Employee placed hand in small place to remove chain and smashed right hand causing an abrasion and bruising. EE called the supervisor and then we sent EE to Urgent Care for treatment.

Employee was getting ready to refuel an Excavator. Employee placed the hose and pump handle from the fuel truck on the Excavator platform and was about to climb the ladder to fuel when the hose and pump handle fell from the Excavator striking the employee in the left cheek. The handle caused a small cut and contusion to the left cheek.

employee was loading a steel drill bit into the back of a truck. They propped the drill bit on the truck and placed hand down on the bed of the truck, while doing so the bit slipped off of where it was propped and landed on the employee's right index finger causing a contusion. The accident was reported and employee was sent to urgent care clinic to be seen by the doctor.

Employee getting into off road haul truck and felt pain in his back. He had previously been shoveling around the quarry. Employee went to medical clinic and received diagnosis of lumbar strain.

EE PULLED LEVER DOWN TO WATER HIS LOAD NOT WEARING HIS HARD HAT HIT HIMSELF IN THE FOREHEAD. IT TOOK 5 STITCHES.

BUMPED HIS HEAD ON PIECE OF METAL SMALL CUT.

REPLACING START9R IN TRUCK WHEN STARTER FELL ON FINGER AND FRACTURED IT.

EMPLOYEE WAS SH2VELING ROCK WHEN HE PULLED MUSCLE IN HIS RT SHOULDER

EMP WAS HOOKING BUCKET BELT ONTO CRUSHER WHEN IT SLIPPED OFF AND BRUISED HIS RT LEG

EMPLOYEE SCRAPED LEFT LEG ON ROCK BIN WHEN HE WAS UNLOOSENING A BOULDER WITH HIS LEG.

EMPLOYEE WAS CHANGING DRILL STEEL WHEN IT DROPPED AND SMASHED HIS FINGER REQUIRING STITCHES.
