The EE was in the bagger area stacking one load of 50lb bags on pallets. As they lifted their arm, they felt their right elbow go numb. Later in the day the EE started to feel pain in their right shoulder.

Employee was stretch wrapping a supersack when it became unbalanced and tipped over pinning employee between supersack and crossbeam-railing.
In mid-March employee was raising the roll up garage door when the door roll caught on the roll cover causing the chain to stop moving. The employee indicated that the left middle finger flexed when the chain stopped and was sore. On May 19,2022 employee indicated that the finger was still sore and was seen at doctors office.
EE suffered a small cut on the right index finger while removing a bearing from a shaft on 02/22/22. On 03/31/22 employee reported some swelling and went the Doctor where a small sliver of metal was removed requiring 2 stitches and prescribed antibiotics.
Employee was entering the mill through the lab door. As employee exited, EE pushed on the door glass a little harder than EE should have causing the door glass to break. Employee was seen at ER and received seven stitches for two small cuts on the right hand.
Employee was removing hard hat to put on a safety harness. Employee caught the side of left eye with the hard hat's visor shield causing a scratch to eye.
Employee cut his right index finger while placing a bag on the four spout bag packer. While initially believed to require only first aid the day of the incident, to ensure no infection occurred, a physician prescribed antibiotic the following day.
Employee was stretch wrapping and he pulled the wrap across his body to fasten it to the corner of palletized material when he felt a sharp pain in his left shoulder.
The employee was replacing the water trough at the end of #4 Hardinge mill when a piece of debris entered his right eye.
Employee was assisting in the removal of a steel plate from the floor beneath the crusher. The plate was dropped, striking the employee on the right leg.
The employee was emptying barrels that fill up during production and employee strained lower right back. Employee decided to wait and see if it would get better on its own. The injury did not get better so employee made an appointment with Dr. on 1/31/2013
Employee and another electrician were bench testing an electriclly operated valve. As the unit operated, his right index finger was pinched by the actuating arm. A flap of skin and flesh was torn loose from his finger. Employee was seen at the Emergency Room and required 8 stitches to repair the finger. Employee returned to work-No Lost Time.
EE was measuring timber and the top timber fell on his ankle.
Employee said he was picking up broken 50lb bags of talc from the floor and dumping them into carts to be disposed of. While shaking out a bag in the cart, he he said he felt pain in his lower back. Employee was seen by Doctor on 2-23-09. Employee started losing time 2-24-09.
Employee was climbing over transport pipelines on roof to check operation of a diverter valve electric motor. Employee claims that later in the day he began to feel tightness and pain in the middle of his back. He saw Dr. (Chiropractor) on 1-23-2008
Employee was helping H.D.Mechanic change bucket teeth on an excavator. He was holding the drive pin when his right thumb was pinched between the drive pin and the retainer pin. The thumb nail was torn off. He continued to work until lunch time. Later in the evening he was seen at the emer. room for x-ray ( not broken ) and dressing. He returned to work on his next scheduled shift.
Employee was helping disassemble a machine frame work. When he picked up a piece of the frame it slipped and pinched the index,middle & ring fingers of his right hand. He saw the Doctor and had a xray taken on 7/24/07. He returned to work 7/25/07 and also saw the Doctor to have pressure relieved from under the nail on his ring finger. There has been no lost time.
Employee and a partner were closing a feed valve on a talc pump they were working on. Injured employee's foot was stuck in material on floor and when he turned to move he twisted his Knee. On 4-18-2007 he went to his Doctor and is scheduled for more tests.
While cleaning area employee was lifting a Barrel to dump waste and felt a small pain in his Back. Employee continued to work until 3-1-07 and Dr took him off work for at least 1 week.
On 1-25-07 employee reported feeling pain in his lower stomach area. He was seen by a Doctor on 1-26-07 and a hernia was found. The employee has not lost any work and has not had any treatment yet.** Employee missing time as of 4/20/07 for Surgery**
Employee was lowering the top portion of a high jacker (man lift). He removed the 2 pins to pivot the top down and when it came down suddenly he said it pinched his finger between the cage and frame work. Employee has seen a doctor and will have fluid removed. (2/18/08 appointment) Amendment: EE had fluid & cyst removed. He lost one day (5/5/08); resigned for other employment.
