EE was removing a broken coil spring from a jaw crusher. EE thought had released all tension on the spring, but bottom broken section was still under tension. As EE was removing spring, bottom section released smashing LT thumb.

Mostrando 50 de 62miner was attempting to break a stuck boulder on a jaw crusher feed chute using a pry bar. While striking downward EE's RT thumb was crushed between the bar and toe plate of the platform EE was kneeling on.
Employee was removing/unjamming rocks from a cone crusher. While adjusting the rocks one fell on the tip of EE's right ring finger causing a small laceration and swelling.
Employee was welding a nut to a bolt and did not wear welding helmet for about 2 minutes. Employee experienced welder's flash to both eyes and eye pain/burning occurred about 12 hours later.
Miner felt dizzy/sick so miner parked and got out of the front-end loader. After getting down from the loader miner passed out. A co-worker came to miner and called 911. An ambulance took miner to the nearest hospital. EMS said miner reported having chest pains. The hospital is still analyizing results.
Worker experienced an allergic reaction and exhibited redness/swelling in forehead and cheeks. Worker was given a prescription by DR to alleviate symptoms. DR Diagnosis: Unspecified contact dermatitis. The cause of the reaction is unknown. No lost time.
Employee was placing metal screens onto a screener box. While placing the 3rd screen of 4, EE caught EE's left thumb between the screen and the screener wall. This resulted in a broken left thumb.
Miner was walking down the stairs of a tool trailer carrying tools. The first step of the stairs had been repaired with grating but when the miner stepped on the step the repair came loose causing the miner to lose footing. The miner dropped what EE was carrying and grabbed the railing with right hand. As EE caught self on the railing, EE's weight twisted right wrist.
Employee was kneeling and putting a belt onto a crushing machine. Employee heard a "popping" sound from left knee and tried to stand up. While trying to stand ee felt pain and fell back to the ground.
Employee was heating up bolts with an acetaline torch. Employee had the torch in their left hand and crossed their right arm over the flame burning their right forearm from elbow to wrist.
Employee said right eye was irritated. Employee taken to Davis Duchr eye clinic. Clinic said there was no foreign object in the eye. Eye was flushed and was told to put drops in and it would be fine. Employee does wear contacts which might have scratched the lens.
Employee was about to climb onto loader ladder when employee felt pain in lower back.
Employee was working on tail pulley when it slipped and partially fell on left thumb. Left thumb badly swollen - clinic said ice only - no stitches - no broken bones - was given narcotic for pain.
Employee said right eye was irritated. EE was taken to eye clinic. Clinic said there were no foreign objects in eye. Eye was flushed and EE was told to put drops in and EE would be fine. Employee does wear contacts which might have scratched lens.
Employee was walking on ice when EE slipped and fell and EE's rt. wrist.
I.E. was closing door on screen plant and pulled door against middle finger on rt hand.
Employee was changing cutting edge on loader bucket with impact wrench when box wrench holding nut slipped off nut and bolt shaft spun on palm of left hand.
Employee was lifting crusher screen when he dropped it against left shin causing scrape.
Employee complained of sore neck and upper shoulder. Went to doctor and will re-evaluate on 08/14/14.
Employee sliced right thumb while doing repairs. Employee is still working.

Employee has filed for hearing loss claim under worker's compensation. employee could not file claim until he was removed from exposure. He retired on 8/30/13 at 3:00pm. No lost time
Employee was taken to ER for possible heat stoke symptoms. Told to drink plenty of fluids by medical staff. Employee said he felt fine and was going to report to work tomorrow (7/16/13) will monitor.
Employee notified management on 2/21/13 that he had went to doctor on 1/25/13. Diagnosis was possible carpal tunnel in both hands. No treatment plan as of 2/21/13. No lost time as of 2/21/13. Will update 7000-1 as needed.
Employee said he was lifting equipment and his lower back was hurting. Disclosed that he has been treating at same chiro for non-work related injury of lower back.

Walking over to loader #193, he slipped on ice, injuring his low back. Heavy rain with night temperatures below freezing.
EMPLOYEE WAS IN THE PROCESS OF CHANGING SCREENS. HE HIT HIS LEFT KNEEE AND SHIN ON THE BEARING AND GEAR RESERVOIR. HE RECEIVED A CONTUSION TO HIS KNEE AND SHIN (SWOLLEN).
