EE reported on 1/13/15 that he was loading a smoker into his pickup at home and fell on the trailer hitch ball. On 1/14/15 he said it happened on the job assisting another employee, hooking up a pallet to a front end loader. They argued and another ee grabbed ee and threw him on the ground, breaking a rib.

Mostrando los 11Employee was clearing rock out of the primary crusher feed hopper and smashed his finger between a rock and the metal feed hopper. His left fifth finger was lacerated, requiring seven stitches.
EE was attempting to remove a guard from a tail pulley underneath a bin. When the guard came off suddenly, it struck him in the mouth, causing a 2 cm laceration of his upper lip, requiring stitches.
EE was trying to lift a 3" electric pump to check a problem. EE was swinging the swing gate as EE was cranking the cable lift. EE let go of the gate, causing the pump to drop. EE then tried to stop it by grabing the crank handle and the rotation of the handle caused a laceration to the palm of his right hand.
Employee was helping clean trapped material from behind the driven sheave on the primary crusher. He was manually turning the sheave while another employee knocked the material out. He caught his left index finger between the sheave and the crusher frame, fracturing the finger.
Employee was helping install a handrail on a piece of crushing equipment and caught his hand between the handrail and the bracket, cutting the palm and little finger of his left hand.
UNKNOWN- EE THOUGHT HE GOT SOME DUST IN HIS EYE SOMETIME DURING THE DAY. BY THE NEXT MORNING IT WAS WORSE- TURNED OUT TO BE A PIECE OF METAL.
EE WAS DOING MAINTENANCE AROUND CRUSHING PLANT, SOMETIME DURING THE DAY GOT SOMETHING IN HIS EYE.
EE WAS REPLACING HAMMERS & GRATE BARS INSIDE A SECONDARY CRUSHER, APPARENTLY GOT A PIECE OF METAL IN HIS EYE.
EMPLOYEE WAS FIXING A TWISTED V-BELT ON THE MAIN DRIVE OF THE PRIMARY CRUSHER. WHEN THE BELT FLIPPED OVER, HE CAUGHT HIS LITTLE FINGER BETWEEN THE BELT AND THE DRIVE SHEAVE. THE TIP OF HIS FI NGER WAS FRACTURED AND CUT. HIS FINGER WAS SUTURED AND SPLINTED.
EMPLOYEE WAS ATTEMPTING TO PUT A KEEPER PIN IN TO HOLD UP THE BED OF A 25 TON HAUL TRUCK AND SMASHED HIS FINGER BETWEEN THE BED AND THE HANDLE OF THE KEEPER PIN. THE TIP OF HIS MIDDLE FINGER O N THE RIGHT HAND WAS FRACTURED AND CUT. HIS FINGER WAS SUTURED AND SPLINTED.
