Employee was removing debris from cutter in cutless area. When removing a large rock the rock shifted, smashing the employee's left middle finger. Employee was wearing gloves. Went to clinic for evaluation and received five stitches. Employee was also placed on restricted duty.

Became reportable on 7/28/25 when employee had hernia surgery. Employee was attempting to put return roller back into bracket by raising the roller with their legs. The roller then came out of the bracket on the right side and fell to the ground. The employee jerked their legs when the roller came out and they felt instant pain in the left groin area.

Employee was pulling a 2" hose, 50' long to wash out sand from under the tail pulley on #14 belt. EE's back began to hurt after. On 12/06/24 it still hurt, and EE reported EE wanted to go to the doctor where EE was diagnosed with a lower back strain & placed on restricted duty.

Employee was removing & rolling up approximately 800 feet of 1 inch cable from starboard anchor winch when EE felt a pop in their right forearm; causing immediate pain to EE's forearm, bicep & shoulder. Employee was taken to clinic and placed on restricted duty.

Customer truck driver smashed their left thumb in the tailgate of their truck when cleaning off excess 57 gravel.

Employee was replacing screens on the shaker deck above the load out bins on the rock plant. While installing the new screen, the injured employee and another employee were lifting the screen into place when the injured employee felt a pop in the right upper bicep.

Employee was reaching into a job tool storage box for a tool and while pulling arm out EE caught right elbow on a sharp corner of an aluminum welding rod box cutting EE's elbow.

Employee was assisting with the removal of conveyor leg, the bottom portion had been cut through with a torch. They started to remove the upper section and the leg sprung forward, striking the employee on the hand.

A Miner was attempting to uncoil cable from a suspended spool, when it became disconnected from its rigging, striking the Miner. The Miner sustained neck and leg injuries

An employee tossed a bolt to a coworker, the bolt struck the coworker in the head causing two lacerations that required stitches.

Employee was moving a portable walkway.

Unloading Screens out of a jon boat and the wind caught the screen.

Employee was putting panels on the back of the boat to work on rudders.

Picked up rollers off of the bank to carry down to the dock.

PICKING UP OUTRIGGER PLATE. PICKED IT UP TO PUT IT IN THE HOLDER. IT SLIPPED AND FELL OUT OF HOLDER. HE TRIED TO CATCH IT AND THE PLATE SMASHED HIS FINGER BETWEEN THE FUEL TANK AND THE PLATE.

LIFTING SPUDS ON BARGE SOMETHING CAME OFF CABLE AND GOT IN MY RIGHT EYE.

EE WAS UNLOADING 6 INCH PIPE FROM TRUCK. CO EE PULLED ON PIPE AND END FELL OFF RACK STRIKING INDEX AND MIDDLE FINGER ON LEFT HAND CAUSING SMASHED FINGERS.

WORKERS WERE INSTALLING NEW CABLE ON BUCYRUS-ERIE CRANE. THE LOOP WAS MADE IN CABLE TO GO AROUND THE WEDGE SOCKET. EMPLOYEE BENT OVER TO ASSIST AS ANOTHER EMPLOYEE PULLED ON CABLE. THE DEAD EN D OF LOOP UNFURLED, STRIKING EMPLOYEE ABOVE THE LEFT EYE.

WENT TO SET AN I-BEAM 8' DOWN ON DREDGE AND IT BOUNCED AND ROLLED ONTO TOP OF FOOT.BADLY BRUISED AND BROKE TOE ON RIGHT FOOT.

LFITING ENGINE HEADS OFF BOAT WHEN HE FELT PULL IN LOWER BACK. DID NOT GO TO DR UNTIL 10/8/91,

EMPL WAS INSTALLING DRIVE BELTS ON PULLEY OF NEWLY FABRICATED TAIL PULLEY ASSEMBLY IN MAINTENANCE SHOP. AS HE TURNED PULLEY BY HAND LITTLE FINGER OF LEFT HAND WA LACERATED ON END OF SHAFT ON G EAR BOX.

FOUR MAN CREW WAS REMOVING TIRE FROM 966C CAT FRONTEND LOADER TO REPLACE BEARING. ESTIMATED WEIGHT OF TIRE 950 LBS WHEN TIRE CAME OFF AXLE IT ROLLED PINCHING EMPL RIGHT HAND LONG FINGER BETWEE N TIRE & FENDER WELL CASUING LACERATION TO END JOINT OF RIGHT HAND LONG FINGER. REQUIRED THREE SUTURES & TETNUS SHOT. NO BROKEN BONES.

HE WAS INSTALLING A TORQUE CONVERTER ON THE 988 CAT LOADER AND WAS PUTTING THE FLY WHEEL BACK ON WHEN HE FELT THE PULL IN HIS BACK.
