Employee ascending stairs was struck by material dislodged from handrails by an employee descending. Dust entered left eye behind safety glasses. Employee rubbed eye, resulting in corneal abrasion. Treated with OTC eye drops; prescription cream provided as a preventive measure only, not for treatment.

Mostrando los 8Employee was installing a flange, when chip plate fell, causing the flange to fall, striking EE's left thumb.
Employee was in the process of putting a bolt in a steel beam. With EE's left hand holding the bolt in the top bolt hole. As the left structure piece was closing the gap between the end plate and the main beam the tip of EE's left thumb was briefly caught in the gap.
Right thumb tip was pinched between pallet jack and dunnage.
Grinding on duct, raised face shield, debris got in right eye
Miner was lifting 75lb vibrators to install into vibrator brackets on side of outlet hopper on a railcar. EE placed 3 into their resected brackets but when went to lift the 4th vibrator was struck with sharp lower back pain. Railcar is 37" height and 32" from ground to bracket, and 16" deep. Area was free of debris and without any trip hazards.
An EE was closing a silo gate, in a vertical motion, and got left pinky finger caught in between the gate and the metal frame as it closed. The EE sustained a small fracture to the small bone in the pinky finger tip.
Employee was helping to load I-beams on a dolly. EE lifted the end about a foot and a half and set on dolly. EE felt shooting pain during lift and EE's hip to ankles felt bad. EE continued to work until end of shift at 7:00 pm. Management was made aware of the incident on December 23rd.
