While attempting to pull a support shaft out of a grate, the shaft shifted and caught employees hand between the shaft and grating.

An employee positioning a used return roller for a conveyor pinched their left index finger, resulting in a laceration and sutures.

Employee was making a shovel cable connection when the cable rolled, pulling the employee's left arm forward. The employee experienced left arm pain and was evaluated at the local ED where images revealed an injury to their left bicep. The employee was prescribed work restrictions and is being accommodated.

Employee was hosing in the crusher bowl when the employee tripped and fell landing on their left knee. Employee continued to work and felt something running down their leg when they observed a cut below the left knee. Employee was evaluated at the ER and received medical treatment (stitches).

An employee attempting to clear a jammed chain for a chain fall sustained a fracture to their left index finger.

Employee sustained and unknown injury to the left forearm while manipulating a lifter bar from the inside of the #1 Balling Mill.

While moving a troughing roller frame assembly on 10 Conveyor, Line 3 in the Tilden plant, an employee pinched left thumb resulting in a laceration requiring 2 sutures.

Employee reported pain in their left shoulder while moving a case of boosters from a service truck to a cart. Employee was later prescribed restrictions 4-17-2025.

On 4/5/2025, an incident was reported where an employee took off their hard hat to brush away dust, subsequently getting material in their left eye. The employee had reported the incident their following shift.

While attempting to remove a wrapper rod, it fell down and struck employee in the right hand.

Plant repairperson was discarding worn return roller into dumpster when the employee pinched their left index finger between the return roller and the dumpster. The employee sustained crushing injuries to their left index finger resulting in medical treatment.

Maintenance mechanic ruptured their left biceps tendon while opening the cab door of haul truck 78. The cab door was frozen after the mechanic washed the truck in the wash bay.

An employee was removing a sheave when the sharp edge of the sheave sliced through the employees' glove and into their left thumb.

An employee was removing a cutting edge from a Cat 988 Loader when a sharp bolt head sliced through the employee's glove and into their left palm.

Employee was erecting a scaffold. Employee was attempting to pull two aluminum planks together and felt a "pop" in left arm.

An employee was removing a rock from the top of a grader blade when the operator experienced pain in their shoulder.

Employee was positioning a large clevis to perform a lift when the clevis slipped, causing the employee's left middle finger to be pinched between the clevis and the lifting tray.

Technicians were removing rollers from drill undercarriage. One of the rollers start to roll off support, tech tried stopping it and was unable. When the roller fell off the weight of the other rollers on the other side of the support caused it to raise pinching EE's hand between the support block and machine frame.

An employee pinched the pad of their right finger when installing a clamp for the for 4B Filter.

An employee was accessing an oil tank. When the employee lifted the lid, the employee felt pain in right shoulder. The employee sought medical attention, and was taken off of work.

Fueler was lifting a box of supplies using correct ergonomics when the strap on the box broke causing them to jar their back.

Crew was tasked with removing the hub of the conveyor head pulley. Employee placed a fire blanket over the stiff back and all-thread rod for precautionary measures. The rod snapped and swung upward striking right thumb.

Shovel operator deboarding S31 at shift change experienced pain in calf after hard landing of deboarding ladder. Evaluated by EMT, brought to Bell for further evaluation.

On 6/23/21 series of physical therapy started. 03/23/21 repairman was rigging a mill liner. The mill liner, tipped and fell to ground level. The plant repairman was standing to the side, was struck by a portion of the mill liner on their left knee.

Plant repairmen were working on the 12A Deslime Thickener secondary drive. During disassembly of the thickener drive there was stored energy present. When the drive mounting bolts were removed the drive rotated suddenly, striking the employee. Employee was seen by on-site EMS, NP and transported to UP Health System via ambulance.

EE was transferring cable from one reel to another. EE's hand got caught while guiding the cable with EE's hand pinching thumb between cable and reel. EE was taken to UPHS-Bell and treated for a thumb injury. It became known on 9-10-20 that there was a tiny fracture in thumb. Referred to orthopedics.

The employee was installing a used shank and tooth assembly on S30 shovel bucket. While pushing the shank on, it pivoted striking left thumb causing a laceration. The employee was seen by site responder, and then brought to hospital for further evaluation, where they received medical care. The employee returned to work.

EE was stacking Grizzly Bars to be cut prior to installation and had stacked 4 out of the 6 bars and while moving the 5th one, it slid and pinched EE's finger between the two bars, fracturing it.

Employee was welding in a fan and lifted welding hood and the slag popped and a small piece went in EE's eye. The piece was removed and eye was washed out. The next day the employee was taken to UP Health System to remove an additional piece of slag.

Pellet Plant Operating employee was hosing inside 1A fan duct at time of incident. While adjusting wash-down hose connection lost footing, slip and fell to floor causing pain to right knee. Seen by on-site EMS then evaluated in the ER. Returned to work with restricted duty.

Alleged occupational illness. Employee changed batteries on GR18. Employee reported lower back felt sore, while changing batteries; once sat down felt shooting pain in lower back. Employee did not want to be seen by site responder.

Employee was positioning cribbing and felt tightening of muscles in lower back. Evaluated by NP and given restrictions.

While performing routine daily work assignments of walking, mounting and dismounting equipment, employee states stepping up into a service truck (from ground to rear bumper/truck bed) and felt a pop in right calf. Employee was seen by on site EMT and taken to a hospital for further evaluation.

EE reported working on stock feeder belt when a pulley slipped contacting EE's hand causing injury. Evaluated in Emergency room.

Supervisor was assisting operator in the process of manually closing the South Decant Valve, after releasing the valve handle it sprung back striking employee above left eye. Employee was taken to UP Health System for evaluation. Received stitches to close laceration.

Employee was performing a count of shovel/loader teeth requiring to partially move the position of the teeth. Employee reported discomfort in left groin. Evaluated by surgeon and it was made known on 3-6-19 that employee had a hernia.

Employee was moving electrical cable and felt pain in groin.

Employee cut the bolts loose on a roller assembly and wiggled it free. When the roller assembly became free, it dropped down pinching employee's hand against a vertical upright edge of the drip tray that resulted in a laceration to the palm. Employee was evaluated at hospital where medical treatment was provided to close the wound.

Alleged: On 12-2-18, we became aware of employee alleging incident on 11-11-18 where employee claims employee pinched a finger handling grader cutting edges. Employee sought medical care on 12-2-18 for an infection in the finger. Treated with antibiotics. X-ray on 12-11-18 showed a fracture to finger tip. No treatment advised for fracture.

Employee was removing the saddle block sleeve when contacted by a bolt resulting to an injury to the forehead. Employee was seen by site responder and nurse practitioner, and then brought to hospital for further evaluation where medical treatment was provided to wound.

Shovel operator closed cab door on gloved left hand, thumb pinched. X-ray revealed tip of thumb fracture. No treatment indicated for this type of fracture. Returned to work.

Employee reports discomfort to right wrist/forearm area the employee believes secondary to shoveling during previous shift. Did not report until 3-18-18. Diagnosed with tenosynovitis. Put on restricted duty.

EE reported having discomfort in shoulder from attempting to connect fast fuel nozzle on T-71. Seen by site responder and worked rest of shift. Continued complaints of discomfort lead to further treatment. Had surgery to shoulder on 5-9-18.

Employee was inside LN12 primary mill performing lining. After moving a rock out of the way, employee stood back up and when doing so contacted a small shard of steel on the duckbill of the liner handler with forearm resulting in a laceration. Employee was seen by nurse and later received sutures at local hospital. Employee returned to work following treatment.

Alleged: employee next shift stated to nurse, EE's back is sore, that EE lifted a box of bolts the previous day. No report was made to management on date of alleged claim. Employee has long-standing documented physical health matters.

EE was moving impact rollers from a pallet to a storage rack when one of the rollers moved rolling on top of left hand. EE pulled EE's hand out and felt a pop and pain in the wrist. Evaluated by physician and returned to work with restrictions.

EE participated in several consecutive shifts of manual labor (bucket brigade) requiring repetitive motion. Employee picked up plastic pail containing scraps (approx. 10lbs). EE states felt a pop in right shoulder and experienced immediate pain down the outside of upper arm. Employee was evaluated by two EMTs, and then transported to Bell for further evaluation. Restricted duty.

Mechanic reported lifting a battery(about 30#)out of a loader and felt "pop" in right shoulder. Mechanic initially declined medical assessment, but later provided conservative care instructions. After a follow up assessment, restrictions were ordered on 1-30-17. Mechanic reports having removed numerous batteries recently. X-ray report showed multiple chronic changes.

Employee was installing sheeting and hit elbow on a stud on a scaffold head while pulling a piece of sheeting. No pain at the time of incident but three days later it became swollen and very painful.

After shoveling concentrate for several hours employee c/o neck pain. Discomfort was not resolving with conservative treatment so PT was ordered. Employee completed a series of two treatments on 2-9-17.
