IE was assisting in taking a bad piece of cable out of the slope hoist cable. They were picking the cable up to move the cable over. As the IE raised up from picking cable up, IE felt a pain in the lower to middle back. IE did not go to the Dr until 3/16/19 & Dr could only find a possible strained muscle in IE's back. IE started missing on 3/18/19.

Mostrando 50 de 147IE was carrying old belt structure out of the slope drive house to get rid of it from where they had stacked it on the surface after changing it out for new structure in the slope. EE and another employee lifted the structure & ee felt pain in lower back & left buttock. All that they could find wrong at the Dr. was possibly a pulled muscle.
IE was throwing roof bolter cable over against the rib so it would be out of the way for haulage an the MN 5 section. As IE stepped forward IE felt a pain in the left knee.
IE was assisting in the building of an overcast. As the crew lifted an I-beam IE felt a pain in the right shoulder. IE continued to work until 1/30/19 and that is when they started missed work due to this incident that happened on 10/21/2018.
IE was helping on longwall recovery. IE had been sitting on the mine floor taking hoses off of shields. The material in the J-Cribs had mixed with the mud on the mine floor causing burns on IE's buttocks & back of legs. IE started missing on 5/23/2018.

Crew was pulling gate box cable slack on the trollies. The cable dog got caught & broke. When the cables came down it hit the I.E. in the right ankle causing a small cut which required 3 stitches. I.E. did not miss any work due to this incident.
EE was the MN2 take-up shoveling and washing with a hose. EE was shoveling a large amount of gob and when lifting it to the belt, felt a sharp pain to lower back. EE continued to shovel and stated aggravated back even more.
EE was removing a slider on the 18 east section when EE felt a strain to lower back. EE proceeded to go to get a grease gun to grease the miner, and upon returning the grease gun, EE's back pain became more intense. EE informed the section foreman that EE needed to seek medical help for back strain.
EE was lifting a bottom roller in the longwall belt entry from the muck when ee felt a strain in lower back. EE was taken to Brookwood medical center for assessment and released.
Bottom of the dump, Muck area. Employee was cleaning the area when accumulations returned back down the belt and employee had water and muck up to the waist. When trying to remove themselves from the muck, they felt a strain in their lower back and right ankle.
EE was unloading roof bolt plates and roof bolts from the cage when they felt a sharp pain in their hand and swelling about 30 minutes later.
Crew were loading I-beams across the belt line. EE attempted to lift the beam by self to place on the belt line.
EE was attempting to move a 2 inch air pump from 59 cross-cut on the belt line to the sump.EE thought the air line and depressurized and attempted to move out from the belt area. When moving the pump, the air line came off of the pump, which caused EE to lose balance and fall into the old structure and rollers, causing a sprain to right wrist and right knee.
Employee was loading a bottom roller onto #2 man bus, while setting a roller down it slipped out of employee's right hand smashing left little finger between the roller and man bus.
While replacing filter sector, the sector slide back from its resting spot. Employee attempted to grab it but while doing so felt a pull and pop in right shoulder.
While changing scoop batteries an attempt resulted in the batteries positioned improperly on the forks of the scoop. The operator lifted the batteries and prepared to back against the rib. The injured employee was standing at the back of the scoop. EE flagged operator to stop. While getting jumper out of the way batteries slid off end of forks falling onto employees left foot.
EE and the evening belt crew were making a belt splice on MN2. EE was releasing the belt clamps from side of the belt, while the other side had been released, which resulted in left thumb being struck by the belt clamps. EE was taken to the hospital for evaluation, which resulted in gluing of the affected area, along with 2 stitches placed.
Injured was examining fire suppression bottles on the #9 locomotive when the injured went to the lower the lid, the lid fell and struck the right thumb, resulting in a laceration requiring stitches.

Injured was helping to position a shear jack onto a continuous miner on the East Main section. The jack need to be re-positioned when the injured's hand was caught between the jack and the conveyor chain. 6 stitches were needed in the right middle finger.
Employee was helping to move 2 shield ram jacks from the top of the stageloader onto the cable tray of the panline. The employee was guiding said jacks when the jacks left the stageloader fully onto panline, one slide off and caught employees leg between the jack and a pontoon, causing 3 breaks to the right leg.
Injured was washing with a water hose under the belt when the hose got away from the employee, striking the employee several times, resulting in a cut, requiring stitches, to the left leg.
Injured was assisting the equipment operator in installing the forks back on the lo-trac when the forks fell off striking the injured on top of the left foot resulting in a broken toe. The break was not diagnosed until 12/1/15. The injured was never place on restriction or had any lost time.
Injured was shoveling on the Main North 2 belt when a pain was felt in the right shoulder. Injured had surgery on 7/28/15 after continued discomfort and a diagnosis of a torn rotator cuff. Injured had no restrictions until 7/28/15.
injured was helping to change scoop battery. the scoop placed the forks under the battery and lifted it up and moved backward. the battery pulled off the forks. as the battery was raised again injured placed his hand under the battery to remove a concrete block the block crushed out allowing the battery to set down on injured right thumb.
Employee was installing a roof bolt, reached for a drill steel and right ear made contact with a previously installed roof bolt plate and suffered a laceration to the ear lobe, requiring stitches.
Injured was plastering a stopping when plaster splashed back and got onto forehead. The plaster then rolled down the injured face into left eye.

On 5/28/14 addition medical treatment was requested and provided, revealing an undiagnosed injury. Surgery was performed on 6/4/14. The first day of lost work was 6/4/14.

Employee was replacing a fire valve. When preparing to shut off the main valve a 1 1/2 hose blew off and a ball valve came apart striking the employee in the left shoulder. This caused a dislocation.
Injured was changing screen panel in vibrating screen. The vibrating screen was de-energized and locked out. As injured exited the screen deck the injured pulled on the drive mechanism vee belt. This caused the sheave wheel to rotate pulling his right hand between the belt and sheave wheel resulting in a laceration to the palm and fracture of the 5th finger .
Employee was setting up a water jack. While a shield was put up the pin from the shield shot out and hit the employee in the left leg.
Employee was putting water prop in set pressure. When the shear went by the water prop flicked out and hit the employee in the back.
