EE was walking to the work area when he stepped in a hole of water. He worked with wet socks & boots, this may have caused an abrasion on his foot. Mgt. wasn't notified until 9-25-2014 of this event. EE has a med. Cond. (diabetes) that may contributed to this alleged event. Mgt. is protesting the validity of the event with workers comp.

Employee was installing bolts in the #6 entry when one of the bolts became bent. the wrench came loose from the bolt, the bent bolt to struck his (L) hand causing a fracture and laceration to his (L) middle finger.

Employee states that while carrying a ladder and dragging a piece of ventilation curtain across the section he felt a pain in (L) shoulder and neck. This incident remains under investigation.
Employee was using a bucket to scoop up mud and water. As he turned to pass off the bucket, he felt something in his lower back pop and had pain go down his right leg. This incident remains under investigation.

It was determined thru a Part 50 audit the operator, 4608763 was required to also report this incident. Citation #7166777
Employee was carrying a ladder to hang curtin. he states that he felt something pull in his groin area. Empoyee continues to work. He was diagnosed 02/26/2013 with a hernia. He was then placed on modified duty.
Employee was working around the C1 belt head, as he bent over to pick up a rock he felt a pain in his lower back. Employee continued to work until 02/01/2013 when he went to the Doctor. He was then placed off work.
Employe was lifting on a piece of metal on a tail piece when he felt 3 places in his groin area start to burn. Employee continued to work until 03-25-2013 when he had surgery to repair the hernia, he then began to miss work.
While loading scoop with supplies, a band on a bundle of roof straps flipped up and cut his right forearm.

At the end of the shift on 2/18/12, employee states the ride to outside derailed, he helped lift the ride back onto the rail and strained his back. He continued working until 2/22/12 and went to the doctor....He was off 2 days and returned to work 2/25/12.
Employee states that while putting up rib boards, the bolt spacer fell from the T-Bar striking him in the right shoulder and neck. He also states that it knocked him to the ground.

EE states that while he was hanging mine phone at the old "C" section power center, as he was attempting to climb up on the power center his left knee hyper extended. EE finished his shift. Accident was not reported to mgmt. until 12/17/09. EE did not notify mgmt. of medical findings until 12/31/09.
EE was plastering on stopping he stated he threw a hand full of plaster on stopping & rib and it splashed black in his eye, he also stated his glasses were scratched up and he couldn't see to plaster, so he took them off.
EE states while installing a head shaft on the #2 shuttle car, the head shaft inadvertently came down and caused his thumb to be caught between the frame of the shuttle car and the head shaft.
Employee states that he had bolts made up and staged along the rib. The bolts slid off the rib and the corner of the pizza pan struck his left forearm causing a 1" laceration.
Injured was reaching to get a bottom roller, while on move crew, was pulling back and a top roller fell, striking him in the right side of back. He finished the shift, took the next day off, returned to work on Friday then went to the dr after work on Friday.

On 8/23/07 @ 3:40PM EE alleges that an injury occurred Monday 8/20/07 but did not report it to his supervisor. He states that he smashed his hand with a crib block but didn't think he was hurt until after work. He said that his hand started to swell and he went to Logan Regional ER about 6 o'clock & was seen about 8 or 9 o'clock. I contacted his work site & there were no witness.
