When changing screens, employee grabbed a screen to pull it out of the bottom deck. The screen was frayed and 3 thin pieces of metal punctured through EE's gloves and into EE's fingers.

Mostrando los 31Employee was working on changing screens and splicing a conveyor belt at a wash plant, while operating a drill the employee felt a discomfort in their stomach. This led to employee seeking medical attention and was diagnosed with an abdominal hernia.
Miner was reinstalling a suction pipe on the ladder of the dredge, miner went to place bolt on the underside when sand fell into miner's right eye.
Miner had been assisting changing a screen and patching a belt. Felt pressure in lower stomach area that afternoon that turned into discomfort later in the evening. Reported the following morning asking to have a doctor check it out. After initial visit and scan it was determined to be a fatty hernia. Follow up with surgeon forth coming.
EE was assisting on the ground moving a stacker conveyor. The stacker was being moved with a front end loader. The stacker was moved into position and movement had stopped. EE unhooked chain from front end loader. Stacker started to move, edge of mud flap frame came into contact with right leg of EE causing a laceration which needed stitches.
Two team members were installing a guard on a conveyor after repairs had been made. As one team member was attempting to install bolts into the guard it shifted smashing a finger. Stitches and outpatient surgery were needed to properly repair the finger. No broken bones.
Employee was bending over to pick up a 1-1/2" water hose to wash the loader when they felt a sharp pain in the lower back.
Working on the plant classifier putting the rising current plants back in place, the plate wedge and when it came loose, sprang back catching the employees right thumb between the plate and the tank wall. The thumb required three stitches.
Employee was going to get in the loader started up the stairs forgot to get ink pen turned around and let go of safety gate, bolt on handle hit his left thumb cutting his thumb, required six stitches
Employees were placing the dredge power cord into the roller hangers on the pontoon hangers and the employees gloves were wet and he dropped the power cord on his right wrist. Which got pinched between the power cord and the pipeline.
Employees were removing pipeline off the water from the dredge. Employee was removing bolts from the pipeline flange, after the bolts were removed employee made sure everyone was clear and swung the pipeline behind him and one of the employees walked forward and was struck in the right ankle.
Employee was disconnecting dredge pipe on the shoreline. Employee was standing next to pipe removing the last bolt from the flange. When bolt was removed the pipe shifted and struck the employee in the lower leg causing contusion.
The crew was re-attaching the counterweight to the 390 cat excavator and employee got this left Index finger pinched between the bolt head and the washer.
Employees were loading oil barrels into the box on a pickup and the barrel began to tip and the employee tried to catch the barrel and strained his back.
EE was leaning over the undercarriage attempting to grab his lifting chain while installing the belly pans. EE was leaning directly over the lifting eye. He grabbed the his chain and was backing out away from the undercarriage when the lifting eye caused a contusion to his left chest area.
After performing some routine maintenance, the miner felt some pain in his lower left back. he strained a muscle which inflamed putting pressure on a nerve.
Employee dropped a screen with no shroud on his right leg above his knee resulting in a laceration.
Employee was removing flashing from cross conveyor. He unbolted the holder and put up on screen deck. Caught middle finger between holder and screen box laceration had 8 stitches to close and a prescription for antibiotics.
Employee was filling in a hole with a shovel, felt like something gave away with right elbow - employee later went to install a disconnect box and noticed elbow was sore which resulted in difficulty to lift. Initially hurt on 10/22/12; employee did not find relief; doctor visit on 02/13/13 resulted in recordable from a steroid injection and 2 days of light duty.
Employee was kneeling on the bank while connecting two 8" suction hoses. Employee was pulling the two hoses together so they could fasten the coupling clamp and felt a sharp pain on the lower left side of his back.
Windy conditions; performing task alone
Using expanded metal to make the guard the employee was using his knee to hold the metal in place so he could mark the spot where the self tapping screw was going to go. While marking the spot on the guard the expanded metal slipped off his knee and sliced his left index finger.
The employee was crouched down with his head under the lift arm of a loader putting a rubber seal on the bucket pin after greasing it. When he went to get back up he struck the top of his head on the loader arm.
EE's were putting the crusher back together after changing liners. EE was checking the space on the main shaft when the inner liner shifted and dropped pinching his fingers.
EE was lifting and stacking 50lb. bags of grit when he felt pain in his abdomen.
They were loading cribbing blocks onto the lowboy trailer for transport; one of the blocks had fallen to the ground after it was loaded. The employee went to pick up the block and set it in the skid steer bucket to load it back onto the trailer, thats when he heard a loud pop in his right shoulder
While using a 2" water hose to remove sand accumulations under the transfer conveyer, employee apparently suffered an abdominal hernia.
Employee was struck on the forehead by a board he was using to straighten out a piece of sheet metal.
EE was replacing impeller & pumpshell on dredge. Was turning impeller by hand to reach bolts and cut back of hand on edge of pumpshell.
Employee was removing a pin and roller from dredge digger chain with help from fellow employee. Once the pin was removed, the chain dropped down, pinching the finger between the chain and the structure.
Caught thumb between screen and screen box causing bruise.
