Employee was pulling wire through conduit and injured right shoulder and bicep. The injury on May 6, and employee reported it but did not get it checked until May 15, and they sent them for Xrays and they had surgery on July 3, 2024.

The injury occurred when adjusting the impeller clearance on a slurry pump. While checking the rotation of the slurry pump shaft, the left thumb of the EE was caught between the v-belt and motor sheave.

Employee had just cleaned out from under tailgate of stockpile truck. Employee started to step up on a concrete platform and placed his right hand on tailgate gate latch. It closed pinching right hand.
Employee was replacing a guard that had came loose and while he was attempting to life guard, it slipped and caught end of left ring finger.
