While poking a silo the employee struck the top of EE's left hand on the spout piece, resulting in discomfort. As EE was walking down the steps, EE hit the top of left hand as well, causing further discomfort.

Mostrando los 35Employee was performing housekeeping, when they picked up a cardboard box with parts from the floor. The box weighed approx. 35 lbs., employee complaint of lower back pain. Employee was taken to the ER for an evaluation and was diagnosed with a lower back strain.

Employee was closing the shop (garage) door with the chain mechanism. The support cable broke, causing the door to free fall to the ground. With the momentum of the door going down, the chain whipped and wrapped around the right hand and ring finger of the same hand, causing a cut on ring finger.

Employee was coming down ladder on #2 dozer and got to the bottom step and it swung in, and hyper extended left knee. Employee was taken to E.R. and put on restricted duty.
Employee was cleaning #2 water softener bin when a chunk of salt fell from the side of bin and hit left shoulder and EE fell against metal door opening. EE had an abrasion / hematoma on left tricep. The Dr. restricted EE's work duties
In the maintenance parts warehouse near the east door. EE was accessing a piece of pvc conduit near the wall, when he bumped A 30# fire extinguisher with his hardhat causing it to fall and hit his left forearm. He was taken to ER and received staples to close a four inch laceration
Strain to the right shoulder occurred while holding on to the AMFO tote's bottom liner outlet in an attempt to stop the leakage of AMFO while transferring from large to small tote bag.
Employee was diagnosed with a hernia when he saw a doctor for a physical checkup. His new-employee physical from 9/18/13 did not diagnose the hernia. After interviewing him, he said that he felt a pull in the groin area while moving a pallet on 11/25/13. He did not report the incident prior to his diagnosis. We assume this is the date of his injury.
Employee went to the top of water softener elevator to clean pipe and to make sure the it was switched to #1 bin. While he was up there a bug flew inside his right ear. He went to ER and the Doctor that removed the bug said there was a small hole in his ear drum ER Dr. also prescribed antibiotics
The operator pulled a 80 bag and felt a slight pain in his stomach. The injury was not reported until the end of the shift the next day. He said he was ok but was sent to the doctor on 3-1-2013 for evaluation and was put on restricted duty.
EE was working on the press when he felt a slight pull in his groin. He reported the incident to his supervisor and did not request a doctor visit at the time. EE reported pending surgery to the company on Feb 12th 2013 and was sent to the company doctor and was released on restricted duty.
The operator was stacking 50# blocks when he felt a pull in his back. The operator was questioned the next morning and he said he was ok and could work. Late into his shift on 5-2-2012 he said that he was hurting.
The operator was conducting cleanup work on a belt conveyor. He pulled the belt around by hand & caught his finger between the belt and the pulley which rolled somewhat less than 1/4 turn. Operators in the area said the conveyor was off and not running. It appears that his pulling momentum caught tip of the finger between the belt and the roller mashing it.

The EE was trying to tighten the belt on infeed to the sewing machine. When he pushed the pulley, the infeed belt grabbed his left thumb. As a result of that, he had bad cuts on his left thumb. The thumb required sutures.

EMPLOYEE WAS PULLING BAGS OFF OF CONVEYOR BELT AND PULLED MUSCLE.
EMPLOYEE WAS PICKING UP A 50 LB BAG OF SALT WHEN IT SLIPPED OUT OF HIS HAND WHEN HE TRIED TO CATCH IT, AND WHILE DOING SO FELT PAIN IN HIS LEFT SHOULDER.
