EE was on foot. Pushed a stuck man lift they were following. EE initially strained left calf muscle but DID NOT seek medical attention. 8/17/18 EE sought medical attention due to pain shift/discoloration. Medical personnel diagnosed as left gastrocnemius strain and put EE on restricted duty. Follow up visit 8/21/2018 closed the case. Medical released the EE to Full/Regular duty.

Mostrando los 8EE had just completed swapping thrust roller on kiln. The EE removed employee's safety glasses to wipe employee's eyes. Employee believes a piece of debris came off EE's glove when EE rubbed EE's eyes. EE worked rest of day. Returning to work the following day, EE was sent for medical assistance due to redness and discomfort. Medical personnel swabbed EE's eye to removed object.
Employee was performing maintenance on a conveyor belt when sand particle got into employee's right eye. Sand particle caused irritation that subsequently turned into conjunctivitis.
Employee was dislodging stone lining blocks from mill shell in standing position. EE was turned and looking away when several blocks fell from the side of mill shell onto employee's lower, back, right leg. Employee's exertion of forward, downward pressure on right foot in reaction to block hitting back of lower leg caused a closed fracture to one bone on top of right foot.
EMPLOYEE WAS WORKING ON A DUCT CHUTE PATCHING HOLES. HE PLACED HIS PINCH BAR UP NEXT TO A DIVERTER GATE. AFTER WORK WAS COMPLETED, HE GRABBED THE PINCH BAR, WHEN HE DID THIS THE GATE SLAMMED SHUT PINCHING HIS FINGER BETWEEN THE BAR & THE DUCT. THEY WERE NOT WORKING ON THE GATE, HE JUST SET HIS TOOL NEXT TO THE GATE.
UNLOADING PALLETS, VENDOR HAD PALLETS IN MIDDLE OF LOAD AND EE WAS STACKING THE PALLETS THAT WERE SETTING UP ON END. THE PALLET FELL FROM TWOSTACKS HIGH STRIKING EE IN THE HEAD & RT.SHOULDE
LIFTING A 1 CUBIC FOOT BOX OF SAND ON TO SCALES.
CLIMBING DOWN OFF MILL #1 AFTER ADJUSTING FEED RATE, HE "TURNED TOO FAST" AT BOTTOM & ROLLED ANKLE. HEIGHT WAS 6-8".
