Employee stated that while attempting to remove the pin from the belt structure, the belt rail fell and the bottom roller hanger struck EE on top of EE's left foot.
Employee stated that employee and a co-worker were loading top belt structure into a scoop bucket and the structure twisted and smashed employee's right hand index finger.
Employee stated that they were in the process of pulling miner cable loop out from under the belt drift to rack on # 1 miner, when they threw a loop of cable on their shoulder they felt a sharp pain towards the middle of their back towards their spine.
Employee stated was attempting to remove the drill steel after drilling a hole in the top when a piece of draw rock struck employee on right forearm causing a laceration that required sutures. IE did not miss any scheduled work due this incident.
On 9/18/2019 a fire was discovered at the clean coal bin. This was an off shift and no employees were on property. Mingo 1 Plant only operates on the day shift. There was a security guard on site at the time the fire was discovered. Fire department was called and the fire was extinguished. no persons were injured.
Employee stated that employee was attempting to pull a P-40 Pump out of the mud when employee felt a pain in employee's back.
Employee stated that EE was attempting to hang bolter cable when the cable pulled unexpectedly causing EE's hand to be pulled into a pizza pan resulting in a laceration to EE's right thumb.
Employee stated that they were attempting to carry empty oil cans from the feeder through the left buggy dump when they stepped in a hole and felt pain in their left knee.
Employee stated was walking back to the section in the #7 Entry (Return) when employee stepped in a hole and felt a pain in back.
Employee stated was moving the RB Machine through fly pads when a bundle of plates fell into the deck and struck right arm just above the elbow.
Employee stated that was attempting to remove a top roller from the belt line when EE felt pain in back.
Employee stated that IE was attempting to load a shuttle car when the car became hooked on the cable hook of the boom. When the shuttle car pulled away from the miner, the hook broke off the miner. IE stated IE picked up the hook to toss it against the rib and was cut by a bur on the hook.
Employee stated that EE was attempting to unlatch EE's seat belt when the seat belt buckle struck EE in the mouth chipping EE's left front tooth. The employee had EE's tooth fixed the next day, but has not missed any scheduled work due to the incident.
Employee stated was attempting to service the CM when the employee slipped on a slick rock and leg slid under the back of the machine. This caused pain in left knee.
Employee stated that when attempting to open the inby set of airlock doors near the section, they attempted to grab the top of the door to keep it from slamming against the rib. this caused them to get a finger caught between the door and a roof bolt in the mine roof.
Employee was attempting to drop the miner cable during a power move when their foot became stuck in the mud. while dropping the cable they twisted and felt a pain in the lower back and left knee.
OUTSIDE OF GUARDSHACK RIGHT FOOT SLIPPED ON GRAVEL HURT BACK AND KNEE.
Employee stated that when they attempted to turn and duck underneath the canopy of the bolting machine, they struck their face on a roof bolt plate that was hanging over the edge of the bolt tray. This caused a laceration to the right cheek that required sutures. The employee did not miss any scheduled work due to this incident.
Employee stated that as EE was attempting to raise the battery lid on the scoop, the lid slipped and caught EE's left ring finger between the frame of the battery and the battery lid. This caused a laceration and a fracture to the finger.
Employee stated that EE was cutting a piece of structure off the tail piece, when the structure came loose and struck EE in EE's left leg.
Employee stated that EE was attempting to bend a bolt to fit it into EE's drill mast when EE felt a pain in EE's lower back.
Employee stated EE was taking the rub rail off the CM when EE's hand slipped and pinched between the rub rails causing a small laceration and a fracture to EE's left ring finger.
Employee was making a cable splice on the #4 S/C when EE's hand slipped which caused the knife to cut the tip of EE's left ring finger requiring sutures. Employee did not miss any scheduled work days due to the injury.
Employee stated ee was attempting to shove the feeder shaft from the flat car to the end loader bucket by using knee when ee felt a pain in knee.
Employee stated that ee was walking to tool box when ee struck head on a low hanging brow. This caused pain in neck.
Employee stated that the GT568 4 Wheeler jumped into high tram causing employee to strike head on the steering wheel. This caused a laceration to the forehead that required sutures. Employee did not miss any scheduled work days due to this incident.
Employee stated that EE stepped off of the back of the feeder onto a piece of coal which caused pain in EE's right ankle.
Employee stated was attempting to remove a rock from the continuous miner with a pry bar when the rock slid off onto EE's left foot. This caused a fracture to the left foot.
Employee stated that EE bent over to pick up a battery jumper cable when EE felt a pain in EE's lower back. Employee was taken off work by physician.
Employee stated that as employee was attempting to cut a piece of belt with a belt knife, the knife slipped and employee cut right thigh. Employee received staples. Employee returned with no restrictions on next scheduled shift 12/7/2018.
Employee was shoveling belt at Brk 7 on the #4 Belt when a piece of draw rock struck them on the top of the head. Employee was taken off work on 11/16/2018 and has not returned to work.
Employee was shoveling along belt when a piece of rock fell and stuck EE on the back of EE's hard hat.
Employee stated that when ee was attempting to pick up a stopping block ee twisted and felt a pain in back.
Employee stated EE was at the feeder attempting to restart the feeder when the left S/C struck EE.
Employee was loading bolts into the tray when they felt a pain in their back. Employee returned to work on 10/26/18, but was taken off work by a physician on 10/29/18 awaiting an MRI.
Employee was attempting to install a 6 foot resin bolt but the entry height was only5' 8". When ee attempted to force the bolt into the hole ee felt a pain in back.
***We tried to report this Injury on 10/23/2018 but got caught kicked out of the MSHA site under a specific login*** Employee was loading cinder blocks into a scoop and another employee put blocks into the scoop and the blocks smashed injured employee's right middle finger.
Employee stated was moving cutting slack out of the roadway when a small piece of rib rock fell out from the top of the rib and struck employee on top of right foot. IE sought medical attention on Friday 10/12/2018 where IE was diagnosed with a fractured bone in foot and was taken off work by IE's doctor.
A fall occurred at the mine on the MB #2 Panel in an intersection between the #2 and #3 entries inby the section tailpiece.
While exiting a Coal Hauler, EE stepped down on uneven bottom. Felt pain in the right hip and back.
While operating a Shuttle Car, EE's shirt sleeve caught between the coal rib and the SC. frame pulling EE's left arm into contact with the rib resulting in an evulsion to the Left forearm. EE received 16 Sutures.
Employee stated that ee had pulled onto the feeder to dump the shuttle car and stepped out of the buggy to turn seat around without setting the park brake on the center drive shuttle car. The car rolled back and caught lower right leg (calf) between the shuttle car and the mine rib.
Employee stated was walking along #2 Belt line and slipped but did not fall. At this time EE felt a pain in lower right side.
Employee stated ee was walking in the cross cut between the # and #4 entries at Brk 7 when ee slipped and attempted to catch self on the rib with left hand. This caused a pain in shoulder. Employee worked until a doctor took ee off work on 7/26/2018.
Employee stated that ee was attempting to hook a chain to the scoop bucket when the operator of the scoop inadvertently lowered the scoop bucket onto left leg just above knee.
Employee stated that as EE was kneeling near the rib on the #1 Section during the last lift of pillaring on the solid block in the #1 entry, a section of rib fell out and struck EE in the lower back.
Employee stated was attempting to install a roof bolt that was partially bent, when employee activated the rotation to line the bolt up in the drill chuck, the corner of the bearing plate struck the employee in the right forearm causing a laceration. The employee received medical treatment (3 sutures) and returned to the mine for next scheduled shift.
The employee stated EE was attempting to make a belt splice when EE struck left middle finger with a hammer causing a laceration to the finger.
Employee stated that ee went underground to make the face area. On ee way back outside on the number 5 belt line ee fell hurting back and left arm
Employee stated that employee was attempting to lift a shield on the continuous miner when the shield slipped and struck employee on employee's left middle finger causing a fracture to the finger.
