EE was dismounting the haul truck (Euclid R-35)using three points of contact. EE stepped down on a piece of rock and felt a pop in right foot. EE has a metatarsal fracture in right foot.
Employee was walking down the stairs which provide access to the front end loader and employee's foot slipped, causing employee to fall on employee's back. Employee has 4 non-displaced fractures of transverse processes, lumbar vertebra 1, 2, 3, 4.
EE was attempting to attach a chain from a front end loader bucket to a trailer to lift it up and remove jack stands. The front end loader moved forward and ee stepped back, stumbled and fell on back. EE has a cracked T7 vertebrae.
EE reported on 1/13/15 that he was loading a smoker into his pickup at home and fell on the trailer hitch ball. On 1/14/15 he said it happened on the job assisting another employee, hooking up a pallet to a front end loader. They argued and another ee grabbed ee and threw him on the ground, breaking a rib.
Employee was using a shovel cleaning up spilled material. He slipped in the mud and twisted his left knee.
Employee was checking the oil on the track hoe that he was operating. His foot slipped and he fell against the track, fracturing two ribs.
Employee was adjusting a parking brake on a haul truck. He was using a pry bar that slipped and he fell back against the frame of the truck, fracturing his 10th rib.
Employee was clearing rock out of the primary crusher feed hopper and smashed his finger between a rock and the metal feed hopper. His left fifth finger was lacerated, requiring seven stitches.
EE climbed down from his haul truck and stepped on a piece of rock, causing him to fall backwards.When he reached back to break his fall he caused a slight fracture to his right wrist (distal radius).
EE was attempting to remove a guard from a tail pulley underneath a bin. When the guard came off suddenly, it struck him in the mouth, causing a 2 cm laceration of his upper lip, requiring stitches.
EE was trying to lift a 3" electric pump to check a problem. EE was swinging the swing gate as EE was cranking the cable lift. EE let go of the gate, causing the pump to drop. EE then tried to stop it by grabing the crank handle and the rotation of the handle caused a laceration to the palm of his right hand.
Employee was using a pry bar. The bar slipped and he hit his hand on the machinery he was prying on, causing a slight fracture to the back of his left hand.
EE was fueling the loader when she stepped down on the berm around the fuel tank, she lost her balance and fell backward, injuring her left wrist.
Employee was cleaning rock out of the primary crusher & smashed his toes on his right foot. He received first aid treatment and returned to work. On 5\19\08 at a follow up doctor visit, it was determined that poor circulation would require the amputation of three toes, making this a lost time accident.
Employee was helping clean trapped material from behind the driven sheave on the primary crusher. He was manually turning the sheave while another employee knocked the material out. He caught his left index finger between the sheave and the crusher frame, fracturing the finger.
EE was using a 3 pound hammer to try & free a stuck troughing roller on a belt conveyor. The conveyor was running at the time. The conveyor belt grabbed the hammer and pulled his arm into the head pulley, causing his right arm to be severed.
Employee was helping install a handrail on a piece of crushing equipment and caught his hand between the handrail and the bracket, cutting the palm and little finger of his left hand.
EE WAS CLEANING OUT THE PRIMARY IMPACT CRUSHER, WHICH HAD STALLED, USING A PRY BAR. A LARGE ROCK ROLLED UNEXPECTEDLY, SMASHING HIS FINGER AGAINST ANOTHER ROCK. THE FINGER WAS CUT AND REQUIRED STITCHES.
UNKNOWN- EE THOUGHT HE GOT SOME DUST IN HIS EYE SOMETIME DURING THE DAY. BY THE NEXT MORNING IT WAS WORSE- TURNED OUT TO BE A PIECE OF METAL.
EE WAS DOING MAINTENANCE AROUND CRUSHING PLANT, SOMETIME DURING THE DAY GOT SOMETHING IN HIS EYE.
EE WAS REPLACING HAMMERS & GRATE BARS INSIDE A SECONDARY CRUSHER, APPARENTLY GOT A PIECE OF METAL IN HIS EYE.
EMPLOYEE CLAIMS THAT IN EARLY OCT. HE WAS OPERATING A DOZER PUSHING ROCK, CLEANING UP PIT FLOOR.HE STATES HE WOKE UP THE NEXT MORNING & HIS LEGSWERE NUMB. HE NOTIFIED US ON 11/10/00 AFTER AN MRI SHOWED A HERNIATED DISK. HE'S BEEN OFF WORK SINCE 11/8/00. SURGERY PERFORMED ON 11/13/00.
EMPLOYEE WAS FIXING A TWISTED V-BELT ON THE MAIN DRIVE OF THE PRIMARY CRUSHER. WHEN THE BELT FLIPPED OVER, HE CAUGHT HIS LITTLE FINGER BETWEEN THE BELT AND THE DRIVE SHEAVE. THE TIP OF HIS FI NGER WAS FRACTURED AND CUT. HIS FINGER WAS SUTURED AND SPLINTED.
EMPLOYEE WAS ATTEMPTING TO PUT A KEEPER PIN IN TO HOLD UP THE BED OF A 25 TON HAUL TRUCK AND SMASHED HIS FINGER BETWEEN THE BED AND THE HANDLE OF THE KEEPER PIN. THE TIP OF HIS MIDDLE FINGER O N THE RIGHT HAND WAS FRACTURED AND CUT. HIS FINGER WAS SUTURED AND SPLINTED.
EMPLOYEE IS A MECHANIC & WAS WORKING ON HIS TRUCK. HE WAS STANDINGON THE TRUCK FRAME RAIL AND HIS FOOT SLIPPED OFF, CAUSING HIM TO FALL AND STRADDLE THE RAIL. HE AS DIAGNOSED WITH BILATERAL IGNUINAL STRAIN AND PERINEAL CONTUSION.
EMPLOYEE IS A MECHAIC AND HAD BEEN CRAWLING UNDER EQUIPMENT TO REPAIR. HE COMPLAINED OF KNEE PAIN AND WAS TREATED FOR BURSITIS WHEN KNEE DID NTO GET ANY BETTER, HEFINALLY WAS SEEN BY AN ORTH OPEDIC SURG. DIAGNOSIS TORN MEDIAL MENISCUS. WILL BE SURGICALLY REPAIRED ON 6-29-00. OFF WORK STARTING 6-29-00.
