EE was found to have 10% pulmonary function impairment.

An unplanned roof fall was discovered in the #6 intersection at #41 crosscut on the #10 belt. This entry was the primary escapeway at the time the fall was discovered.

EE was attempting to lift a timber to be placed in a scoop, EE felt a sharp pain in EE's back.

Employee was carrying a 40lb. bag of B-Bond to place on a scoop, their feet got tangled in a banding strap and they fell twisting a knee. Employee worked next scheduled shift then returned to a specialist on 6/2/2025 where they were diagnosed with a torn meniscus.

Employee reported that their eye was irritated after making a cable splice. Employee was unable to identify the mechanism of injury, but was diagnosed with having a foreign object in their eye that was removed in an outpatient surgery.

EE was shoveling at a v-plow on the #10 Belt when EE slipped resulting in straining EE's hamstring. EE was placed off work on 5/28/2025 after seeing a specialist that diagnosed EE with two torn tendons in the hamstring.

EE was struck by a rib roll in the right foot while setting over in cut with EE's continuous miner. EE did not initially seek medical treatment for EE's injury. EE reported a sharp pain on 5/17/2025 and sought medical treatment at that time. EE was diagnosed with an Achilles Tendon rupture on 5/28/2025 and placed off work at that time.

A continuation of a roof fall was found during the weekly airway exam. The fall is located at 15-16 crosscut 13-2 belt #2 entry, and now impedes passage. The fall now extends from 15-17 crosscuts.

A rock dislodged from the mine roof on the #2 section and struck employee in the right arm and hand.

EE was assisting in pulling cable slack, the cable became fouled on a rock, pulled tight and struck EE in the left knee. EE was placed off work after seeing a specialist on 1/9/2025.

EE was assisting the electrician with a cable repair and a rock measuring 54" x 24" x 2" fell from the mine roof and struck EE in the head and shoulder.

A roof fall measuring 20' x 40' x 8' was found in the return air course on 13-2 Belt in the #9 entry at 43 break.

A roof fall was discovered in the primary escapeway at 39 crosscut on #11 Belt in the #6 entry measuring approximately 70' wide x 20' long x 6' thick. This was a continuation of a roof fall that had previously been reported and dangered off.

A roof fall was discovered between crosscuts 20-21 in the #4 Entry in the construction area of #1 section measuring approximately 90 feet in length and 10-12 feet in thickness.

A motorman at Maple Eagle #1 injured right pinky finger while attempting to put the 15-ton Brookville locomotive back on the rail. EE grabbed the handle of the lifting jack to adjust its position on the mine floor and had not removed their hand. When the weight of the locomotive was applied, this pinched the victim's pinky finger between the jack handle and lifting mechanism.

A roof fall occurred on 11 Belt 46 Break. The roof fall measured approximately 150' in length and 6' in thickness. Ventilation was not affected, but the primary escapeway had to be rerouted.

Employee was in the process of knocking the miner boom pin back in on unit #2 on #112 C/M, when a piece of metal came off the pin and struck the employee in the left pinky finger. On 04/25/2024 the employee had the metal taken out of EE's finger at CAMC and was placed off work.

A roof fall occurred at 9 belt intake #7 entry between 9 and 10 break. Ventilation was not affected; the area was timbered off and the lifeline was rerouted. The roof fall measured 20 feet in length and approximately 6 - 7 feet high.

Employee was in the process of loading the roof bolter in #5 entry break 17 on Unit 2. Employee was sliding a bundle of roof bolt plates over to make room for roof bolts. As employee pushed on a bundle of plates their left forearm was cut from another bundle of plates. Employee was seen at hospital and received 5 stitches and was given a full work release.

Employee had got off the mantrip to open a set of airlock doors on 12 track. As EE started to walk towards the doors EE felt a pop in the right knee. EE was seen at a medical clinic and was given a full work release. On 05/17/2024 EE was placed off work to have the graph repaired on the meniscus and ACL.

On 01/10/2023 at 3:00pm we had a roof fall at 13brk on 6 belt. The mine was being fire bossed at this time and had been idle due to power loss at the mine. Once the roof fall was discovered the area was dangered off and a plan was submitted to clean up the area and re-bolt the area. No injuries occurred from this incident.

An unintentional roof fall above the anchor point occurred on 10/30/2023 on Unit 2 inby the feeder line at 12 break between 5 right extending to 7 right. The fall was approximately 240 feet in length and above the anchorage point. The operator has decided to pull off the section and move to a new location.

An unintentional roof fall above the anchor point occurred on 10/25/2023 on 10 belt break 41 in the #2 entry. The fall measured 20' long by 20' wide, and approximately 10 foot in height. No injuries occurred. The Travel way for the air way was blocked, ventilation was not affected.

Employee was mining in 2-left at 19 brk on #1 section. EE heard the rib start popping so EE moved to the face of #2 that was drove up 4'. After loading the shuttle car, a rock rolled out of the face striking EE on the lower left leg / foot. The rock measured 18' L x 2' w x 1' thick.

Employee was bolting in #5 entry on #1 section. After drilling with the 3' starter, EE inserted a 6' stack steel and used the fast feed to run the steel up the hole. EE let off the fast feed about 1 foot from the termination point but the fast feed stuck causing the steel to break striking EE on the right index finger resulting in 7 stitches.

EE was spot bolting in #6 entry on 2 section. EE was operating the 406 R/B while swinging the drill station out the drill steel caught the top and broke near the chuck. The steel sprung toward EE and struck EE on the left ring finger. EE transported self to plateau medical and was given 4 stitches in the finger, EE reported back to work on EE's next shift.

Employee was helping get a mantrip through a track switch on the surface when the mantrip struck employee's right foot causing employee to fall and hurt left knee. On 6/28/2023 the operator received the report of an MRI showing a non-displaced fracture of the patella. This is now being reported as an NFDL incident.

Employee stated was setting the roof bolter up in #9 entry on #1 section when EE noticed a rock falling. Employee stated tried to move out of the way, but the rock struck EE's left forearm resulting in a laceration. The rock measured 6"Lx2"Wx1/4"thick. The employee received 7 stitches and returned to work on EE's next scheduled shift.

A contract motorman working through a contractor company was in the process of jacking a flatcar back on the track, when the motorman slipped and fell on right knee resulting in pain in lower back. The motorman sought treatment at a clinic where the motorman was placed on restricted duty.

Employee was dragging an airlock door away from the track when employee felt pain in left arm. Employee sought medical treatment and returned to work. On 12/5/22 employee was placed off work and surgery was scheduled for 12/8/22 to repair bicep tear.

Employee was drilling a hole in the mine roof when drill steel broke and hit EE's left forearm, resulting in a laceration requiring 10 staples. Employee returned to work their next scheduled shift with a full release.

A portion of the highwall at the mine fan slid in causing a void to the fan portal, this caused the first 2 rows of bolts to fall out.

Injured employee was in the process of cutting expanded metal with a grinder to fabricate a guard for a belt head, when the grinder caught the expanded metal causing the disc to climb up the metal and contact employee's left index finger. Employee finished shift and sought medical treatment where employee received 7 sutures. Medical Only.

EE was walking inside the mine when EE stepped into a sump hole approx 18"-20" deep and lost EE's balance hitting EE's knee on a valve. Not tools or equip involved

Had Roof Fall at Cross Cut #46 in the track entry that measured 8Ft thick, 80 to 90 FEET long and 18 Ft wide.

Employee stated EE was cleaning out the Scoop Deck and a sharp pain occurred in the lower center of EE's back.

A roof fall had occurred on Number 10 Belt in the Belt Entry at Cross Cut #41. The fall was above anchorage and was 15 FT long, 18 FT wide and approximately 12 FT high.

While IE was assisting building a block stopping, prior to it being wedged tight, the stopping fell over, catching IE's foot between blocks, fracturing foot. Injury was not the result of a training or compliance issue.

Operator was installing the outside roof bolt when a piece of rock fell striking EE on the outside of EE's right foot. Employee was taken to the hospital where xrays showed EE had a fractured metatarsal.

Employee was changing drill steel on Roof Bolter when a piece of rock fell striking right hand causing a laceration to right middle finger.

Roof Fall in the Number 1 Entry at Break 35 11 Belt. Fall was above Anchorage blocking Lifeline Entry of Primary Escapeway. Area has been dangered off and Lifeline Re-Routed.

Employee was riding a 2-passenger mantrip. They were traveling up a hill when the yoke on the rear drive shaft broke. When the yoke broke from the rear drive shaft, the mantrip lost braking capabilities, causing the mantrip to coast backwards down the hill. The front passenger tire struck the employee's left lower leg.

An unintentional roof fall above anchor occurred on 8/7/2020 on 10 Belt break 33 in the no 4 entry. The fall measured 90 foot long, 20 foot wide, and approximately 10 foot in height. No injuries occurred and ventilation was not affected. The escape way was re-routed around the fall.

Employee was setting a beam across number 10 belt, after setting the beam and while gathering up tools, a piece of slate broke loose above the beam and struck them on the hard hat, sliding down back, compressing neck area.

Employee was operating a rubber tired mantrip when the front wheel caught a track tie causing the steering wheel to spin quickly causing a pain in EE's right shoulder.

Employee was installing a glue stick and bolt into the rib when the rib broke loose and struck the employee on the right leg.

Employee was loading supplies on the roof bolter when a strap slipped cutting thru EE's glove and causing a laceration on the thumb.

Employee was operating a 2 man personnel carrier when the axle broke causing the employee to lose control of the vehicle causing it to wreck. The employee suffered from a laceration and contusions.

Employee was walking down the belt entry when employee struck their head on an overcast causing pain in neck and back

Employee was stepping onto the roof bolt machine when they lost footing falling onto their back.

IE hit head in canopy of scoop when they trammed it over obstacle in roadway.

Employee was riding on a rubber tired mantrip when a piece of rock fell out between the supported mine roof striking the employee in the left upper leg area.

Employee was driving a golf cart and a small piece of rock fell from the mine roof striking a hand causing a laceration that required stitches.

Employee lost footing and rolled ankle

Employee was removing a chain from the scoop when the load shifted causing the chain to contact finger.

Roof fall found in the Johnson Fork Butt off's in the #3 entry, between 55 break to 56 break. Approximately 80 feet long, 19 feet wide and 8 feet thick. The fall does not impede travel or air movement. No injuries or entrapment. A location and map has been sent to Mount Hope Office.

Employee was bolting top and dropped wrench. EE stepped down off of the platform to get the wrench and slipped on the muddy bottom. EE's knee went one way and EE's body went the other.

Employee was loading belt structure onto a flat car and mashed left ring finger between a bottom roller and cradle thus causing a laceration that required four stitches.

Piece of rock struck employee on hat causing it to push into safety glasses thus causing a laceration to nose requiring stitches to close.

Employee was crossing belt line with tools in hands and jumped from side of the belt line landing on a rock thus twisting ankle

Employee was attempting to load a draw head from the rock duster up on top of the duster and when lifting the draw head said that EE felt and heard back pop. The draw head weighs between 100 and 150 lbs.

employee was bolting top and when pulled forward to install the next row of bolts a piece of rock fell from between last two rows of bolts installed and struck employee on head and foot

Employee was in the process of doing pre-shift examination on the off side of #12 belt drive. As employee was walking around the drive, employee stepped on something in the mud, twisted left foot, ankle and knee

Attempted to roll a pump motor off mantrip when I bolt on top of motor broke causing employee to lose balance falling to the ground. Employee hit hard enough causing EE to chip tooth and jam neck and back.

Employee was dropping miner cable and water line from cab cable hanger and hook penetrated employee's glove lacerating hand between thumb and forefinger

The employee was helping spot a rib bolt when a rock rolled out of the rib and struck EE behind the metatarsal guard of EE's boot. The rock punched a hole in the top of EE's foot that was too deep the clean, so the medical facility did not clean it right away. This prevented stitching of the wound.

EE was pulling excess curtain from the mine floor to advance curtain on bolt cycle in the face. A rock measuring approximately six inches wide by seven inches long by three inches thick dislodged from the mine roof and struck ee on left shoulder. EE finished shift but went to the emergency room later in the evening with complaints of shoulder pain.

Employee was in the process of cutting belt rubber with a knife and while cutting the belt the knife slipped, causing them to cut their left thumb. This resulted in a laceration that required 5 stitches to close the wound.

Employee was on knees cutting plastic bands off of a new joint of miner waterline for replacement. EE lost balance causing EE's weight to shift. As EE tried to regain balance, EE cut left thumb.

2 employees were supplying 2 section. The outby motorman told the inby motorman to drop down 10' to be lined up to unload. When the inby motorman applied the brakes, the trip started sliding downgrade toward the mainline switch. The inby motorman stated EE was ejected when the motor derailed at the mainline switch.

Employee was retrieving CO cable from 2-A belt head for installation on the 2 section belt move. EE placed right foot on the fresh water pipe to gain height to reach the cable. EE's foot slipped and caused right knee to buckle. EE sought medical attention after the shift and was placed on restricted duty.

Employee was dropping cables in preparation for a power move. As EE dropped the cable, EE's left ring finger came in contact with a rib support (pizza pan) causing a laceration to the side of EE's left ring finger. EE did not receive any stitches, but was referred to a had specialist for further examination/ treatment. EE was allowed to return to work on restricted duty.

Employee was loading roof bolts onto the Roof Bolter. While doing so, a piece of mine roof approximately 8" x 8" x 2" fell and struck EE on the right middle finger causing a laceration that required part of the fingernail to have to be removed and 2 stitches to close the wound. The mining height is 8 1/2 feet in this area.

The employee was constructing a stopping at 59cc #1 Section between #7 and #8 entries. The area was 9' 3" high and 18' 8" wide. EE was standing on a ladder to place another block on the stopping. As EE placed the block, a section of rock measuring 24" x 13" x 1" fell out and struck EE on top of the right hand. The injury took 7 stitches to close.

Employee was walking toward the section and slipped in a Scoop rut twisting right ankle. The mine floor was wet and muddy at the time of the accident. Employee returned to work the next day (6-1-18) with no restrictions. Employee returned to the Doctor on 6-8-18 and was put on restricted duty for 2 weeks beginning on 6-9-18.

An unplanned roof fall on the off side of the belt in a crosscut was discovered by the examiner during EE's examination. It did not impede travel or airflow. There were no injuries. The roof fall measured approximately 90'L x 20'W x 6-8'T.

Employee was trying to remove a piece of rock that was resting on the drill boom. The rock flipped back and struck the end of EE's right index finger causing a laceration.

Contractor was placing the pin in the draw head to connect the locomotive to the rockduster and smashed left middle finger.

Employee was riding on the back seat of the shuttle bus when the bus operator abruptly hit the brakes due to holes in the road. This caused the employee to be thrown out of the seat and strike the seat in front of EE. EE's left leg was caught under the seat in front of EE and EE's weight went over the seat. This incident became reportable on 3/27/2018.

Employee had just finished drilling a roof bolt hole in the #4 entry on the #1 section. The drill steel and chuck saver detached from the drill pot and stay in the hole as EE was lowering the drill pot. The chuck saver suddenly released from the drill steel and fell approximately 3 feet striking the employee on the left hand.

Employee was getting ready to drill a roof bolt hole in the mine roof. A piece of rock measuring approximately 1'L x 1'W x 3"T fell from the mine roof when the drill steel touched the mine roof. This resulted in a laceration to EE's right forearm and took 4 staples to close the wound.

Employee was operating a roof bolter in the intersection of #5 entry on the #2 section when a piece of rock approximately 7"L X 3-5"W X 0-2"T fell from the mine roof and struck on the right forearm causing a laceration that required stitches to close.

Employee was lifting a full oxygen cylinder when ee felt something "pop" in lower back.

Employee was attempting to use the quick release to let down a 10 ton Simplex Jack. The plunger on the jack quickly came down and caught the employee's left middle and ring fingers between the jack base and plunger causing lacerations to both fingers.

There was a roof discovered along 11 Belt at 4 Break between the #4 and #5 Entries. The roof fall was approximately 16'H x 60'L x 18'W.

Employee was bolting in the face of the #6 entry when a piece of rock measuring approximately 2'L x 1'W x 2" slide down the T-Bar and struck them on the right calf.

Employee was exiting the mine when they struck their face/nose on the frame of a manual airlock door.

Employee was pulling on the Continuous Miner trailing cable in order to get it out of the way of the continuous miner. EE strained left shoulder while doing so. *** Employee worked normal schedule until ee was taken of work on 12/13/2017.

An unplanned roof fall was discovered in the intersection of the #2 entry at 85 break on the old #10 belt area. The fall measured approximately 20'L x 20'W x 6 - 8' thick.

An unplanned roof fall was discovered in the intersection of the # 6 entry at 35 break on the old #8 Belt area. The fall measured approximately 20'L x 20'W x 6 - 8' Thick.

A fall of the mine roof was discovered at Brk 40 of the Johnson Fork Mains, the fall occurred from between the #4 and #5 entry over to the intersection of #6 entry. This fall measured approximately 120 ft Long X 10 ft Thick X 20 ft wide. This fall does not impair ventilation or impede travel in this area.

Injured employee and two other employees were changing the cable reel on a shuttle car. They were using a 3/4 ton chain ratchet to pull the cable reel out. Injured employee was using left hand to operate the chain ratchet and had right hand resting on the chain the chain ratchet was hanging on. The cable reel shifted causing the chain ratchet to slide down the chain.

Employee was restocking the supply tray with 6"x 6" roof bolt plates on the Fletcher Walk-Thru roof bolter ee was operating. EE was carrying 2 bundles of roof bolt plates to the supply tray. EE lost balance. This caused the 2 bundles of roof bolt plates to smash right index finger.

An unplanned roof was discovered in the #7 Entry on the #1 Section at approximately 8:50 a.m. The roof fall measured approximately 90 feet in length and was approximately 6 to 6 1/2 feet thick. It occurred approximately 25 feet outby the #23 crosscut to approximately 40 feet inby the #21 crosscut.

Employee stated employee was walking in #4 entry Inby the feeder line and employee rolled employee's ankle causing employee to fall onto the mine floor.

EE was operating a remote box for a miner when a piece of rock struck right hand causing a small laceration. On 3/1/2017 during a follow-up visit to Dr. EE was placed on restricted duty to prevent potential infection.

Employee was struck by a small piece of rock on the right foot. Employee suffered a contusion to the top of foot. Employee continued to work until 2/14 when employee went to the ER with a small swollen area. The employee is a diabetic and as a precaution the ER Doctor recommended restricted duty for 3 days. Employee returned to normal duties.

An unintentional roof fall was found by weekly examiner in the Johnson Fork Mains at spad no. 7478 in the no. 7 entry beginning at x-cut 34 (inby approx 1/2 way of the block) and extending to the 36 x-cut, 20 feet wide and above the anchorage of the bolts.

DURING THE WEEKLY EXAM THE FIRE BOSS FOUND AN UNINTENTIONAL ROOF FALL AT 34BK IN THE #5 ENTRY. THE FALL WAS 7 TO 8 FT THICK IN THE Intersection. this FALL WAS IN A AREA THAT IS NOT NORMAL TRAVELED AND DOES NOT AFFECT VENTILATION CONTROLS.

Injured employee was trouble shooting when a single piece of draw rock fell from a supported area and struck injured on the back and left ankle resulting in a contusion.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #3 ENTRY AT 38 CROSS CUT ON THE MMU 0020. THE FALL WAS 0 TO 8 FT THICK IN THE INTERSECTION.

Employee was roof bolting and a rock struck EE causing EE to stay overnight in the hospital.

Employee had hand on loose shield of the man trip and suffered a laceration.

Two employees were unloading 4" I-beams to use as roof support. One employee picked up one end of a beam causing it to roll over pinching the injured employee's finger against the bed of the flat car.

Employee was stepping up on bolter to travel up through bolter to start roof bolting. As he entered the bolter he struck his Rt arm on a stack of roof bolt plates which cut him on the Rt forearm.

While cutting straps on bundle of timbers, the timbers started to roll over causing employee to reach out to block timbers from hitting his legs and feet while doing this he cut his Lt index finger with a razor knife he had in his Rt hand.

Employee was loading pizza pans on the roof bolter when he slipped and as he was falling, his hands slipped down the side of the pizza pan. The edges of the pizza pan were rolled over but still had sharp edges that cut his fingers.

An unplanned roof fall occurred at 4:10 am on 6/19/13 on the MMU-001 section in the last open break in the connecting crosscuts between #5 and #7 entries. MSHA and State were notified immediately. Roof fall did not affect ventilation or passage of persons, support consisted of 6' glue bolts. Fall was rehabilitated.

Employee was using a hammer to try to uncouple flat cars from a motor and struck his right index finger causing a laceration which took 3 sutures to close.

Employee was drilling out a PUR nozzle with a drill and bit, the drill slipped and the 3/16" drill bit broke and hit employees left ring finger. The employee was holding the nozzle with a pair of channel locks and the broken bit attached to the drill still hit employees finger. The employee received sutures, and is on restricted duty and will return to full duty on 03/25/2013

EE was batting top when his drill steel became hung inside the chuck,removed plug under the chuck and inserted a roof bolt in the bottom to knock steel loose,dropped wrench bent down to pick it up when his short sleeve caught the rotation lever causing him to struck in rt hand & inside area of his leg rt by roof bolt causing fracture to rt hand and laceration to groin area

Employee and 2 other employees was installing top panels onto an over cast when the panel they were lifting slid down and struck the outside area of his right wrist causing a laceration which required 5 sutures to close.

EE was observing the roof bolting crew installing wire screen on the rib when a piece of rock measuring 39" x 37" x 6" fell out from between bolts in the mine roof and struck him on top of his right foot causing fractures to 3 of his toes.

EE was using a utility knife to cut a rope hanger that was holding a cable up with pressure on it and when he cut the rope the cable shifted causing him to lose his balance and the utility knife blade cut his right leg causing a laceration which took 8 sutures to close.

Employee was operating a D6T dozer to build a new road above the existing road when the outside bank gave away and the dozer rolled over the bank. Employee was initially treated for a concussion but has not returned to work, exact injuries unknown at this time, but he is off work and undergoing physical therapy for his neck and knee.

Employee was using a utility knife to cut excess tape off of a dust hose when he slipped on rock he was standing on causing him to run blade across his left forearm causing a laceration which took 5 stitches to close. Treated at Montgomery Hospital and release he returned to work same day. No days lost, reportable due to stitches.

Employees were in the process of making a belt splice. EE was holding a knocker steady while another employee was using a Flexco hammer to drive it, when a piece of the knocker or hammer broke loose and struck employee in his right arm. EE later felt discomfort and was examined at Somerville Hospital where a piece of metal was removed & placed on restricted duty until 7/26/12.

EE was on his knees using a utility knife to cut belt rubber when he fell forward and the knife blade cut through his left glove into his hand causing a laceration which required 4 stitches. EE did not miss any work due to injury.

EE was moving C.M. cable out of the haulage roadway when a piece of rock fell out from between bolts in the mine roof and struck him on top of his left arm causing a laceration that required staples to close. Returned to work but dirt inside the opening caused him to return to hospital where he has been taken off work at this time. This is a LTA.

While lining up the tail piece during a belt move, the employee slipped putting increased pressure on his right foot causing it to roll over on to the side of the foot.

While walking past the #1 belt take-up power pack a hydraulic hose burst causing hydraulic oil to spray on the EE. The EE was taken to the hospital where his eyes were cleaned and he was prescribed an antibiotic cream for his eyes. He was released to RTW on his next scheduled shift.

Employee was in the #2 entry at the last open break on the 001 section supplying the roof bolting machine, when the roof bolter operator turned the machine on & was not aware the cable reel lever was on & when the reel was engaged, employee was struck in the right arm & shoulder. Employee finished his shift but later was examined by a physician & was taken off work

Employee was roof bolting the mine roof when he was struck in the left hand by the drill steel when the insert broke. Employee completed his shift but later was examined by a physician at Summersville Memorial Hospital and was placed on modified duty for one week with no use of his left hand. This is a reportable only injury with no lost time due to incident.

EE was performing routine maint. repairs on C.M. when he removed his left glove to take out planetary plug to install fluid, a piece of rock meas. 4"X 7"X 1/4" thick fell out from between bolts and hit him on his left hand causing a laceration which required 6 sutures to close. EE has been released to full duty on 3/6/12.

Shuttle car was hung up on feeder, when shuttle car operator tried to pull off feeder, tire struck a piece of rock and it struck employee in the forehead causing a contusion. He was transported to MGH and cat scan revealed negative findings and he was told to remain off work until March 1, 2012.

Employee had pulled motor outside and was exiting the deck of the motor and stepped in a small ditch line and twisted his right ankle. He finished his shift but was examined by a doctor on 2/15/12 and was taken off work due to restrictions and instructed to return to be re-evaluated on 2/19/12 at which time we may know his return to work date.

Employee was supplying roof bolter when a piece of rock fell out from between bolts and struck him on his left shoulder and arm causing a laceration to his left arm which required 5 sutures to close and he was released back to full duty on 2-3-12.

Pulling feeder cable by hand and as he let go of cable to drop it to mine floor, he extended his left arm about downward and felt his elbow pop. He completed his shift but later went to hospital to be examined and was told he has a pinched nerve and possible ligament damage and he been taken off work until 2/16/12.

EE suffered a heart attack on 12/5/11 and passed away on 12/10/11.

EE alleges that his lower back was injured while prying upwards with a slate bar while performing maintenance repairs on a cont. miner. Two EE's working with him stated this never took place and that the IEE never informed either of them that he was injured. EE rec. treatment the next day at St. Francis and was taken off work by Dr.

Employee was stepping off the back end of the U307 roof bolter on the 002 section to go assist other workers with building a stopping, when he slipped and fell off the roof bolter and his SCSR located on his belt struck the step on the back of the machine causing pain in his lower back. X-rays were negative at Montgomery General Hospital and his is on modified duty until 11-11-11.

Employee was assisting another individual in picking up Heinzman jacks they were going to move, when as he picked up the top end, the adjustable top section of jack came down and smashed his right thumb between it and the frame of the jack causing a laceration to the inside of his right thumb which required 3 stitches to close at CAMC where he was treated.

Moving cable from the roadway felt a pulling in his shoulder. EE continued working until April 5, 2012 when he went off. The safety department was not informed of him being off until 5/20/12.

EE was a passenger riding from underground to the outside when the mantrip he was riding on failed to stop in time and ran through an airlock doors causing blockage above the door to dislodge & fall and strike him on his left leg & foot. Transported himself to Summerville Hospital where x-rays were negative, placed on restricted duty for 1 week.

Employee was climbing down ladder of end loader when his boot became caught in the last step causing him to twist his right knee. Employee first visited doctor on 10/15/11 with follow up appointment of later dates. Due to continued discomfort has been taken off work effective 12/12/11 until he receives an MRI on knee by doctor at hospital in Beckley.

Employee was looking up a cheve wheel to the mine roof when his right glove got caught in wheel and pulled his right pinky finger into the cheve wheel causing the nail and shin to come off of end of finger. Employee was later examined and x-rays shows slight fracture to pinky finger. No missed days. Prescribed medicine for inflammation.

Employee was using a utility knife to cut line curtain when the knife slipped and cut the inside of his left hand across the palm severing two ligaments and twenty some sutures were required to close. Surgery to repair ligaments was performed on 10/5/11 and he returned to restricted duty on 10/6/11

EE was inside deck of shuttle car he had just moved to use scoop to clean feeder dump. When exiting shuttle car his right leg gave out causing him to fall and his right hip popped out of socket due to previous injuries from vehicle accident in which reconstructive surgery had been done.

Employee was walking up roadway when he slipped and fell striking his right elbow on mine floor. Employee was examined at hospital and referred to specialist.

Walking around scoop located outside on mine surface area when he lost his footing and fell striking his right leg just under the knee cap on a rock. Employee drove himself to Montgomery General Hospital where x-rays showed a fracture and he was referred to specialist. Employee missed no work, but was restricted duty.

Employee was roof bolting and had steel hung in hole and while pulling on steel to free it, he took a quick step back and hyper extended his left knee. Employee was examined & returned to specialist but could not be treated until after a heart cath had been performed due to other health issues and therefore missed days have begun to accrue on 9/26/11.

Employee was stepping from platform of roof bolting machine to the walkway area of machine when his foot slipped causing him to lose his balance and fall causing pain in his lower back. Transported to Montgomery General Hospital where x-rays were negative. MRI is scheduled.

Employee was walking around roof bolter where a piece of rock had just been pulled on the opposite side of the machine when a piece of rock measuring 24" x 36" x 1.5" fell out from between rows of bolts and struck him on top of the head. Bolts were 3'4" from rib and 4' from row to row. Put on restricted days 5/4/11 - 5/7/11.

Employee was walking in roadway at #9 break on the #1 bolt when he stepped into a rut in the mine floor with his right foot causing him to lose his balance and fall twisting his right ankle. Employee transported himself to hospital where x-rays revealed a lateral malleolar fracture to his right ankle. Employee is currently on light duty, no days lost.

Employee was located in the track entry where the track crew was pulling rails off the rail car, one of the rails caught on another rail in a bind, when it broke free, it swung across the entry and struck the employee in the left leg. Employee was transported to Montgomery Hospital where x-rays revealed a fracture to his proximal tibial plateau, which would require surgery.

Employee was firebossing when he slipped and fell on a water line

Employee was in the process of making a cable repair to the left roof bolting machine on the 002 section. While holding the cable with his left hand and cutting the outer insulation off the cable with his right hand using a utility knife, the knife slipped and caused a laceration to the fat portion of his left thumb, which required 7 stitches to close.

EE was trouble shooting a ground fault in the master station on the miner. After isolating the control breaker and transformer, he started hooking heads back to the control breaker. He had locked the main breaker, but did not lock out at the power center. While connecting leads, the EE removed trouble shooting gloves, his hand came in contact with a lead on the main breaker.

EE was stepping along belt line and twisted his left ankle.

While crossing the inby airlock door, someone attempted to open the outby airlock door causing an increase in air pressure on the inby airlock door. In doing so, the 2nd door of the inby set of doors tried to close and struck the employee on the right arm.

The employee was riding on the side of a scoop. When the scoop turned to go around a corner, the employee's left foot was caught in a pinch point area.

Employee claims to have been loading a take up jack into a scoop bucket and felt pain in his right shoulder. Note: Company feels an accident did not occur. He left work after filling out the accident report himself and did not contact the company until 6-21-10. Asked is he had seen a doctor, he replied NO. During Part 50 Audit it was discovered he had.

While removing insulation from a cable, EE was using improper cutting procedures. The knife he was using slipped from the insulation and struck him on the left palm causing a laceration.

While supplying the roof bolter with roof bolt plates a piece of wire came in contact with the EE's right eye. The doctor "prescribed medication for eye irritation" for 5 days. Discovered during a Part 50 Audit.

This form is filled out due to a Part 50 audit. The company does not agree with the decision. He was under doctor's care when incident occurred. To the best of my knowledge. Employee claims to have walked into the TRS and jammed his neck.

Employee was shoveling along the #6 belt tail when a piece of coal and rock rib rolled off striking his left leg and lower back. Employee works through Superior Elite Coal Services.

Employee was walking across the 001 section in the #5 entry when he stepped in a hole and hurt his right ankle. Employee works through Superior Elite Coal Services.

A piece of rock fell and struck the EE on the right arm causing a small laceration 2 CM in length. This incident was discovered during a Part 50 Audit. The incident was not reported to the Safety Dept. due to no loss time. Sutures makes it reportable.

EE was moving two 13" tires from the back of the mantrip to the front deck of the mantrip. When he placed the tires on the deck he felt pain in his lower back. The EE went to Med Express where he was examined by a Physician Assistant. EE was seen by a Doctor on 3/22/2010.

Loading new plastic waterline onto a scoop EE felt pain in his lower back. The EE went to the doctor and did not inform management that he had went to a doctor and or had restrictions. Discovered due to Part 50 Audit.

During pre-shift exam. a roof fall was discovered. The fall was in the cross cut. Fall measured 160' long X 16' wide X 7-8' high. No persons trapped, no roadways or airways were blocked. The fall was approx. 2400' outby the working section.

Employee claims that while installing bolts in #1 entry a rock fell and struck him on the head, neck and shoulder. The employee worked until 2-13-10 when he went off work. The employee has had numerous back surgeries before this incident. The comp. board hearing concluded his incident would have contributed to his back problem. Discovered during Part 50 audit.

While welding on an airlock door the employee was loading materials into the scoop bucket and fell striking tailbone on the scoop bucket. The employee did not inform the safety department that he went to doctor and was removed from work or had restricted duty.

EE was installing a connecting pin in the cable sled toe plate when he dropped the pin and pinched his finger between the lip of the pin and the sled toe plate.

While roof bolting a small piece of sharp rock fell and struck EE on shoulder. This incident was not reported to mine management until 12/14/09.

An unplanned fall occurred at break 44 cross cut between #5 & #6 entries. The fall was approx 40'l X 18'w X 7'h. The area had been cribbed, timbered and dangered off at the mouth of the cross cut on both sides of the cross cut.

EE was walking, slipped and fell onto a water line.

At approx. 6:30a.m. a fall above anchorage was discovered at break #44 near spad 1108 and 1109. The fall was approx. 40' long x 11' high x 18' wide. No roadway, airway or escape ways were blocked. No injuries occurred.

While filling the rock dusting maachine with rock dust, Injured complained with pain to his groin area.

While installing roof bolts a piece of rib fell and struck ee on the back and right arm. EE was transported to General Hospital where he was treated and released and referred to a specialist. To this point a diagnosis has not been provided by the doctor or the specialist.

A fall was discovered at approximately 8:32 AM in the #4 entry on the Laurel Mains. The fall is approximately 18' wide X 20' long X 6' high. Located approximately 2700' outby the active working area.

During an inspection a fall was discovered along the return airway on the Tom's Branch section. The fall was located between spad 600 and 605 in an intersection. Fall measured approximately 20' W x 20' L x 10' H. The fall was discovered by MSHA & Mine Management.

While walking along the beltline. Entry Injury stepped between two rocks and stumbled, in doing so he fell against the rib and accidently pulled rock from the rib when fell and struck him on the left ankle. He didn't report accident until 5/13/09 to his forearm.

The EE claims that he bent over to pick up an empty bag that was laying in his locker and he felt a pain in his lower back. The EE then dressed for work and reported to work. After reporting to work, he stated his back hurt too badly to work. At that point the EE went home. Note: "Accident occurred in the bath house".

During the shift, while walking the Airways, mine foreman and another EE discovered 2 unplanned roof fall's on Tom's Branch. The falls are located in the Return Airways near spads 598 and 604.

During a preshift inspection the fall was discovered in #4 right entry on Laurel Mains section.

While cutting plastic shrink wrap from a pallet of rock dust. Injury was holding the plastic wrap with his left hand and cutting with his right hand. In doing so he cut his left hand.

EE had his right index finger on the pot area when he raised the pot to install a roof bolt. His finger was pinched between the drill pot and the roof bolt plate.

While operating a 4 seat mantrip, injured claims to have run over a rock in the roadway. When he ran over the rock, he claims to have felt pain in his back. He continued working until 1-31-09, at which time he went to the dr and the dr removed him from work for 3 days.

While training number 6 belt, injury struck his left index finger on a gob switch.

While stepping from the roof bolter, injury felt pain in his back, He completed his shift. While he was off work for the holidays. He went to the hospital on 12/26 for pain in his back, x-rays were negative and he was referred to a special in VA, who had preformed surgury on his back approx. 2 years previously.

While backing up/moving the miner, a rock that was lying on the miner fell from the miner and struck the employee on the right foot.

Employee was cutting plastic wrap from around a new bolt starter box and he struck his right leg with the knife.

While trimming a piece of conveyor belt, the knife slipped from the belt and struck EE on the right thigh causing a small laceration.

Raw Coal Area Plant - Outside upper part of yard. Employee was working on a dozer transmission when he placed a wrench on a slate bar to line the wrench up on the transmission. When he grabbed the slate bar pieces of metal were on the bar. The metal pieces went into his left hand middle finger.

EE was running SC and hooked back fender in #7 entry. When EE turned corner his canopy caught the corner, broke off a piece of rib and it came in the deck and hit his head and left leg, causing injury.

EE was working on a 980 Cat Endloader brakes. He took off the master cylinder top, his hand slipped, cutting his right ring finger knuckle. The cut started getting infected, he went ot the doctor on 5/21/08. No stitches.

Employee had hand laying on top of pump when a piece of rock fell and hit him on the back of his rt hand cutting it.

Employee was pulling choker cable on miner to hook up pull rope. Metal wire in choker cable went into left hand cutting it.

While tramming scoop, employee hit hole jamming his head into canopy of scoop. Accident was reported on 4/3/08.

Employee was making a cable splice using a cable anvil and a piece of the anvil chipped off cutting his right hand.

Employee got in a hurry to pull cat head and didn't knock breaker first. Power arced and burnt thumb and index finger on right hand.

While bolting in #2 face, ee had set the outside bolt and swung bolter head in to set outside bolt in that row. A piece of rib rolled off and hit his left ankle. 001 section.

While lowering drill head down after setting a bolt, the bolt wrench came out of drill head and hit the roof bolter operator on the chin.

EE was pulling a piece of rock off top of Joy miner when his hand slipped and was cut on edge of rock.

While putting a new sprocket on feeder the sprocket was being hit with a hammer. A piece of metal broke off (chip) and hit employee in the left cheek.

EE was moving shuttle car in #6 entry, so ribs could be rock dusted. When shuttle car caught a rib board which flew back and struck him in the right ear and check.

Employee was tramming scoop outside on roadway at Break 13-M when he ran over a block of wood that had fallen out of scoop bucket.
