Mostrando los 21A can of starter fluid was tossed to employee. He missed catching it and it hit a motor, bounced off and stuck him above his left eye. He had 6 stitches above eyebrow to close up the cut.

EE WAS UNLOADING A BED ROLLER WHEN HIS RIGHT HAND GOT PINCHED BETWEEN 2 OF THEM.
He attempted to lift a 4D battery & strained back.
THIS ACCIDENT WAS JUST REPORTED TO US THIS WEEK (4/22/04). EMPLOYEE NEVER MENTIONED ANY INJURY WHILE EMPLOYED. WE HAVE NO DETAILS ABOUT WHEN OR HOW THE INJURY HAPPENED OR ABOUT ANY TREATMENT RECEIVED. EMPLOYEE SAYS INJURY HAPPENED 8/10/03, A SUNDAY, WHEN THE PLANT WAS NOT WORKING. HE WORKED THE FOLLOWING 6 DAYS AND WAS FINE.
EMPLOYEE CALLED 3/7/03 TO SAY HE HAD A SHARP PAIN IN RIGHT THIGH. HE SAYS HE MAY HAVE HURT IT BY WALKING THE "WRONG WAY" ON THE MUD BELT ON 3/5/03 - OR HE MAY HAVE HURT IT 2 WEEKS AGO WHILE WO RKING ON THE DRILL TRUCK TOWER. NORE: ON 2/21/03 EMPLOYEE CALLED IN TO SAY HE HAD HURT HIS GROIN AT HOME THE NIGHT BEFORE. HE WAS OFF WORK UNTIL 2/27/03.(SAME TIME HE SAYS HE HURT IT ON DRILL
EE WAS WORKING OFF THE PADDLE WHEEL DREDGE, PULLING A CABLE TO WRAP AROUND A PIPE. HE CALLED THE NEXT DAY & REPORTED THAT HE HAD PULLED A MUSCLEIN HIS BACK.
EE WAS CLIMBING INTO A BOAT FROM DREDGE. THE HANDRAIL BROKE. HE GRABBED THE SPUD CARRIAGE TO KEEP FROM FALLING, AND THE HANDRAIL FELL ON HIS LEFT HAND BRUISING HIS THUMB AND INDEX FINGER.
EMPLOYEE WAS PULLING SAFETY GOGGLES DOWN OVER THE BRIM OF HIS HARD HAT. THE GOGGLES SLIPPED AND HIT HIM IN THE LEFT EYE, SCRATCHING HIS EYE.
THE EMPLOYEE DOES NOT KNOW HOW OR WHEN OR WHERE HE SUSTAINED HIS INJURY. WE HAVE ASSUMED RESPONSIBILITY FOR HIS TREATMENT UNDER THE ASSUMPTION THAT IT POSSSIBLY/PROBABLY HAPPENED AT WORK. HE R EPORTED ON 9/11/01 THAT HE WAS EXPERIENCING PAIN IN HIS LOWER LEFT ABDOMEN.
EMPLOYEE WAS ON THE MUDBELT. THE BACKHOE WAS PUSHING THE MUDBELT BACK. THE HEADLINE ROPE CAME LOOSE AND HIT EMPLOYEE ON THE LEFT ARM CAUSING A FRACTURE.
EE WAS PUTTING A SAND PIPELINE TOGETHER-PART OF PIPE WAS BEING MOVED WITH TRACKHOE. OPERATORS SWUNG PIPE AROUND AND HIT EE ON RIGHT THIGH CAUSING TRAUMA & HEMATOMA.
EMPLOYEE WAS WORKING ON CREW WITH DRILLD RIG OBTAINING SAND AND GRAVEL SAMPLES. HE WAS SHOVELING SAND AND GRAVEL SAMPLES WHNE HE HURT THE BACK OF HIS NECK AND LEFT SHOULDER.
EE WAS HELPING TEAR DOWN A GRAVEL PROCESSING PLANT. HE WAS HELPING LOWER THE FRAME FOR A CONVEYOR BELT. FRAME WAS SET DOWN BUT NOT SECURED. THE FRAME SHIFTED AND SLIPPED DOWN ON HIS FOOT.
