EE was pulling extra hose into a coarse material washer to wash it out for maintenance purposes and ran EE's right ring finger across one of the flights lacerating the fingertip. EE received 3 sutures making this accident reportable to MSHA.

EE was changing a steering cylinder on a haul truck. The cylinder slipped off the ear it fastens to and mashed the EE's left middle and ring fingers between the ear and cylinder lacerating the middle finger requiring stitches, thus making this accident reportable to MSHA.

EE was attempting to put a sheave on a pulley shaft that had been cut, the EE cut EE's left middle finger on a bur on the shaft. EE was taken to the urgent care where EE received stitches in EE's finger, making this reportable to MSHA.

EE was loading a hydraulic cylinder and was holding by the head cap, when EE dropped the cylinder to the floor the barrel collapsed mashing EE's fingers in between the eye and head cap. This resulted in broken fingers on both hands. EE has splints on 3 different fingers making this accident reportable.

EE was helping to change a return roller, claims they hurt a shoulder stating they didn't know they hurt it but that was the only task that could've hurt it. After some discussion we have decided to file a worker's comp claim until further investigation. EE has missed work because of this making it a lost time accident and reportable to MSHA.

EE was assisting installing impeller in a GIW pump. The impeller was sitting upright between the forks on the forklift. The impeller shifted a little forward on the forks, and the EE got in front of the forks to reposition the impeller. The impeller fell forward off the forks striking the EE in the right ankle and foot.

EE was lifting a bucket of ABC with EE's right hand and placed it on a hand truck. EE felt a sharp pain in EE's sternum. This was on Friday. On Monday, it was not any better, so EE went to the doctor and was diagnosed with a muscle strain and placed on light duty, thus making the incident reportable.

The EE was helping to change a return roller on the No. 1 belt. Another EE was holding one side, and the roller slipped out of EE's hands. The roller came down and hit the injured EE's hand, causing a fracture on the right hand.

EE was working on a side door on a step chute at the wash plant. The door hung up going down. Tried tapping on it to get it moving. The plate turned loose, slid down and hit the ring finger and index finger of the left hand, requiring stitches.

EEs were lowering jaw die into chute for wear purposes when the die got hung on the walls. They cut one side of the die off, the die fell, and the chain hit the EE in the right knee. The EE has a fractured patella on the right leg, causing the injury to be reportable.

EE was helping to change pulley on No.1 belt. Bearing on the pulley was stuck. The EE thought that they could free the bearing up with their hands. When they freed the bearing up, the pulley and bearing came free and pinched EE's fingers between conveyor frame. EE had to get Stitches in fingers, making accident reportable.

On Saturday, 12/5/15, EE complained of pain to his right hand and swelling. He was unsure as to cause. The doctors found metal in his hand on 12/13/15 from where he had cleaned out hydraulic tank on 12/4/15, which caused infection and hospitalization, thereby making it a reportable accident. Until 12/13/15, the doctors thought it could be a spider bite or boil under the skin.

On 3/5/2014, the EE was attempting to pull two 5-gallon buckets apart when he felt a sharp pain in his lower back, hip, side and leg. In the last couple of days, the EE felt a bulge in his right lower abdomen, with some pain. He was sent to the doctor and diagnosed with a hernia and placed on restrictions on 4/24/2014, thereby making it reportable.

EE was welding in a chute. He finished with one welding rod and attempted to throw it onto a shelf close to him. When he threw it, it bounced off a plate and came back into his left shoe burning his leg. Accident occurred on 7/11/13, but EE did not feel need to go to doctor until 7/23/13, which is when we got diagnosis of 3rd degree burns, making it reportable.

EE was hooking up a welding lead on the welding truck. He looked down to step off of the concrete where his truck was parked, and then looked up as he was stepping. As he looked up, he ran into a piece of angle iron that was placed on the truck that they planned to use on the job they were working on. The angle iron hit him on the left side of his face, requiring 8 stitches.

EE was helping to install new guard on a conveyor drive. While in the seated position, the EE felt a pain in his groin area. He stood up and felt a knot in his right groin area. Went to the doctor and had a hernia. The doctor restricted activity for 10 days to no lifting over 10 lbs.

EE was helping raise up guard rails on scales at the load out. One EE picked up their end of pipe and when the other EE picked up their end, it slipped out of their hands. The pipe fell and struck the EE in the right knee, requiring 8 stitches.

EE was helping place bracing on screen tower. The EE was holding the bracing against the screen tower, when the bracing came loose, swung out and struck his left hand.

Employee was closing the tailgate on a company pick up truck when he caught his right hand index finger between the right side of the gate and side of the bed.

EMPLOYEE WAS TAKING A SAMPLE AND FELT SOMETHING POP IN HER RIGHT SHOULDER

EE WAS WASHING OFF A TAIL PULLEY WHEN A ROCK FELLOFF THE BELT, STRIKING THE INDEX FINGER ON HIS LEFT HAND. HE WAS GIVEN A SHOT TO NUMB THE FINGER WHEN THE DR PUSHED THE NAIL BACK INTO PLACE. NO BROKEN BONES OR STITCHES

EE was unloading a d-7 dozer at the quarry. He was reconnecting the angle arm on the blade prior to unloaidng and felt a burning in his lower back.

EE WAS INSTALLING RETURN ROLLER ON BELT, PUSHING UPWARD WITH LEFT SHOULDER & FELT A POP IN SHOULDER. WENT TO DR & GIVEN SOMETHING FOR PAIN.EE RET'D TO DR 12/27 WHEN PAIN RETURNED. THE ACCIDENT BECAME REPORTABLE AT THAT TIME.

EMPLOYEE STEPPED ON A 3 FOOT PORTABLE LADDER, CAUSING THE LADDER TO SWING BACK AND STRIKE HIM ON THE LEFT KNEE. EMPLOYEE HAD SURGERY ON JAN 21, 2005. TO REMOVE A PIECE OF BONE THAT WAS IRRITATING LIGIMENTS.

EE WAS STTEMPTING TO OPEN AN INSPECTION DOOR ON A CRUSHER. HE OPENED THE DOOR WITH TOO MUCH FORCE STRIKING THE CRUSHER WITH HIS LEFT HAND. HE CONTINUED TO WORK THE 12TH AND 13TH. IT STILL WAS SORE ON MONDAY THE 16TH AND HE WENT TO THE CLINIC FOR AN X-RAY. HE HAD A CRACKED BONE IN HIS HAND. NO LOST TIME OR RESTRICTED DUTY.

INJURED MAN IS AN OVER THE ROAD TRUCK DRIVER, SELF EMPLOYED. HE WAS ADJUSTING HIS TAILGATE ON COMPANY PROPERTY WHEN HE GOT HIS RIGHT HAND CAUGHTBETWEEN THE TAILGATE AND TRUCK FRAME.

EE WAS STEAMCLEANING A PICKUP TRUCK WHEN HE ACCIDENTALLY SPRAYED STEAM DOWN HIS LEFT BOOT. NOTE: EE DID NOT SEEK MEDICAL AID UNTIL THE NEXT WEEK WHEN IT WAS DETERMINED HE SUFFERED 2ND AND POSS IBLY 3RD DEGREE BURNS.

EE WAS WASHING OUT UNDER A CONVEYOR WHILE KNEELING. WHEN HE STOOD UP HE FELT A PAIN IN HIS BACK.EE DID NOT SEEK MEDICAL ATTENTION BUT DID NOT COME BACK TO WORK ON JAN. 1 (NOT A HOLIDAY FOR US) .

EE WAS WORKING ON A SHORT HEAD CRUSHER LOOSENING A VALVE. WHEN IT CAME LOOSE IT DROPPED HITTING THE RING FINGER OF HIS RIGHT HAND. CUT REQUIRED 4 STITCHES. X-RAYS SHOWED NO BROKEN BONES.

2 EMPLOYEE WERE LIFTING THE ROOFF OF A SMALL SHED (100 LBS) WHEN IT COLLAPSED CAUSING A TWISTING MOTION TO ONE EE. HE FELT A PAIN IN HIS RIGHT GROIN. IT GRADUALLY WORSENED. HE SAW DR. 1/10/00. DETERMINED TO HAVE A HERNIA. NOTE: EE RETURNED TO HIS REGULAR JOB BUT IS SCHEDULED TO HAVE SURGERY LATER THIS MONTH.

EE WAS TRANSFERRING OIL FROM A BARREL INTO A BUCKET WEHN HE BARREL SLIPPED CAUSING HIM TO STRAINHIS LEFT ARM FROM THE ELBOW TOTHE SHOULDER

EMPLOYEE WAS HELPING CHANGE TIRE ON B70 EUCLID. HE WAS BRUSHING LOOSE DIRT FROM INSIDE WHEEL RIMWHEN HE CUT HIS FINGER ON A STEEL BAR MOUNTED ON TIRE RIM.

THE EE WAS MOVIG A DREDGE BY PULLING A ROPE ATTACHED TO A CABLE. HE PULLED A MUSCLE IN HIS NECK SHOULDER & ELBOW. NOTE: EE MISSED WORK ON 10/6 WHEN HE RECD MED TREATMENT THIS WILL BE LOST TIME EE NOT BACK TO WORK AS OF 10/20/99

SLIDING A PIECE OF STEEL ONTO THE FORKS OF THE FORKLIFT WHEN IT OVERTURNED & MASHED HIS FINGER BETWEEN THE TWO PIECES OF STEEL.

PLACING CHEEK PLATES IN JAW CRUSHER AND ONE PLATE HE WAS HOLDING SLIPPED AND CUT LEFT PALM.

EE PICKED UP THE BIT END OF A BROKEN DRILL STEEL AND THEN COMPLAINED OF A BURNING IN HIS STOMACH.

EMPLOYEE WAS MOUNTING A DRAGLINE AFTER GREASING A DUMP SHIEVE. HE WAS STANDING ON THE TRACKS STEPPING ONTO THE CATWAL WHEN HIS LEFT KNEE LOCKED UP.

EMPLOYEE HAD SHAFT AND REDUCER BLANCED ON WORK STAND AND ATTEMPTED TO HOOK THE OVERHEAD TO IT. SHAFT STARTED TO FALL AND HE TRIED TO CATCH IT. THE FLANGE ON THE END OF THE SHAFT HIT HIS ARM AND CUT IT. REQUIRING SIX STITCHES.

EMPLOYEE WAS CHOCKING HIS HAUL TRUCK TO PUMP UP A TIRE. THE WOODEN CHOCK BLOCK SLIPPED AND HE GOT A SPLINTER IN HIS FINGER. THE SPLINTER HAD TO BE CUT OUT REQUIRING 5 STITCHES.

EE WAS ASSISTING WITH THE LOADING OF A 15HP ELECTRIC MOTOR INTO THE BUCKET OF A BOBCAT LOADER. HE HAD HIS LEFT HAND ON THE MOTOR SHAFT WHEN IT SHIFTED, PINCHING HIS FINGER.

EE WAS ATTEMPTING TO THROW THE GATE TO CHANGE THE DIRECTION OF MATERIAL DISCHARGING FROM LOADOUTSCREEN. THE GATE WOULD NOT SHUT PROPERLY & HE CUT HIS FINGER ON THE SCREEN SPRING BRACKET AS HE TRIED TO FORCE THE GATE HANDLE CLOSED.

EE CAUGHT FINGER BETWEEN HORRIZONAL GRAB IRON LOCATED ON LEFT SIDE OF SEAT AND THE SEAT BELT BRACKET MOUNTED ON SEAT FRAME WHEN HE ATTEMPTED TO DISMOUNT. THE SEAT WAS DEPRESSED BY HIS BODY WEI GHT AND RAISED UP CATCHING HIS FINGER AS HE WAS GETTING UP.

EMPLOYEE WAS ATTEMPTING TO GUIDE A SCREW AUGER WHICH WAS SUSPENDED FROM A CHAN UNDER SOME BEAMS INTO THE FINE MATERIAL CLASSIFIER TANK AS HE WAS PUSHING DOWN ON TOP OF THE AUGER GUIDING IT UND ER THE BEAM IT SWUNG UPWARD MASHING HIS FINGER BETWEEN THE AUGER & THE BEAM

EMPLOYEE WAS INSTALLING AN OVERHEAD FAN ABOUT TWO DAYS AFTER HE HAS HANDLED SOME HEAVY SYNTRON FEEDER MAGNETS. HE FELT A TIGNLING IN ARM & SOME NUMBNESS OF RING FINGER.

ROCK FELL ON EE S HAND WHEN DROPPED BY A SECOND EE SUFFERING A HERNIA REQUIRED 6 STITCHES ON ONE FINGER.

EE WAS LIFTING HEAVY ROCKS OUT OF A CHOKED CRUSHER WHEN HE FELT A PAIN IN THE GROIN AREA, CAUSING HIM TO DROP THE ROCK ON ANOTHER EE HAND TWO INJURIES.

WHEN REMOVING A CROSSTIE FROM UNDER A DREDGE THAT HAD BEEN SITTING ON IT, THE CROSS TIE SLIPPED FROM INJURIED PERSONS HAND AND SLID ACROSS HIS RIGHT FOREARM PUSHING A SPLINTER INTO GAME.

WHEN PLACING A CABLE ON A DRAGLINE THE CABLE FLIPPED THROUGH THE GROUND. SAND WAS TOSSED IN THE FACE OF THE OPERATOR, GETTING SAND IN HIS EYES.

RAGLINE OPERATOR WAS REPLACING FAIRLEGED ROLLERS ON DRAGLINE WHEN ONE OF THE ROLLERS SLIPPED FROM HIS GRASP FALLING ACROSS HIS RIGHT ARM.

EMPL WAS CHAINGING CUTTING EDGES & SLIDES ON BLADE OF 814 DOZER WHILE HOLDING SLIDE IN PLACE TO PUT BOLT INTO PLACE SLIDE FELL & MASHED HIS FINGER.
