Employee stated that IE was attempting to load a shuttle car when the car became hooked on the cable hook of the boom. When the shuttle car pulled away from the miner, the hook broke off the miner. IE stated IE picked up the hook to toss it against the rib and was cut by a bur on the hook.
Employee stated that EE was attempting to unlatch EE's seat belt when the seat belt buckle struck EE in the mouth chipping EE's left front tooth. The employee had EE's tooth fixed the next day, but has not missed any scheduled work due to the incident.
Employee stated that when attempting to open the inby set of airlock doors near the section, they attempted to grab the top of the door to keep it from slamming against the rib. this caused them to get a finger caught between the door and a roof bolt in the mine roof.
Employee was attempting to drop the miner cable during a power move when their foot became stuck in the mud. while dropping the cable they twisted and felt a pain in the lower back and left knee.
Employee stated that as EE was attempting to raise the battery lid on the scoop, the lid slipped and caught EE's left ring finger between the frame of the battery and the battery lid. This caused a laceration and a fracture to the finger.
Employee stated that EE was attempting to bend a bolt to fit it into EE's drill mast when EE felt a pain in EE's lower back.

Employee stated that employee was attempting to lift a shield on the continuous miner when the shield slipped and struck employee on employee's left middle finger causing a fracture to the finger.
employee stated that employee was changing a bottom roller on the overland belt. standing in mud employee's foot got stuck in mud employee went to turn and walk and felt something pull in right knee.
Employee stated that on 10/2/2017 EE was attempting to lift a 5 gallon can of hydraulic oil when EE felt a pain in the back. The employee finished the shift on that day and did not report the incident to the supervisor. On 10/3/2017 the employee reported it to the supervisor at approximately 10:25 a.m.
Employee stated that ee was attempting to throw a 1/2" chain onto a scoop when ee felt a pain in left shoulder. Employee did not miss any scheduled work days, but was reported due to modified duty.
Employee stated that EE was moving the miner in the outby direction. EE stopped the miner to move the CM cable. EE positioned self over top of the cable, when EE picked up on the cable, EE felt a sharp pain in groin area and lower back.

Employee stated that employee was attempting to unload stopping plaster, when employee stood up with a 50 lb. bag of "B- Bond", employee felt a pain in employee's back.

Employee was attempting to put a fork back onto the CAT 930H Forklift that had fallen off. The fork was not in place on the bar where it slides and when employee attempted to shove the fork into place it fell off the bracket and struck the employee's left foot.
INJURED WAS CHANGING GUIDE CHAINS ON A ROOF BOLTER. THE POT SLID MASHING EE'S RIGHT INDEX FINGER BETWEEN THE POT AND FRAME.
INJURED WAS INSTALLING BELT STRUCTURE OVER A BELT DRIVE. EE PLACED ONE END OF A RAIL IN THE SPREADER AND WAS CLIMBING DOWN TO PLACE THE OTHER END OF RAIL. EE PUT HAND ON THE MOTOR TO CLIMB DOWN AND THE RAIL FELL STRIKING HAND.
Employee was putting belt clamps on top belt and EE felt a "pop" in right shoulder. Employee was transported by ambulance to Logan Regional Medical Center. X-rays and CT scan was performed and no damage indicated.
Employee claimed injured back while moving rail on 5/13/16. Supervisor or coworkers were not informed nor was anything reported until 5/15/16 at which time the accident investigation began.
INJURED WAS PULLING CABLE UP A BANK AND TWISTED EMPLOYEE'S KNEE.
INJURED WAS PUTTING HYDRAULIC OIL IN ROOF BOLTER. WHEN THE OIL CAN WAS EMPTY HE PULLED THE FILL HOSE OUT OF CAN, GRABBED THE OIL CAN WITH HIS RIGHT HAND AND THREW IT TO THE RIB. A SHARP EDGE ON THE CAN CAUGHT HIS GLOVED FINGER AND SHAVED THE SKIN FROM IT. IT WAS THE MIDDLE FINGER ON HIS RIGHT HAND.
Injured was attempting to open a door on number one fan. The hinges broke allowing the door to fall on injured. injured received a broken left scapula.
Injured was installing roof bolts. he put the resin up in the drilled hole, stuck the roof bolt in the hole, then slid his hand down the bolt in order to put the bolt in the chuck. His hand came into contact with a sliver of steel, from where the bolt was punched in manufacturing. the sliver of steel went through the side of his glove and into the small finger on his right hand.
Injured was working on reel on shuttle car. The hinged lid has a prop to hold lid up. When he put prop in place and let go of lid, the lid fell and mashed his right hand, braking bone or bones in his hand. Injured was able to return to work on his next scheduled shift.
Injured was pulling cable for a belt drive and strained his back. Injured did not quit work at this time. Injured quit work on 02/06/2015
INJURED WAS LOADING CINDER BLOCKS INTO A SCOOP BUCKET. WHEN HE THREW THE LAST BLOCK, HE CAUGHT HIS HAND BETWEEN THE BLOCK AND THE SIDE OF THE SCOOP BUCKET. WHICH MASHED AND CUT THE MIDDLE AND RING FINGERS ON HIS RIGHT HAND. STITCHES WERE USED ON THE TOP AND BOTTOM OF THE FINGERS. X-RAYS WERE NEGATIVE ON ANY BROKEN BONES.
EE was climbing up hill and felt sharp lower back pain on the right side of spinal area.
