While employee was attempting to open a pipe gate, middle finger on right hand got caught in between a cross piece and the lock on the gate. When the EE tried to pull their finger loose, they ended up pulling off the tip of their finger.

Miner was changing engine air filters on Caterpillar 769C haul truck. When EE pulled the filters out of the housing, EE twisted at the waist to set them down on the deck of the truck. The miner felt a shooting pain in lower back.

Miner was pulling 50 lb bags of limestone and started experiencing pain in left shoulder and neck area.

Miner was on top of TAMKO bin shoveling 1 1/4"X 1/2" aggregate that had overflowed onto the bin top. When going into the rock pile with shovel, the miner felt a sharp pain in elbow.

As stated by miner, shoulder discomfort has occurred to both shoulders, over a reported period of 10 years. Issue began gradual, but with worsening discomfort, over the last six years. This is the first time miner/powderman has officially sought diagnosis/treatment for reported issues. States after loading production holes for explosive detonation, shoulder hurts at end of day!

M-5 Maint. Mechanic/miner was raising over-head garage doors to open shop. In process, miner felt pain along right "Ribcage" area. Miner thought he'd "Pulled" a muscle. Did not seek immediate treatment. Treated on 10/14/11.

Miner was positioning wooden blocks and sliding them (blocks), under the primary shell housing of the crusher. Miner had been working with other personnel, but decided to slide/spot the wooden block himself, straining his left shoulder. Miner did not seek immediate treatment, until mid-day (next date) 10/09/07 @ noon.

Miner was moving long sections of (approx. 21') steel piping from/between trailer and storage area. Miner thought this was how he was injured ? Miner had been instructed to wait for assisstance to arrive, but proceeded to move material on own. Accident/injury not reported until late Monday, Oct. 8th. Rolla Field office called, as well as (800) 746-1553.

Miner (Chief Electrician) was perfoming work on a conveyor (Weigh) belt, adding a load cell unit, when metal piece shifted slightly, causing an approx. 1" length laceration to the miner's index finger. Miner was transported to medical facility, where physician cleaned wound and applied (5) sutures. Miner released to full duty.

Miner, part of team changing Bull-ring at mill, strained his stomach muscle, R. side. Miner did not report accident immediately, thought pain would subside. Mngt. to reveiw and work to improve JSA. Accident report date 7/18/05, medical restrictions applied. Light duty. Will follow with Sec "D".

MAKING STEPS WITH ROCKS AND STATES HE PUSHED A ROCK WITH THE BOTTOM OF HIS FOOT & HIS KNEE GAVE OUT. STRAIN TO LEFT KNEE.

WHILE PULLING CHAINS EMPLOYEE STRAINED NECK, SHOULDER, ARM.

AFTER WORKING ON CRUSHER FELT PAIN IN LOWER BACK.

EMPLOYEE PICKED UP AND THREW A ROCK OF APPROX. 20 LBS WHICH BLOCKED THE TRAVELWAY OF HIS DRILL. WHEN HE THREW THE ROCK, HE FELT A PAIN IN HIS RIGHT ELBOW.

EMP HAD BEEN PREPPING A TRUCK BED FOR PAINTING. WAS WEARING EYE PROTECTION BUT AFTER THE WORK WAS DONE HE REMOVED HIS GLASSES AND WIPED HIS FACE WITH A RAG. A SPEC OF RUST MUST HAVE BEEN BRUSH ED FROM HIS FOREHEAD TO HIS EYE.

UNKNOWN-MILLING PLANTS ALWAYS HAVE AIRBORNE DUST PLANTMAN MAY HAVE HAD DUST ON HIS HAND OR GLOVE WHEN HE RUBBED HIS EYE.

EMPL INSTALLING PULLEY ON 50 HP ELECTRIC MOTOR.THE MOTOR HAD BEEN RAISED ON ONE END BY STEEL BAR WHICH WAS AGAINST FOOT OF MOTOR.THE FOOT BROKE OFF,BAR SLIPPED LUT AND THE MOTOR DROPPED DOWN S MASHING EMPL FINGER BETWEEN MOTOR PULLEY &I-BEAM

LIFTING BULL HOSE ON HOOK FELT TWING IN BACK HAD NUMBNESS TO R LEG OF EMP

LINING UP 6" STEEL PIPE FOR WELDING,EMPL GOT METAL SPLINTER IN RIGHT THUMB.WAS UNABLE TO REMOVE SPLINTER AND THUMB BECAME INFECTED.

WHILE EMPLOYEE WAS LOADING A TRUCK UNDER THE PLANT BINS, A PIECE OF ROCK WENT INTO HIS LEFT EYE.

EMPLOYEE WAS IN8TALLING A WHEEL HUB ON THE FRONT AXLE OF A CLINE HAUL TRUCK. THE HUB SLIPPED AND EMPLOYEE CAUGHT THE BACK OF HIS HAND ON THE COARSE THREADS OF THE AXLE HOUSING.
