the right knee 7/16/10. On 6/7/10 while performing normal duties and stepping up on a rail car approximately 8" from the mine floor the employee felt moderate pain in the knee No EVENT to cause damage. Employee suffered from torn knee cartlidge right knee in 2004. Determined during investigation repeated trauma to the knee.

Employee stated he felt pain and a pop in shoulder when he grabbed the bolt with one hand and attempted to twist it.

On 4/28/2010 management was notified by MSHA that EE MIIN M60820349 had exersiced rights under 30CFR Part 90 for pneumoconiosis.

During an examination a roof fall measuring 20' L, 20' W, and 5' thick in the intake entries 4000' from the working section. The fall was located in the #9 entry inby spad 58227 along #3 belt which was developed Aug 07. The type of roof support used was a 5' glue bolt. Fall did not impede travel or ventilation and occurred due to laminated rock and coal in the immediate roof.

Employee was hitting ratchet with sledge hammer to break bolts loose, ratchet sprung back causing hammer to hit him in the face on right side of his nose. After further examination it was determined on Nov 20 that the nose was fractured.

Driver fell asleep, steering off to the left of the roadway. Got too close to a sump hole, the edge gave way causing to truck to partially overturn. The truck was going uphill loaded. The driver suffered bruising & stiff neck.

A fall measuring 25' long, 30' wide, and 8' high was discovered 3500' outby the working place during a routine exam. The fall was located in the return entry of the 001 section 25' left of spad 58814. Affected area was developed Dec 07 with 5' torque tension roof bolts as the primary roof support. Fall occurred due to laminated coal and rock above the anchorage zone.

EE started down hill at 19 marker towrd Haney Bro. Tk. Lot loaded. EE tried to shift to 3rd gear. EE said truck went into gear and immediatley came back out. EE set park brake, hand brake and pushed foot brake. Truck wouldn't stop, causing it to get out of control, then struck another truck and over turned. EE said brakes wouldn't stop the truck.

The miner operator was tramming the miner to a new cut and as the miner crossed over a dip in the mine floor, it slid and the boom contacted the operator's right leg. The miner operator's extent of injuries was a puncture wound caused by the miner boom. Sutures to close the wound was applied.

Employee recieved a laceration, requiring sutures, while using a utility knife to cut conveyor belt.

A roof fall measuring, 80' L, 19' W, and 8' thick was found in the crosscut between 3&4 entries on 4 belt spad 58667. The area developed Oct 07 was 5500' from the working place, it affected no persons. It occurred due to coal and rock streaks above the 5' torque tension roof bolts.

Employee felt pain in his back after bending and pulling on a small loop of miner cable, a normal work duty. He began working in a restricted duty capacity 01/10/09 and is working in a restricted duty capacity.

Employee was installing a 6' glue bolt at a surface crack and misjudged the elevation of the bolter boom for a 5' bolt. The boom needed to be lowered 1' and while doing this the operator lowered the boom on the right foot. The employee continued working until 01/09/09 when he sought medical attention. Work continues in a restricted duty capacity.

Driver got out of truck to tighten lug nut when his hard hat fell off and rolled under truck. When he stood up, his head hit the truck bed before he could put his hat back on.

EE was tightening up tire, when wheel wedge broke causing EE to fracture his arm at an old break site. EE did not know arm was broke until he went to ER at 1730 7/14/08.

A roof fall measuring 30' length, 20' wide, and 7' thick was discovered in the return entry 1500' outby the working face. The area was developed Nov 07 with 5' torque tension roof bolts. The fall occurred due to shale mixed with coal streaks above the anchorage zone. Fall location was in # 2 entry on 1st left panel near spad 58748.

A roof fall measuring 50' length, 20' wide, and 6' thick was discovered in the return entry 1500' outby the working face. The area was initially developed Nov 07 with 5' torque tension roof bolts used. The fall occurred due to shale mixed with coal streaks above the anchorage zone. Fall location was #6 entry 1st left panel.

A roof fall 60'L 18'W and 6'H was discovered during an exam. It was 1500' from an active section, did not impeade travel, ventilation, & no injuries occurred. The fall was in the old 1st left panel off the west mains # 6 entry inby spad 58744 and was developed Nov 07. 5' torque tension bolts were used. Laminated shale and rock above the anchorage zone was a cause

Employee was struck by falling roof rock (shale) while operating the continuous miner. The rock fell from between permanent supports causing a laceration to the index finger right hand.
