Mostrando los 12Employee was attempting to load a draw head from the rock duster up on top of the duster and when lifting the draw head said that EE felt and heard back pop. The draw head weighs between 100 and 150 lbs.
Attempted to roll a pump motor off mantrip when I bolt on top of motor broke causing employee to lose balance falling to the ground. Employee hit hard enough causing EE to chip tooth and jam neck and back.

Employee was restocking the supply tray with 6"x 6" roof bolt plates on the Fletcher Walk-Thru roof bolter ee was operating. EE was carrying 2 bundles of roof bolt plates to the supply tray. EE lost balance. This caused the 2 bundles of roof bolt plates to smash right index finger.
Employee had hand on loose shield of the man trip and suffered a laceration.
