EE was removing a broken coil spring from a jaw crusher. EE thought had released all tension on the spring, but bottom broken section was still under tension. As EE was removing spring, bottom section released smashing LT thumb.

Mostrando los 25Employee was removing/unjamming rocks from a cone crusher. While adjusting the rocks one fell on the tip of EE's right ring finger causing a small laceration and swelling.
Employee was placing metal screens onto a screener box. While placing the 3rd screen of 4, EE caught EE's left thumb between the screen and the screener wall. This resulted in a broken left thumb.
Employee was working on tail pulley when it slipped and partially fell on left thumb. Left thumb badly swollen - clinic said ice only - no stitches - no broken bones - was given narcotic for pain.

Employee was lifting crusher screen when he dropped it against left shin causing scrape.
Employee pulled metal shelf down onto his RT thumb employee went to ER and was treated.
Employee leaned on hot muffler with palm of left hand. Employee went to E.R. and was treated. Employee said he felt fine and reported back to work on 9/13/13 with limited duty.
Employee said he was lifting equipment and his lower back was hurting. Disclosed that he has been treating at same chiro for non-work related injury of lower back.
Employee was lifting a bucket of aggregate for testing lab when his back went out needing chiropractic visit.
Employee was changing crusher dies when the welded hook fastened to the die failed. Legs of the employee were trapped underneath the crusher die.
EE was assisting in setting up the crusher and said he strained his back. Said he was ok, this claimed turned to reportable because ee is seeing a chiropractor.
EE was lifting blocks when he felt slight pain in stomach went to MD 9/11/2009 diagnosed with hernia. Able to work until surgery can be scheduled.

EMPLOYEE WAS IN THE PROCESS OF CHANGING SCREENS. HE HIT HIS LEFT KNEEE AND SHIN ON THE BEARING AND GEAR RESERVOIR. HE RECEIVED A CONTUSION TO HIS KNEE AND SHIN (SWOLLEN).

EMPLOYEE CALLED & STATED HE HURT HIS BACK AWHILE AGO. HE SAID HE NORMALLY GOES TO THE CHIROPRACTOR 2 OR 3 TIMES & IT HELPS THE PAIN BUT IT DID NOT HAPPEN THIS TIME. HE IS IN SEVERE PAIN & WALK ING AWKWARDLY.
EE WAS CHANGING SCREENS WITH HIS CREW. THEY WERE MANUALLY LIFTING THE SCREENS. LATER THE NEXT DAY, HE NOTICED A LUMP IN THE ABDOMINAL AREA.
RESETTING CONVEYOR WITH LOADER AND LIFTING CHAIN. CHAIN WAS UNHOOKED AND SWINGING WHILE LOADER OPERATOR WAS BACKING UP. CHIN LATCHED ON THE CONVEYOR WHILE LOADER WAS STILL BACK UP, SWINGING TH E CONVEYOR INTO EE AND LANDING ON HIS RIGHT FOOT.
LIFTING AND INJURED/ STRAINED RT HIP.
