Employee was moving a shuttle car cable to the rib and as they threw it they felt a pain in right elbow. Employee was seen the following day by a physician and was taken off work as they were unable to perform their normal duties.

Employee was loading glue onto the back of EE's bolter. When EE turned EE felt pain in EE's back. EE treated at the ER where EE was taken off work for back pain.

The employee picked up a sheet of 4x8 plywood when the wind caught it. Trying to hold on to the sheet it twisted causing the employee to tear the right bicep. Started missing work on 2/12/2026.

The Mechanic was assisting in changing the cats on a miner. While the mechanic was picking the cats up to pull slack towards the back of the miner it pinched the left pinky finger in the hinge point of the cats causing a laceration and receiving stitches. NO work was missed.

Employee was raising a light bar with a crank handle to illuminate a dump site on the refuge pile when they felt pain in left shoulder. Employee started missing work on 1/15/2026.

When hanging a cable in the belt line during a power move, the employee felt a pop in left shoulder causing pain in an area that had been repaired before. They continued to work. Started missing work on 3/3/2026.

Employee was preparing a cable bolt for installation and as EE reached across EE's body to grab a pin plate, EE drug the plate across EE's right bicep causing a laceration requiring sutures. EE was treated at the ER and given a full release with no restrictions to return EE's following shift.

Employee was lifting a diesel particulate filter when EE felt pain in EE's right shoulder. A later MRI revealed a tear in EE's rotator cuff. EE continued to work until 1/14/25 when EE had surgery to repair the tear and was taken off work as EE was unable to perform EE's job without restriction.

While lifting the framing at a header with right arm the belt foreman felt a burning sensation in right shoulder. Worked until surgery date. Started missing on 03/25/2026.

Employee was drilling a hole when steels became stuck. As they pulled them apart, one struck them in the nose. Employee was seen at the ER where x-rays showed a fracture. Employee was given a full release and continued working until 11/12/25 when seen again and given restrictions preventing them from performing their normal job.

Employee was using their foot to slide boards under the tongue of an RA and when they put their weight on their right leg, they felt their right knee pop. Continued working until 12/1/25 when taken off for surgery to repair their right knee.

The miner operator used EE's index finger to rub EE's right eye causing dirt from EE's glove to enter and scratch the eyeball. Seen the doctor on 10/15/2025 where medication eye drops were prescribed and no work was missed.

Employee was operating a grease gun injecting grease into a fitting on the continuous miner to tighten the cats. The connection was stuck, while prying on the connection, it popped off and caused a laceration to EE's forearm. EE received sutures to EE's forearm, was given a full release without restrictions and performed EE's normal occupation.

while hanging the miner cable and water line that goes to the miner, the shuttle car operator strained EE's left shoulder. Started missing work on 10/03/2025 and returned to work on 10/08/2025.

Employee was helping load some belt framing on the 3/60N beltline. As EE was dragging a 60 inch top roller under some water line, EE felt pain in EE's lower middle back and down EE's left leg. Employee started physical therapy on 9/22/25.

The mechanic was pulling an old piece of high voltage into a crosscut and pulled EE's right bicep. The mechanic continued to work until surgery was needed to repair. Started missing work on 10/22/2025.

Employee was plastering a brattice when some plaster got in EE's right eye. Employee was seen by an Eye doctor and was given a prescription eye drop for a scratch on EE's right eye.

Employee was rock dusting on the belt line and reached up to steady themselves on a piece of corrugated metal and their hand slid across it causing a laceration to left hand. EE was treated at the ER where they received four sutures and a full release to return for their next scheduled shift.

Employee was digging through the tool box on EE's diesel ride when the heavy steel tool box lid fell smashing the three middle fingers of EE's right hand. EE finished EE's shift then went to the mine nurse where EE received sutures in EE's right ring finger and middle finger due to the lacerations. EE returned to work at EE's normal job without restriction.

Employee was throwing loops of feeder cable up against a rib and strained EE's right shoulder. Started missing work on 09/11/2025.

Employee was drilling a hole to install a pin when the steel became stuck. As they went to pull the steel out they felt a pain in their right shoulder. They continued to work their normal job until they were taken off work beginning 9/5/25 as they were unable to perform their normal job.

Employee was lifting a man door for a brattice when they felt a sharp pain in lower back. They finished the shift, but when they awoke the next day, they were having lower back pain. EE was seen by a physician on 7/31/25 and was taken off work as they were unable to perform their normal job.

The rock duster was dusting the belt line when EE had carried the dust hose back to the duster, EE felt sharp pain in EE's back. Started missing work on 07/22/2025.

Employee was opening a set of air lock doors when they felt their knee pop. Employee was seen by a doctor the same day and was taken off work beginning 7/22/25 as they were unable to perform their normal job.

The employee was loading miner cable into the bed of a miner when EE felt pain in EE's right shoulder. No work was missed, continued to have pain, started physical therapy on 09/04/2025.

The mechanic and miner operator picked up the rub rail for the miner to access the panel. The mechanic felt a sharp pain in abdomen. The mechanic started missing work on 7/30/2025 for hernia repair.

Employee was discharging a fire suppression hose to install on a new header when they lost control causing the hose to come back striking them in the groin. They had surgery the same day and began missing work on 7/8/25 as they were unable to perform their regular duties.

Employee was climbing an extension ladder and felt a pain in EE's right ankle.

While raising the lids on a battery scoop the operator strained their groin. The employee started missing work on 7/22/2025 for a hernia.

Reclaiming belt rollers, the outby worker was stacking rollers onto the ground when they caught their right ring finger between two causing a small cut. Two stitches in the end of the finger and no work missed.

While installing a chain guard on a belt drive. The left middle finger of the belt mechanic was caught between the two chain guard handles. This was a two person install, when positioning the guard, it slid into the guard handle next to it. Antibiotics where given No work was missed.

On 5/13/2025 employee was standing a bundle of mesh up to lean it against the rib when EE felt a pop in EE's left shoulder. Employee continued to work without restrictions performing EE's normal task, and was seen by a doctor and was taken off work on 5/21/2025 as EE would not be able to perform EE's normal work duties

While cleaning the scalping screens on the 3rd floor there was some gob that broke loose in the chute just above and hit the utility man in the top of the head. Continued to work until 12/11/2025 when surgery was required to repair EE's neck.

While picking up a trailer hitch to install on a tractor the employee strained their lower back. Started missing work on 04/29/2025.

Employee was cleaning out around a clamp on a waterline and cut EE's right index finger on a sharp rock. Glued the finger and no work was missed.

Employee was drilling a hole for a roof bolt. EE was reaching for a second steel and when EE turned back around the first steel fell out of the mine roof and bounced off the drill pot knocking EE's glasses off causing the steel to hit EE on the left eye. EE was seen by a doctor and was given eye drops and a protective contact lens.

Roof bolter strained EE's lower back loading pin boards onto the bolter. Started missing work on 02/25/2025.

Employee was loading cable bolts on the bolter when EE felt a pain in EE's abdomen and noticed a knot on EE's navel. EE was diagnosed with a hernia on 01/24/2025 and was taken off work as EE was unable to perform EE's normal work duties.

Employee was throwing crib blocks from a concrete sump when EE smashed left ring finger between the crib block and concrete wall busting finger open. EE received four sutures and was given a full release. EE returned for EE's next scheduled shift on 12/20/24.

Employee was installing a 6" water coupling. The coupling dropped and they attempted to catch it and the edge caused a laceration to their left index finger resulting in seven sutures.

Employee was removing a chain boomer from a supply car when the handle of the boomer came back striking EE's right hand. EE finished EE's shift and then went to the mine nurse where EE received four sutures in the fourth finger of EE's right hand. EE returned to work EE's following shift.

The employee was cleaning rocks off of the top of the miner when they felt a strain in their lower back on the right side. The employee started physical therapy on 11/04/2024. No work was missed.

The employee was loading the pin tray with roof bolts and felt a sharp pain in lower back. Started missing work on 10/08/2024

The employee had reached across body to pick up a wheel and tire, as employee picked it up to install it onto the golf cart employee's left shoulder popped. Started missing work on 10/04/2024.

Employee was helping put a spool of leaky feeder cable on a spool cart and pinched right index finger between the spool bar and cradle. Employee received 4 sutures.

The employee was changing a 72" bottom roller, when the employee picked the roller up to put it in the stand employee felt a burning pain in lower back. The employee was taken off work on 8/21/2024.

The employee was loading pin plates onto the bolter. Employee swung around to place some more plates on the bolter and hit right elbow on the plates causing a laceration receiving eight sutures.

Employee was pulling a yield prop across the belt when it dropped of the belt and onto the belt framing causing a laceration to EE's left middle finger requiring sutures.

Employee was picking up crib block out of a scoop bucket and strained lower back.

Miner was straightening a bent roof bolt with miner's hands, as miner was trying to straighten it the bolt slipped in the miner's hands causing a laceration to the side of the right hand requiring 5 stitches. Miner was injured on 05/09/2024.
