Mostrando los 33Employee was cleaning up material at the base of the scalping screen due to spillage. Another employee was on top of loose material on top the screen to dislodge (approx 35-50 lb) rock. That rock fell, hit the guard, then dropped down about 4" onto employee's right shoulder.
Employee was lifting a full argon gas cylinder for the welder. Felt pain in lower back. Returned 1/08/21 - didn't report until 1/11/2021 - at that time we sent EE to the Doctor.

employee was stepping down out of man lift basket onto catwalk with a piece of metal to replace in the scalper. When EE stepped onto catwalk EE heard a pop and felt pain in the back of EE's lower right leg.

Employee was sent to the eye clinic where they washed his eye out and gave him eye drops. MSHA SITE DOWN 10/24 DUE TO MAINTENANCE said to try again today 10/27/14 for reporting.
unloading rocks out of primary housing with a platform hooked to Linkbelt. platform hung up on one end, employee stepped on platform, held the cables and tried to wiggle it free, the platform broke free causing a jerking motion. FIRST WORKDAY WITH RESTRICTIONS 7/07/14
Employee was helping to move a gate in a chute under construction. Employee was pushing down on the gate handle with his rt hand. When the gate handle moved downward the employee lost his balance, grabbing the corner of the chute with his left hand to catch himself, inadvertently causing his ring and middle finger to become pinched by the moving gate, causing lacerations.
Employee was rolling air hose onto reel by hand when a twist at the end of the hose came undone and hit him in the mouth, causing his tooth next to the front tooth to break off.
EMPLOYEE WAS LIFTING AND ROLLING ONE END OF AN OAK TIMBER OFF OF A LARGE CONCRETE BLOCK WHEN HE FELT A PULLING IN HIS RIGHT UPPER ARM.
employee was walking up on hill # 4 (with safety glasses on) to work on hydraulic drill, when he felt scratchiness, burning and watering of the left eye. foreign object in eye.
Employee was removing bent conveyor for repairs. While using a 12' x 12' timber to block the conveyor, the timber was pushed to far on one end causing his end to drop down onto angle iron which caught his pinky on right hand, which partially tore the fingernail from the nailbed.
EMPLOYEE WAS PUTTING A CONVEYOR BELT ON WITH A ROPE ATTACHED TO A PULLEY THAT WAS THEN ATTACHED TO A TRUCK TO PULL. THE PULLEY BROKE AND THE ROPE HIT THE EMPLOYEE IN THE FOOT/ANKLE (R).
EMPLOYEE WAS AT THE PLANT INSTALLING DUST SHIELD OVER THE CONVEYOR WHEN HE FELT A SHARP PAIN IN HIS LEFT CALF. POSSIBLE REPETITIVE MOTION FOR THE PAST 2 DAYS APPROX 5 HRS/DAY STANDING ON THE BALL OF HIS FOOT.
Employee was helping remove a hoist cylinder from a 980G cat loader. He was holding the eye of the push rod guiding the cylinder out. When it cleared, it swung forward carrying him with it, knocking him into a support pipe holding the boom arm up. When he let go of the rod eye, the middle and ring fingers of his right hand were cut below the end knuckles.
Employee was on the haul road with his haul truck. There was a large rock in the roadway. Employee proceeded to roll the rock off the road and in the process hurt his shoulder. *****1st day of treatment = 9/26/06******
EMPLOYING WAS THROWING A ROCK OUT THAT HAD JAMMED IN THE CRUSHER. ****1ST DR VISIT ON 12/07/06****** MSHA REPORT NOT FILED AT THE TIME..DID NOT ANTICIPATE THIS TREATMENT.
EMPLOYEE WAS CLIMBING LADDER WITH A PROPANE TANK, HE USED HIS KNEE AS A HELPER TO HOIST TANK TO PLATFORM, FELT PAIN IN RIGHT KNEE. *** 1st DOCTOR VISIT 2/08/06***
Employee was making a J bolt out of a concrete anchor. Instead of using a hammer he used a 2" steel pin to strike the bolt. While hammering he slipped causing him to slide his little finger into the bolt causing laceration.
EE WAS REPLACING THE BEARING ON THE BEND ROLLER. THE CABLE HOLDING THE ROLLER SLIPPED CAUSING IT TO HIT HIS FINGER CAUSING A LACERATION.
EE WAS STANDING ON A LADDER, UP APPROX. 6', WHEN HE OVER EXTENDED, CAUSING THE LADDER TO FALL TO THE SIDE. EE HIT HIS SHIN.
EMPLOYEE WAS PUSHING A GRIZZLY BAR WITH HIS BODY OFF THE BED OF HIS PICKUP TRUCK, WORKING IT BACK AND FORTH WHEN HE FELT SOMETHING PULL IN HIS UPPER THIGH AND GROIN.
EE WAS PULLING A 50 LB. BAG OF ANFO FROM PICKUP BED. FELT POPPING IN HIS ARM.
EE WAS LIFTING A ROCK TO UNJAM CRUSHER, WHEN THE ROCK ROLLED INTO ANOTHER ROCK, PINNING HIS FINGER, CAUSING LACERATION.
EMPLOYEE WAS LIFTING A SAW HORSE WHEN HE FELT PAIN IN HIS RIGHT ARM, SHOULDER AND NECK.
EE WAS LIFTING A CHAIN TO HOOK ONTO FRONT-END LOADER TO PULL HIS HAUL TRUCK OUT OF MUD. FELT PAIN IN ABDOMEN.
