Supervisor decided to help out EE's crew with shoveling under the crusher for about 20 minutes. EE felt some tension on EE's lower back but did not think much of it. The next morning on 05/27/2025 EE decided to rest it off because EE felt pain. EE refused medical attention stating that it would go away with rest.

Employee was replacing a body valve on the 966H and felt a strain on left shoulder that radiated pain. Employee did not think much of it and kept working. At the time of the incident, a co-worker was also there and knew how heavy the equipment parts were as well as the difficulty they were having in lifting the parts on to the equipment.

EE cut right hand, second finger from the thumb, underside of finger. EE was pulling a screen out of the back of a pickup. The screen got lodged in the bed of the truck while EE was pulling on it. There were no witness'. EE drove self to the Uvalde Memorial Hospital where EE received stitches and a Tetanus shot.

Employee removing rock stuck in crusher, rock was sharp and cut right pinky finger of employee. 5 stitches required.

Employee was lifting railroad tie out from under plant and strained his lower back.

Employee installing screen on screening plant, screen slipped and landed on middle finger of left hand, causing a contusion.

Employee helping carry oxygen/acetylene tank rack and when setting it down pinched finger between rack and rock.

EMPLOYEE WAS LIFTING A LARGE ROCK FROM THE CRUSHER, AND SUFFERED AN ABDOMINAL STRAIN.

EE WAS MOVING OIL BARRELS AND CAUGHT RIGHT FINGER IN BETWEEN TWO MOVING BARRELS.

EE WAS HELPING LIFT A CRUSHER HAMMER, POSSIBLE ABDOMINAL STRAIN.

EE WAS LOADING HOLES IN MUD WHEN LEFT KNEE TWISTED.

EMPLOYEE WAS CLEANING CRUSHER BIN, ANOTHER EMPLOYEE ABOVE HIM ACCIDENTLY KICKED A ROCK WHICH FELL ON EMLOYEE'S BACK.

EMPL WAS LOADING POWDER BAGS INTO HOLES & STRAINED BACK.

CHANGING PLANT SCREEN WHEN THE PERSON ON THE OTHER END LOST HIS HOLD & I.NAME TRIED TO HOLD ONTO IT & MASHED HIS FINGER BETWEEN THE SCREEN & PLANT FRAME
