While maneuvering a screw tub into position so it could be bolted into place, the crew noticed a clevis that needed to be removed that was still attached to the end of the tub. While the EE was attempting to remove the clevis, the tub shifted and pinched the EE's right index finger between the side of the tub and the catwalk.

employee was carrying toughing roller and stepped on a rock, lost balance and dropped the roller on their rt foot. EE continued to work stating they did not need to seek medical treatment. EE sought medical treatment the next day and was taken off work for rt foot pain and can return to work on 09/09/2019. EE has returned to full duty

Employee was picking up the center chute for a screening unit. When the employee picked up the unit, the employee suffered a left shoulder strain.

Employee cut the palm of right hand while handling a piece of metal.

Employee reported that while wearing safety glasses, ee got particle of foreign material between eye and contact lens.

Employee reported that employee hurt employee's back while installing a plate.

Employee noted that small telescoping boom on the JLG Manlift broke loose and suddenly slid down inside the bigger boom causing ee to hit chest on the control panel.

Employee reported EE was replacing sheave wheel on the 6X10 Diester screen when the wheel fell off the shaft and landed on right middle and ring fingers.

Employee said he was cleaning himself off and felt like he got something in his eye.

Employee reported that he strained his back while performing Jaw Crusher maintenance.

Employee reported he strained his back while climbing/ascending ladder into operator's cab of Komatsu PC-650 Excavator.

Employee was assisting replace loader bucket cutting edge. Cutting edge fell on his right leg.

Employee was in cab of haul truck. Employee reached back for a paper towel, then felt burning sensation in the middle of back.

Employee was loading section of steel railing into the back of service truck when his middle finger of his right gloved hand got caught between the metal and truck bumper.

Employee was attempting to open control gate with left hand, when the gate would not open, he pulled using both hands and then slammed his middle and ring fingers against a support beam.

Employee was helping install a culvert pipe near the Scales Area when he bent over to pick up a chunk of mud which was about to fall in the ditch. While reaching for the mud chunk, he felt something pull in his back.

Employee was working in the 8,s Bin reparing holes in a chute under the screen. He ask another employee to cut him some small pieces of metal to use to patch the holes. The employee cut the metal droped one piece into the bin when the employee doing the welding picked up the second piece was droped strikeing the second finger of the right hand.

Employee sprained his lower back picking up a metal pipe to put thru wall to put oil in crusher.

Employee was attemting to remove ramps from trailer when hinge rod slipped striking his right hand.

Employee was lifting screen housing out of wash take and placing it on work bench when housing slipped out of his hand causing his finger to be caught between tank and screen.

Employee was inspecting Pin Bushing when a metal bur cut his right index finger.

Employee was loading 50lb lime bags on pallet when he strained his back.

EE was shoveling walkway around the cone crusher when 1/2" water pipe fell, striking thumb of left hand.

EE WAS SHOVELING - DOING CLEAN UP WORK UNDER A CONVEYOR BELT, WHEN HE PULLED A BACK MUSCLE.

EE WAS PLACING OXYGEN TANK INTO RACK WHEN HIS SMALL FINGER OF HIS LEFT HAND MADE CONTACT BETWEENTANK AND RACK. THIS REQUIRED MEDICAL TREATMENT OF EIGHT STITCHES.

IN THE GARAGE, INT HE PIT BENEATH WATER TRUCK REMOVING 2 PC DRIVE SHAFT. THE 2 PC DRIVE SHAFT SEPARATED THE UNSECURED PC FELL AND STRUCK EE ON R. ANKLE. EE DID NOT THINK TOO MUCH ABOUT IT AND DID NOT REPORT OR ASK FOR MEDICAL ATTENTION. IT WAS FRI. ANKLE SWELLED UP OVER WEEKEND WITH PAIN. MISSED WORK ON MONDAY. REPORTED.

WHILE REASSEMBING 36" GORE CRUSHER AT SANDPOINT WITH 4 OTHER EMPLOYEES, RIGHT HAND WAS CAUGHT IN PINCH POINT BETWEEN LOCKING RING AND VERTICLE BOX SUPPORT POST WHILE TURNING THE LOCKING RING CLOCKWISE. CUT PALM ON RIGHT HAND.

WHILE SHOVELING GRAVEL & MUD FROM OUT BUILDING, STRAINED BACK.

WHILE CLEARING ROCK JAM IN FEEDER BOX AT 5 1/2' CRUSHER, ROCK SLIPPED AND STRUCK LEFT INDEX FINGER CAUSING CONTUSION AND LACERATION REQUIRING 6 SUTURES.

WHEN CLEARING ROCK FROM FEEDER BOX AT 5.5' CRUSHER CUT SMALL FINGER ON RIGHT HAND ON PIECE OF SHARP METAL EDGE.

WHILE UNLOADING PARTAIL OVERLOAD OF #57 STONE TAILGATE ON 1978 MACK DUMP TRUCK WOULD NOT CLOSE COMPLETELY DUE TO STONE LODGED BETWEEN GATE ANED BED. DRIVER ATTEMPTED TO DISLODGE STONE WITH LE FT HAND. STONE DISLODGED, GATE SLAMMED SHUT AND PINCHED TIP OF MIDDLE FINGER, SEVERING TIP, DRIVER DROVE TRUCK TO SCALEHOUSE AND TAKEN TO HOSPITAL BY RESCUE SQUAD.

WHILE ASSISTING MAINTENANCE CREW CLEARING FROZEN MATERIAL FROM CHUTE AT JAW CRUSHER, A CHUNK OF FROZEN MATERIAL [MUD & STONE] BROKE LOOSE & FELL APPRO 6' & STRUCK EE'S GLOVED RIGHT HAND PENETR ATING GLOVE & CUTTING BACKSIDE OF HAND.

AROUND NOON 9-19-96, EMPLOYEE ADVISED FOREMAN HIS BACK WAS SORE & REQUESTED DAY OFF. IT WAS GRANTED. 9-20-96 EMPLOYEE TELEPHONED HIS FOREMAN & ADVISED HE STRAINED HIS BACK WHILE SHOVELING LOOS E STONE FROM GYRODISC ROOM & THAT HIS BACK WAS VERY SORE. EMPLOYEE WAS DIRECTED TO THE LEWIS GALE CLINIC.

ACCIDENTLY LOCKED KEYS IN SERVICE TRUCK. WHILE TRYING TO RETRIEVE KEYS THRU SIDE VENT GLASS, GLASS BROKE AND CUT LEFT THUMB.

WHHILE PREPARING DECKING FRAME IN MAINTENANCE SHED A 4' SECTION FO 4" CHANNEL STEEL WHICH WAS STANDING VERTICALLY W/O SUPPORT FELL AGAINST LEFT KNEE.

WHILE CLIMBING INTO AN AIR SEPARATOR TO CLEAN OUT AT END OF WORK DAY PULLED MUSCLE IN LOWER BACK.

WHILE HELPING CLEAR A CHOKED JAW CRUSHER, PICKEDUP TOO LARGE A ROCK AND STRAINED LOWER BACK.

WHILE CLEARING ROCK FROM CRUSHER, ROCK FELL ON GLOVED LEFT HAND & CUT HAND. REQUIRED 3 SUTURES.
