IE was assisting in placing an I-Beam on top of a shield during LW Recovery. A sharp edge on the I-Beam cut into the IE's left hand. IE was took to have the cut looked at. IE received stitches and returned to work full duty.

IE removed the keeper that holds the pan line roof bolter to the eichotrac. The pan line bolter fell off of the pan line and squeezed IE between the bolter and the pan line. IE was brought to the surface and took to the hospital for treatment. MSHA was called immediately after determining the severity of the injury.

IE was picking up a "sailboat" out of the pan line on LW setup when EE felt the muscles in EE's lower back tighten up. IE finished the shift and declined medical attention. IE woke up the next morning and decided EE needed to be checked out.

Employee was pre-oping a loco and when EE went to lift a cover EE felt a pop in EE's right shoulder. EE was brought to the surface and took to be checked out.

IE was going through a mandoor between the track entry and return. Used right hand to push against the frame as they pulled the door open with right. The door slipped out of hand and closed striking right index, middle, and ring fingers.

IE was changing rollers on a beltline when EE felt a pain in EE's lower and middle back. IE did not request attention until the end of EE's shift.

IE was laying out belt structure for a belt move when they felt a pain in right upper leg. EE did not report the injury until 5-20-20 when they requested medical treatment.

IE was installing a bolt in a "pot" to support it. As the IE was applying up pressure to drill the hole IE felt a pain in shoulder. IE finished the shift and did not request medical attention until 4-25-20.

IE was carrying material on the slope when EE slipped and fell. When the IE picked up the material EE felt a pain in EE's lower back and a numbness in EE's left hand. EE wanted to be brought out and assessed by a doctor.

IE was shoveling on the belt line when IE twisted right knee. The IE felt a pop and pain in the knee. IE was then brought out and transported to the Dr.

EE was loading belt structure into a scoop bucket. IE stepped with left foot into the scoop bucket and lost footing twisting a knee. EE continued to feel a pain in the knee. EE was then brought out and transported to the Dr.

IE was cleaning up the gate floor. The IE went to drag a wash down hose when they slipped causing them to twist their back. IE was brought outside and transported to a local hospital to be checked out.

IE was going through a mandoor into the return. IE did not get completely through the door before it shut and it caught EE's foot. As the IE pushed the door open EE felt a pop in the left hand and a sharp pain. IE did not want to go to the doctor until the next shift.

IE was mixing block bond to seal a brattice when IE felt a pain in right shoulder. IE requested to be brought out to see a doctor. IE was diagnosed with a shoulder strain.

IE was carrying old structure off of a belt line. IE made the decision to take rollers from a frame by using another roller as a hammer. During this the IE smashed a hand between the roller they were using for a hammer and the top frame they were hitting. IE was brought out and transported to the hospital for further evaluation.

IE was removing a cover from the tram motors on a scoop to do permissibility with MSHA. As the IE reach out to get the cover and pull it they felt a pain in the neck and left shoulder. IE did not request medical attention until 2-17-20.

IE was pulling a hose on 11W belt when the hose got caught on structure. IE then pulled the hose with force and tripped. When IE finished the dusting setup EE noticed groin was hurting. IE was took to the Dr. and has not returned to work.

IE was washing on the slope when the hose slipped out EE's hand as EE fell at the same time. IE then got a cut on the right elbow. EE was seen by a doctor where EE received 2 stitches.

IE was carrying dust to a duster and walked into an I-Beam. IE claimed a headache and stiff neck so was taken to the doctor. IE was told to take some time off work.

IE was dragging a washdown hose over the left shoulder when it became caught on a rock. IE grabbed hose with left hand and jerked to free it. IE then felt a pain in the left shoulder. IE was seen by a doctor and has not returned to work.

Employee was working with pumpable cribs and was exposed to a mixture of concrete and water. Received burns on both legs just above the ankle.

IE was lifting a water prop to load on a car. When the IE picked up the water prop EE felt a pull on EE's left shoulder. IE was brought out and seen by a doctor.

A contract employee was helping set-up a winder. A second contract employee was in an elevated position when a metal plate slipped from EE's hand. The plate struck the injured employee on the shoulder. The following were not a factor: rules, regs., systems, equipment, PPE. Attitude may have been a factor. Did not lose time until September 4, 2019.

IE was turning a splash guard on the 6th floor of the Concord prep Plant. As EE was pushing the splash guard EE felt a pain in EE's left shoulder. IE missed no work until 8/16/19. After IE visited the Dr on 8/16/19 for EE's shoulder pain, the Dr took EE off of work but found no damage in EE's shoulder. Possibly a strained muscle that needed to heal.

IE was removing a piece of metal out of a chute. As IE pushed the piece of metal off of the side of the belt, IE felt something pull in IE's left shoulder. Still waiting on the Dr's report to see what is wrong with IE's shoulder.

IE was getting set up to make a belt splice & was removing structure. As EE lifted EE's side of the 60" top roller assembly, the employee on the other side pulled on it & IE dropped the top structure & pinched EE's left middle finger. EE required stitches in the end of EE's finger.

IE was hanging roof bolter cable. As ee went to hang a section on an orangs hook, ee missed the hook & as the cable came down it put a strain on right shoulder. EE didn't miss any work until 5/10/19.
