Injured was helping 2 other miners re-rail a 555 scoop at the 230 foot mark of the slope track. Two airbags were placed to lift the scoop, blocking was being installed to support the scoop to deflate the bags when a crib block shot out from underneath an airbag and struck the injured employees hard hat, resulting in a small contusion to EEs forehead and a sprain/strain to EEs neck.

Employee was exiting a mantrip when EE's right foot was stuck under the seat. When attempting to pull it free EE injured EE's right knee.

Injured was cutting down roof screen to unload "bagged" material at 3 block on 3 belt with a grinder and some loose rock that was in the screen fell out causing a laceration to the posterior side of EE's left forearm that required sutures to close. No problems with equipment or compliance issues, training or PPE.

Injured loading top belt structure into a belt tub in the Sugar Run Supply yard with the rest of belt crew. When EE placed the structure down the other employee pushed it over before injured's gloved finger was out of way and pinched it between structure and tub resulting in a chipped bone. No problems with equipment or compliance issues, training or PPE.

Employee was on crew changing rollers, and EE's hand was pinned between a roller and a locked out belt while replacing a roller in a cradle. Accident was not the result of a training or compliance issue.

Injured picked up approximately 15-pound rock off of catwalk to the Sugar Run Metso Pump and felt pain in lower right abdomen. Initially treated for appendicitis, but no evidence of this was found at follow up appointment, so employee was diagnosed with an unspecified abdominal muscle strain. No problems with equipment or compliance issues, training or PPE.

Injured employee was picking up a slope chain and placing it on the slope car when they felt a pain in their back. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Injured picked up a lump of coal off the mine floor near 150 shield (6W) to throw into pan, but EE was closer to the pan than EE thought and struck EE's hand on the edge of the pan, pinching right ring finger between pan and coal, resulting in a fracture and open wound. No problems with equipment or compliance issues, training or PPE.

Injured was uncoupling a supply car incorrectly at the slope chute track and got EE's finger caught between the release lever and the coupler shank causing a crushing injury to EE's left middle finger that resulted in an open wound which required sutures to close.

While removing fire suppression from the scoop charger during a power move in St Leo Mains (121 blk 2-3) the clamp slipped and when EE went to catch it EE ran EE's gloved hand down the edge of a roof strap resulting in a laceration that required sutures. No problems with equipment or compliance issues, training or PPE.

Injured was rolling a shuttle car tire from the scoop bucket to the side of the shuttle car to install the tire and felt pain in EE's right shoulder (7 West 1 left entry, 0 to 1 block). No problems with equipment or compliance issues, training or PPE.

Injured was changing winch rope on duckbill scoop in 7W (75 blk 1-2 crosscut). They had their left hand resting on the mast of the scoop and threw the winch hook onto their own left thumb, causing an avulsion to the thumb nail. No problems with equipment or compliance issues, training or PPE.

Injured removed 70" roller and set it along the rib near the St Leo mantrip station, when the roller fell over and struck their right middle finger pinching it between the roller and a flat car resulting in an open wound that required 5 sutures. No problems with equipment or compliance issues, training or PPE.

Injured bent over to pick up a powder bag with 4 16 oz. bottles of water in it and felt pain in EE's lower back from a muscle strain/sprain. No problems with equipment or compliance issues, training or PPE.

Injured was picking up a rock that was caught in the grizzly bars of the loading machine at 42 block for the Harvey rehab project when EE felt pain in EE's left arm/shoulder and neck. Started missing work 08/20/25. No problems with equipment or compliance issues, training or PPE. Previous history with neck/shoulder/arm pain.

Building block wall between track and return entries between #3 and #4 seals of motor barn seal set and felt pain in upper chest area while lifting a block above EE's head and twisting. No problems with equipment or compliance issues, training or PPE.

Injured was de-trashing the 7W section in the #2 entry at 56 block and while attempting to pull a piece of curtain out that was stuck in the mud EE felt pain in EE's lower back. No problems with equipment or compliance issues, training or PPE.

Loading bags of rock dust onto FCT dust rack between 54 and 55 block #1 entry and felt pain in EE's lower back and burning in left side of lower back. Was not scheduled for work 7/22/25 due to roof fall on 3 belt. Diagnosed with muscle strain. No problems with equipment or compliance issues, training or PPE.

Disassembling equipment doors at 73 blk 2-3 6W that were laying on scoop bucket. While removing a support leg the long beam shifted and pinched their left ring fingertip between bucket and beam resulting in a fracture to the distal phalange and small open wound that required 2 sutures to close. No problems with equipment or compliance issues, training or PPE.

While cleaning rock from the walkway at 3 drive EE was lifting an approximately 50 pound rock onto the belt, struck their shoulder on the waterline and felt pain in their groin from a muscle strain. Sought medical attention and started missing work on 7/7/25. No problems with equipment or compliance issues, training, or PPE.

Passing T channel across miner in 7 West (face of #3) and slipped off step on back of miner hitting left arm on rib strap resulting in a laceration that took several sutures to close. No problems with equipment or compliance issues, training or PPE.

While installing a roof bolt off of the miner in St Leo Mains (3-2 107 block) the injured miner cut EE's left thumb (distal joint, anterior side) picking up a short T channel roof strap. EE was wearing gloves at the time. Received 4 sutures, given days off work because cut was on joint. No problems with equipment or compliance issues, training or PPE.

Injured taking a K wall panel out of an O-Belly (trash car) located at 100 block St Leo, that was stuck so they pulled harder and felt pain in shoulder. Continued to work until 9/25/25 when they started missing work pending surgical repair of a tear. No problems with equipment or compliance issues, training or PPE.

Injured was walking through personnel door at 83 block St Leo (track to belt) and shut EE's left hand in the door resulting in an open fracture of the distal phalange to EE's left middle finger and minor lacerations to index. Was wearing gloves. No problems with equipment or compliance issues, training, or PPE.

Employee strained left hip installing a top roller into structure on a 7W belt advance on 5/5/25, did not report injury until 5/8/25 when they went to seek medical attention and missed one day of work for a muscle strain. No problems with equipment or compliance issues, training or PPE.

Injured was picking up and carrying 5-6 16' cable bolts (with help) in the 3 North rehab supply area and felt sharp pain in groin. Diagnosed with inguinal hernia. No problems with compliance issues, equipment, training or PPE.

While holding a rib strap in vertical position to install into the rib (7W #3 face, 18-19 block) the oily strap slid down causing a laceration to the distal end of left index and middle fingers, index requiring 5 sutures to close. No problems with equipment or compliance issues, training or PPE (was wearing gloves).

Injured performing electrical inspection of 43 bolter (9 north mains 103 block 3-4 crosscut) and felt pain and a bulge when picking up trailing cable. Diagnosed with hernia and scheduled for surgery. No problems with equipment or compliance issues, training, or PPE.

While taking belt out of 5 West storage unit and lapping it onto an Irwin flat car the injured stepped off of the car, placing EE's hand on the coupler, when a lap of belt was descending towards the coupler and pinched EE's left hand, causing a laceration to EE's thumb when EE pulled EE's hand out. No problems with mining equipment, compliance issues, training or PPE.

Injured felt pain in right shoulder while sliding slider tube forward (face of #2 in 6W) and then felt pain in upper back around shoulder when lifting new tube up into place. Diagnosed with repetitive motion type injury to right shoulder girdle. No problems with equipment or compliance issues, training or PPE.

While installing a bacon strap, Harvey Run Rehab area at 42 block, to secure screen, and pushing strap into place when the strap got caught in the screen stopping the strap, causing EE's hand to slide along the edge, resulting in a laceration to EE's right index finger requiring 2 sutures. No problems with equipment or compliance issues, training or PPE (was wearing gloves).

While cleaning loose coal out of a ventilation tube by the coal pile behind the miner in 9 north mains (#2 entry) a lump of coal rolled down and struck their left knee resulting in a contusion. No problems with equipment or compliance issues, training or PPE.

Employee was carrying old belt structure off the beltline. EE's hand was caught between a piece of the structure and a pipe when EE set it down. Injury was not the result of a training or compliance issue.

Lifting ventilation tube in 9 north mains #1 entry between 98-99 block to slide on slider. When rotated their left foot got stuck and their upper body and upper leg rotated causing pain and a strain/sprain to their left knee. No problems with equipment or compliance issues, training or PPE.

While carrying an air drill down 10 belt from 53 to 52 block to install belt clips the injured felt pain and burning in their lower back, resulting in a strain/sprain to the soft tissues of the lumber area. No problems with equipment or compliance issues, training or PPE.

While driving mantrip down Main West Haulage a small amount of roof material dislodged, with some of the material getting behind EE's glasses into EE's left eye. EE rubbed their eye and caused a laceration/abrasion to EE's left cornea. No problems with equipment, compliance issues, training or PPE.

Injured was in the 60 pound track spur at Sugar Run unloading a roll of roof mesh with a partner and felt pain in lower back and down right leg. No problems with equipment, compliance, job skills, proficiency, training, or PPE

Injured employee stepped towards a stationary 650 scoop to unhook the winch hook and hit lower left leg on the scoop bucket causing a laceration, that later got infected and started missing work on 7/30/24 to treat the infection. No problems with training, proficiency or PPE issues, and no equipment or compliance issues.

Employee was assisting others install track rails. When EE attempted to move a rail, it moved abruptly and caught EE's right middle and rings fingers between the rail and the mine floor. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

The injured was bolting on a dual boom bolter putting in 16' cable bolts in 9 North Mains. The EE was on the last bolt, when the glue broke in the hole. The EE pulled it out and threw it towards the rib. The injured struck arm on the ATRS, causing a fracture. Began missing work 5/2/24.

In the Sugar Run Supply Yard an EE was lifting up on a draw bar to unhook it from an equipment dolly, when EE felt pain in EE's left shoulder. EE worked until 4/15/24 when EE began missing.

The injured and another EE were loading straps on the miner. The injured put the inby side of the strap up on the miner to be slid forward. As EE turned, two straps slid off striking EE in the right leg. The EE finished shift and began missing work 2-29-24.

The EE was in the process of dragging and installing 4" aqua pipe in the cut-through seals. While doing this felt pain in lower back. Sought treatment on 1/8/24, RTW on 1/9/24. EE was prescribed physical therapy.

The injured was carrying monorail outby on 4W belt line. As EE sat down a piece, EE's left middle finger was between the monorail and bed rail of the belt; smashing EE's finger. EE received 10 stitches and fractured the end of their finger.

The EE was on the track side of 4 ML Belt at 44 block. EE was bending down putting tools away in a bag. The belt crew was on the tight side of the removing old rollers. Another hourly EE threw a roller towards the track side, striking the injured in the back. EE began missing 10/30/23.

Injured was sheaving an old motor from the crosscut near Sugar Run Bottom when the sheave wheel broke and a piece of the sheave wheel struck the injured in the left forearm causing a contusion. No problems with equipment or compliance issues, training or PPE.

While pulling rope slack back from a motor their glove got caught in the rope and pulled their hand partially into a sheeve wheel causing a contusion and abrasion to their left ring finger. No problems with equipment or compliance issues, training or PPE.

EE was picking up an air bolter from 27 block Main West Haulage. The injured lifted the drill and twisted. Felt discomfort in lower back. EE began missing worked on 4/25/23.

EE was assisting loading and shipping belt structure and water line. When the injured raised the end of the water line up on the belt, felt a pain in their right bicep. EE finished the shift and began missing work 4/20/23.

The injured EE was center bolting in the #5 entry of 9 North Mains. The area was high approximately 15'. The EE's drill steel became lodged in the hole. Before EE was able to get it out, it fell from the mine roof and struck EE's left hand near EE's thumb. The EE finished EE's shift, but began missing 3/11/23.
