Harrisburg South- Plant #14 Metal/Non-Metal

Employee was operating mobile equipment (CAT 980K loader & John Deere 460 Haul truck) and came to work the next day complaining of tenderness in EE's back. Employee went to clinic and was placed on light duty on 6/16/25. Employee was re-evaluated on 06/27/25 and released to full duty.

Became reportable on 7/28/25 when employee had hernia surgery. Employee was attempting to put return roller back into bracket by raising the roller with their legs. The roller then came out of the bracket on the right side and fell to the ground. The employee jerked their legs when the roller came out and they felt instant pain in the left groin area.

Employee was trying to tighten the tension adjuster for the tail pulley on the feeder. EE was using a 3ft pipe wrench to tighten the nut and applying pressure down when the wrench slipped causing EE to strike chin on the tail pulley guard. Employee received a laceration to their chin that required three stitches.

Customer truck driver smashed their left thumb in the tailgate of their truck when cleaning off excess 57 gravel.

Employee was doing repair work to oversize chute at the plant when ee went to lift a 10x2 piece of channel iron and it slipped from right hand and fell on left hand smashing left ring finger.

Employee was swinging the arm of a hoist over, to prevent a chair (in which they were going to hoist up to button house) from catching on the pea gravel conveyor, and while pulling on the bottom of chair it came loose and employee lost EE's balance and fell on EE's left elbow.

Employee was using a sledge hammer to remove a pin from track hoe bucket. While striking the pin, a piece of the metal pin sheared off into employee's right shoulder.

Employee was walking the conveyor system at a incline and began to fall and threw his right hand out to catch himself and fell on hand. (Per phone call he was working outside and fell to the ground. Hand was strained.)

Employee was rolling a huge clod of material down the stockpile, when he heard a pop from his right arm and he began to feel pain. He also had swelling in upper arm. He said the pain was tolerable the last few weeks but arm is really no better.

Cleaning return roller with shovel and return roller pulled shovel and right arm between conveyor belt and return roller bruising the right arm and wrist.

employee was using shovel to clean out under conveyor when he fainted. Dr. report dehydration.

Employee standing near conveyor belt,Rock bounce off and hit employee in eye.

Employee laid tools on screen up on vibrator and when screen moved, the tools started falling and employee reached to grab them, causing him to fall.

Employee was welding in shop on metal, flash from welder burnt both eyes.

Employee was shoveling on the conveyor line, began having pain in left forearm and wrist. As the day advanced pain became worse and arm swelled.

Fell while checking crusher.

Employee was shoveling under conveyor and his forearm got caught by screen wire.

EMPLOYEE WENT TO REWIRE ELECRIC BOX. HAD METER TO CHECK VOLTAGE. HAD ALLEN WRENCH IN HIS HAND. EMPOLYEE DROPPED METER AND ALLEN WRENCH HIT HOT WIRE CAUSING BOX TO BLOW UP. (POWER WAS OFF)

Employee was shoveling from under crusher when an oversize rock (2 to 3 inch rock) fell from crusher and hit employee on the right temple.

SCRAPER OPERATOR ENTERED PIT TO LOAD AND HAD SCRAPER IN THE WRONG GEAR AND LOST CONTROL OF MACHINE (EMPLOYEE WAS NOT WEARING SEATBELT) THE EMPLOYEE WAS BOUNCED AROUND IN THE CAB VIOLENTLY.

EMPLOYEE WAS TRYING TO HOOK FAN UP ON LOADER. THE BATTERY DOOR FELL OFF & HIT HIS HEAD, CAUSING AN ABRASION REQUIRING 6 STITCHES.

TRUCK DRIVER COMING ONTO SCALES LOADED. BLOWED OUTSIDE WHEEL AFTER WHEEL CAME IN CONTACT WITH SCALE RAILING. OPERATOR PULLED OFF SCALES STILL LOADED AND ATTEMPTED TO CHANGE TIRE, WITH TRUCK JA CKED UP, THE JACK SLIPPED HITTING THE DRIVER ON THE SHOULDER AND BACK AREA.

EMPLOYEE WAS WORKING ON SCRAPPER MACHINE AND A PART OF THE MACHINE HE HAD TAKEN OFF (BRACE) AND LAID AGAINST THE MACHINE TIPPED OVER HITTING THE EMPLOYEE BEHIND THE RIGHT KNEE (CALF MUSCLE).

EMPLOYEE WAS SHOVELING UNDER CONVEYOR NEAR RETURN ROLLER. THE ROLLER CAUGHT THE SHOVEL AND PULLED EMPLOYEE'S ARM UP TO THE ROLLER, CAUSING A MUSCLE STRAIN.

EE WAS WORKING ON A 627 SCRAPER, REPAIRING CUSHION HITCH, WHILE TRYING TO PUT A WOOD BLOCK UNDER CUSHION HITCH, CUSHION HITCH FLOATED DOWN AND CAUGHT HIS FINGER BETWEEN WOOD AND HITCH.

EE SHUT HIS HAND UP IN HIS VEHICLE DOOR.

EE WAS USING A CUTTING TORCH CUTTING SCREENS WHEN A PIECE OF METAL FLEW INTO HIS RIGHT EYE SCRATCHING HIS CORONA.
