Back strain while trying to lift a blower onto a frame.

When the employee got injured, he was sanding a shaft to a tail pulley of a conveyor belt. The employee had the shaft proped up about two inches off the floor to sand the shaft. The shaft slipped off the prop and crushed the employee's left middle finger. Employee should have been using a different means to prop the shaft up when sanding.

Employee was lifting crusher parts. This job is done by more than one person.

WAS CLEANING DIRT FROM UNDER MOVING TAIL-PULLEY WHEN BELT SNAGGED THE HANDLE OF SCRAPER AND MOVING HANDLE STRUCK HIM ACROSS HIS FACE AND HEAD.

EE WAS TRYING TO UNJAM PORTABLE CRUSHER, STRAINED HIS BACK (HAS A HISTORY OF BACK PROBLEMS AT FORMER EMPLOYER). DID NOT SEEK TREATMENT UNTIL 3/23 - REPORTED PROBLEM HAS WORSENED ON 3-24-2000.

EMPLOYEE PICKED UP 4 FT PIECE OF 3 1/2" O.D. BORE HOLE (METAL) CASING AND STRAINED BACK AS HE TWISTED TO WALK AWAY FROM PILE OF CASING.

LIFTING FEEDER INTO PLACE.

WAS IN ACT OF REMOVING LID-LINER PLATES FROM VSI CRUSHER-MOVED HAND & THUMB SLID ALONG EDGE OF "WORN" BOLT HEAD

WAS USING SHOVEL TO CLEAN UNDER CONVEYOR BELT AND TWISTED/PULLED BACK. SUFFERED LUMBAR STRAIN.

HANDLING MOTOR AND STRAINED BACK.

ROLLIGN AROLL OF BELTING MATERIAL IT ROLLE DBACKWARD INTO HIS FOOT, BENDING SAFETY TOE SHOE UP AND BACK AGAINST HIS FOOT.

WAS TRYING TO LOOSEN PIECE OF ROCK STUCK IN CRUSHER CHUTE- PLACED HAND BETWEEN ROCK AND SIDE OF CHUTE. WHEN ROCK PULLED LOOSE, PINCHED HAND AGAINST METAL SIDE OF CHUTE (WAS WEARING GLOVES, GLO VE NOT CUT OR TORN).

PULLING ON HOOK AT CRUSHER, IT SLIPPED FROM HANDS, AND FELL ONTO FOOT POSSIBLE FRACTURE TO TOES AND FOOT.

CLIMBED DOWN FROM HAUL TRUCK & TWISTED RT KNEE IN PROCESS

SHOVELING ICE AND SNOW OFF TRAILER.

CLEARING ROCK JAM IN CRUSHER, ROCK AT HIGHER LEVEL FELL & PINCHED HAND AGAINST OTHER ROCK, 1ST, 2ND & 4TH FINGERS OF RIGHT HAND LACERATED, REQUIRED STITCHES.

DOING MECHANICAL REPAIRS TO TRAX MACHINE OVERHEAD DIRT DISLODGED & FELL INTO FACE, SOME ENTERED RIGHT EYE.

EMPLOYEE WAS SHOVELING DIRT FROM UNDER A CONVEYOR BELT & TWISTED/PULLED BACK.

EMPLOYEE HAD BEEN SHOVELING WHEN HE BEGAN TO COMPLAIN OF PAIN IN THE LOW BACK.

EMPLOYEE WAS LIFTING A CRUSHER SHOE DURING CLEAN-UP WHEN HE FELT SOMETHING PULL IN HIS GROIN. EMPLOYEE WAS NOT ASSIGNED TO THIS DUTY AT THE TIME THE ACCIDENT OCCURRED. HE JUST THOUGHT HE'D HEL P OUT.

CLIMBED ONTO HOPPER EXITING HOPPER COMPLAINED OF PAIN IN L FOOT ROCK HAD FALLEN ON FOOT
