While employee was attempting to open a pipe gate, middle finger on right hand got caught in between a cross piece and the lock on the gate. When the EE tried to pull their finger loose, they ended up pulling off the tip of their finger.

Mostrando los 13Miner was changing engine air filters on Caterpillar 769C haul truck. When EE pulled the filters out of the housing, EE twisted at the waist to set them down on the deck of the truck. The miner felt a shooting pain in lower back.
Miner was pulling 50 lb bags of limestone and started experiencing pain in left shoulder and neck area.
Miner was on top of TAMKO bin shoveling 1 1/4"X 1/2" aggregate that had overflowed onto the bin top. When going into the rock pile with shovel, the miner felt a sharp pain in elbow.

M-5 Maint. Mechanic/miner was raising over-head garage doors to open shop. In process, miner felt pain along right "Ribcage" area. Miner thought he'd "Pulled" a muscle. Did not seek immediate treatment. Treated on 10/14/11.
Miner was positioning wooden blocks and sliding them (blocks), under the primary shell housing of the crusher. Miner had been working with other personnel, but decided to slide/spot the wooden block himself, straining his left shoulder. Miner did not seek immediate treatment, until mid-day (next date) 10/09/07 @ noon.
Miner was moving long sections of (approx. 21') steel piping from/between trailer and storage area. Miner thought this was how he was injured ? Miner had been instructed to wait for assisstance to arrive, but proceeded to move material on own. Accident/injury not reported until late Monday, Oct. 8th. Rolla Field office called, as well as (800) 746-1553.
Miner (Chief Electrician) was perfoming work on a conveyor (Weigh) belt, adding a load cell unit, when metal piece shifted slightly, causing an approx. 1" length laceration to the miner's index finger. Miner was transported to medical facility, where physician cleaned wound and applied (5) sutures. Miner released to full duty.
Miner, part of team changing Bull-ring at mill, strained his stomach muscle, R. side. Miner did not report accident immediately, thought pain would subside. Mngt. to reveiw and work to improve JSA. Accident report date 7/18/05, medical restrictions applied. Light duty. Will follow with Sec "D".
