While a miner and EE's partner were building a sand wall, the center cap came down coming in contact with the miner's leg. Resulting in a hairline fracture to the tibia.

EE elected to have a procedure to fix the small hernia on 7/3/2023. EE returned to work after procedure on EE's next scheduled shift of 7/8/2023.
An employee was waiting to get truck loaded, after taking a drink from water jug the employee was putting the water jug back in its holder on the truck and an S hook that was attached to a rope on the water jug, swung and hit the employee on right front tooth causing it to break.
EE injured back when EE attempted to lift a 6" x 8' piece of thin wall HDPE pipe.
An employee was loading a mini transformer into the back of a vehicle, when employee got it about half way up the employee's shoulder made a popping sound. The employee did not seek medical treatment at that time. The employee's shoulder did not get better so employee was evaluated on 06/17/2021 by a physician at which time employee was put on restricted duty.
Employee was unloading 4'split set bolts in a laydown. As EE was stacking them, one of the bundles slipped and when EE tried to catch it, EE's left hand ring finger got caught under the bundle of bolts and smashed the end of it.
An employee was helping load a trench round in 15-E, EE was bent over loading a hole in front of the vent bag and the EE next to EE was filling a hole full of stemming material, while doing so some of the dust from the dry material blew in to EE's face getting some into EE's right eye.
Employee felt a sharp pain in the right side of their neck and right shoulder while pushing a 2200# Tote. The employee did not seek medical attention until December 10th after reporting discomfort while trying to sleep. The examining physician suspects a strained rotator cuff and has restricted work to a 20lb weight limit and no over head lifting.
EE was lifting a wooden board while cleaning up the laydown. When EE lifted the board, EE twisted to side to put the board back down. As EE did this, felt a tear in groin area.

An employee was spotting a co-worker who was positioning a pallet with a forklift when a VF bearing assembly with shaft rolled off of a pallet located next to the pallet EE was positioning and smashed the employees left foot.
Employee had arms extended while EE was helping the miners hang chain link fencing to get ready to bolt. When the employee brought their arm back to their side EE felt a pain in their right elbow bicep area. The employee was taken in to the medical clinic for evaluation.
Employee was attempting to remove a mickey bolt from the back with a 4' drill steel, when the mickey broke loose, the 4' drill steel came back and struck the employee in the forehead.
Employee cut his left hand while handling a 15 gallon grease barrel. He was assisting another employee put in the barrel on the lube truck when his hand contacted one of the opening tabs on the lid. The cut required sutures to close.
The employee strained his right wrist while removing an oil cap for the 630. The employee originally returned to work with no restrictions but eventually required wrist surgery in March of 2016.
Employee cut his left forearm on a metal mat while reaching for a split set plate. The metal mat was part of the ground support he was installing.

The employee received Shotcrete burns to his Genitalia and upper thigh. It is believed that the Shotcrete on his shirt spread via perspiration to the affected area.
Two employees were entering the man riding bucket when one employee kicked muck/shotcrete off his boot, the injured was splashed by wet shotcrete that entered his left eye behind the side shield. The injured began to wash the eye immediately and was later taken to the Dr. He received medical attention in the form of a prescription and had a few days of restricted duty.
E was placing jackleg drill onto bolt deck after completion of bolting. While laying drill down into holding cradle E's right hand got pinched between drill and holding cradle causing lacerations to his right index finger that required sutures.
E was welding and grinding at #1 Zn conditioner in Mill. A fan was being used to add extra ventilation. When E stopped grinding and lifted his welding hood there was loose debris on the hood that fell away from the hood and the fan blew the debris into his face with a foreign object becoming embedded in his left eye.
An employee was removing Bucket Bits from a loader, as he was removing the nut from the plow bolt the bolt pushed out of its recess causing the bolt to spin. The employee put his hand on the head of the plow bolt and tried to remove the nut, at that time the bolt spun again causing the sharp edge of the bolt head to cut through his leather glove lacerating his finger.
E was lifting bundles of split bolt plates by hand transferring them from a loader bucket to a shipping container. while lifting bundles he felt a "pop" and pain in his right wrist. E had right wrist put in a splint and is on restricted duty.
EE received cement burns to both feet while walking through water after the shotcrete crew cleaned off the cement trucks.
EE received a chemical burn to his foot while pouring sand in the stope. EE waded through water with cement in it that went over his boot.
The employee was picking up a Jackleg Drill off of the ground and strained his lower back.
Employee was cleaning out the Pioneer shaker screen using a piece of metal. The piece of metal slipped and smashed his little finger on the left hand.
Handing wooden post to partner and it slipped. Reached out to grab it and sliver enter finger. Became infected.
EE was opening a bundle of 4' split set bolts, as he broke the 1 inch strap on the bolts, it hit him in the right forearm causing a laceration.
EE was packing two bundles of monster mats and two bundles of split set plates to an area they were preparing to bolt. Previous shift had bolted in the area earlier in the day. As the EE bent down to set the mats and plates down, the top bundle of split set plates fell and struck a 4' split set leaning up against the rib. The bolt stuck him in the face.
The employee fractured his left thumb when it was pinched between the flanges on the ventline during installation in the shaft.
