Employee alleges they were changing a drill bit and the drill fell from the hammer and pinned the index and ring fingers under the drill bit causing injuries to their right hand. Employee also received stitches.

Employee was patching a chute with a piece of plastic poly board. A co-worker was power drilling from inside chute while Employee was holding poly board in place on the outside. Employee instructed driller to drill top first, inadvertently drilled bottom holes causing injury to Employee's left wrist. Employee sent to Hand/Wrist specialist, where it was cleaned up and sutured.
Employee was cleaning up material at the base of the scalping screen due to spillage. Another employee was on top of loose material on top the screen to dislodge (approx 35-50 lb) rock. That rock fell, hit the guard, then dropped down about 4" onto employee's right shoulder.
Employee was using a Stihl chainsaw clearing trees. Chainsaw kicked and hit EE's left thigh causing wound/abrasion.
Employee was lifting a full argon gas cylinder for the welder. Felt pain in lower back. Returned 1/08/21 - didn't report until 1/11/2021 - at that time we sent EE to the Doctor.
Employee was lifting right leg to climb ladder onto 992K CAT LOADER, EE felt a pull in the right abdomen area. Didn't think much of it, therefore it was not reported that day. That evening while trying to sleep, sharp pain in the lower abdomen/groin area kept EE awake. Reported on 9/23, went to Dr and EE has an Incisional hernia, requiring surgery.
Employee was working on a conveyor scraper. While tapping on scraper bracket with hammer, struck right forefinger knuckle against bracket causing abrasion. EE did not initially report injury, thought it to be minor, on Sunday 9/20 it was infected and sought treatment @ urgent care.
Employee was working on the shop floor underneath the Komatsu 900 pit loader. When coming up from under machine EE hit head on the edge of the hydraulic tank causing a laceration on the top of EE's head, requiring stitches.
Employee was using a utility knife to cut a piece of conveyor belting. In the process the blade pulled from the belting, the knife struck EE's right leg which caused a laceration that required sutures.
employee was walking on crusher catwalk after crusher went down, stepped on rock and rolled ankle causing ligament strain of right ankle.
Employee arrived for the shift, while walking across the wet limestone parking lot EE slipped and fell onto knees, causing contusion on L knee. At time of this injury on 9/3/19, EE thought was okay. On 10/24/19 EE reported knee still bothering EE. Was sent to Dr for 1st visit. On 11/4/19 EE had follow-up & Dr put EE on restrictions.
employee was stepping down out of man lift basket onto catwalk with a piece of metal to replace in the scalper. When EE stepped onto catwalk EE heard a pop and felt pain in the back of EE's lower right leg.
Employee was at the crusher spraying inside of chute with 115 psi. The hose tip was just inside the chute door. Something broke loose above and it fell down hitting tip of the hose which caused employee's arm to go downward causing bone fractures in left arm.
Employee was going up steps of skid steer to pull the keys from the ingnition for the day. After retreiving the key, EE pivoted on EE's foot causing the boot tread to get caught in the step of the machine. Employee was thrown off balance and fell approx 2' to ground on right shoulder.
Employee was using pick axe to clear chute build up. The pick axe hit hard build up and slid off going deeper into chute, causing employee's finger to strike the chute hinge causing laceration.
Employee was pulling on gate to change product flow in crusher finishing tower, put left hand in inappropriate area causing pinching laceration on left hand, ring/middle finger.
Employee was attempting to close a misaligned latch on a utility bed tool box with both hands when ee heard a pop in left shoulder.
Employee was using a magnetic drill when the bit broke, causing the mag drill to spin loose and smashed employee's right thumb. INJURY BECAME REPORTABLE ON 5/30/17 WHEN EMPLOYEE WENT BACK TO DR AND NEW XRAYS SHOW A BREAK - RESTRICTIONS BEGAN ON 5/30/17.
Employee was using a walk behind bobcat to clean up spillage under conveyor structure. After filling bucket employee lifted bucket with controls, causing the back end of the machine to rise pinching left hand between the controls and the conveyor structure. Pinching caused laceration and abrasion.
Employee has been diagnosed with carpal tunnel in both hands. Employee does a lot with EE's hands, loading blast holes, connecting detonators, shoveling, operating skid loaders and general plant maintenance. Scheduled surgery on 3 28 16. ****THIS REPLACES SUBMISSION # 2243074 FROM 03/18/16****
Employee started to climb the ladder on his front-end loader when he felt a pain in his left shoulder.
Employee was lifting a yoke that secures a transmission in place on a R-50 Euc haul truck. Lifted it chest high and was trying to lift it higher when he felt a pop in right shoulder.
Employee was holding a level on a piece of 3/4" rubber belting while co-worker used a box cutter. Box cutter slipped out of belting causing it to strike employee in the top of wrist causing laceration.
Employee was attempting to exit a scissor-manlift, grabbed the handrails above with both hands lifting body weight and swinging out of the machine. Employee heard a pop and had pain in the top of his right shoulder.
Employee lifting 5 gal grease bucket for the rock drill. Lifting with his left arm to place into cabinet on drill and felt pain in his left shoulder.

While pressure washing machine, water splashed back onto face and safety glasses. The following day @ shift close employee reported something in eye from day before. Employee was sent to the eye clinic where they washed his eye out and gave him eye drops. MSHA SITE DOWN 10/24 DUE TO MAINTENANCE said to try again today 10/27/14 for reporting.
unloading rocks out of primary housing with a platform hooked to Linkbelt. platform hung up on one end, employee stepped on platform, held the cables and tried to wiggle it free, the platform broke free causing a jerking motion. FIRST WORKDAY WITH RESTRICTIONS 7/07/14
Employee was working on a Needle Scaler and was cutting the needles with a cut off wheel. The cut off wheel grabbed and jerked causing a laceration to the employee's (L) forefinger.
Employee was helping to move a gate in a chute under construction. Employee was pushing down on the gate handle with his rt hand. When the gate handle moved downward the employee lost his balance, grabbing the corner of the chute with his left hand to catch himself, inadvertently causing his ring and middle finger to become pinched by the moving gate, causing lacerations.
Employee was rolling air hose onto reel by hand when a twist at the end of the hose came undone and hit him in the mouth, causing his tooth next to the front tooth to break off.
EMPLOYEE WAS LIFTING AND ROLLING ONE END OF AN OAK TIMBER OFF OF A LARGE CONCRETE BLOCK WHEN HE FELT A PULLING IN HIS RIGHT UPPER ARM.
Employee was resetting the cone at the crusher with a sledge hammer when he experienced pain in his right shoulder. NOTE: PER MSHA INSPECTOR A 7000-1 WAS NOT NEEDED AT THE TIME OF HIS FIRST DOCTOR VISIT OF 1/04/12. THE EMPLOYEE HAS SINCE BEEN PUT ON RESTRICTIONS STARTING 04/05/2012.
Employee was using a hand grinder with a 4" cutoff wheel to cut an opening on an electrical box when the grinding wheel caught a corner, broke apart, and kicked the grinder back, striking the employee in the left hand, just below the base of his pinky finger causing a laceration.
Employee was working in the primary feeder hopper. He was standing on built up material in the hopper, clamping a piece of belting up so access to the rotor was open. He turned to climb down and his foot slipped, causing him to fall sideways about 2 ft into the bottom of the feeder trough, impacting his right shoulder.
Employee was walking on the grizzly bars inside the feeder hopper to enter the impactor housing to set up wire feed welder on hammers. Employee's foot slipped off top of bars and slipped in between causing top of foot to over extend.
Employee was welding and had a steel hopper raised and secured with nylon straps. One of the straps failed causing the hopper to shift. Something hit the employee on his welding hood or shield causing injury.
EMPLOYEE WAS ON DOZER PLATFORM AND DROPPED HIS PENCIL, BENT OVER TO PICK IT UP AND BUMPED HIS HEAD ON THE MACHINE WHICH CAUSED HIM TO LOSE HIS BALANCE, LOST HIS GRIP AND SLIPPED DOWN ONTO HIS PICKUP TRUCK HOOD WHICH WAS PARKED NEXT TO HIS MACHINE, THEN SLIPPED DOWN TO THE GROUND.
employee was walking up on hill # 4 (with safety glasses on) to work on hydraulic drill, when he felt scratchiness, burning and watering of the left eye. foreign object in eye.
EMPLOYEE WAS HOLDING IN POSITION A PIECE OF WATER PIPE ON ONE END AND THE CRANE WAS HOLDING THE OTHER SO THEY COULD BE WELDED TOGETHER. THE END THE EMPLOYEE WAS HOLDING DROPPED DOWN 6" TO THE GROUND CAUSING THE EMPLOYEE'S PINKY FINGER TO GET BETWEEN THE PIPE AND THE GROUND, CAUSING LACERATION.
Employee was using wheel grinder on a piece of plate steel. Foreign object in right eye.
Employee was using a 6' pry bar on the hood at the shaker. The pry bar slipped and caused him to lunge forward on the catwalk approx 10' falling against the lower hand rail.
Employee was removing bent conveyor for repairs. While using a 12' x 12' timber to block the conveyor, the timber was pushed to far on one end causing his end to drop down onto angle iron which caught his pinky on right hand, which partially tore the fingernail from the nailbed.
Employee was getting off his excavator when he noticed there was mud on the step. To avoid slipping from the step he jumped from the track approx 3' landing on his left heel, which in turn caused pain in his back and leg.

EMPLOYEE WAS PUTTING A CONVEYOR BELT ON WITH A ROPE ATTACHED TO A PULLEY THAT WAS THEN ATTACHED TO A TRUCK TO PULL. THE PULLEY BROKE AND THE ROPE HIT THE EMPLOYEE IN THE FOOT/ANKLE (R).
EMPLOYEE WAS HAMMERING ON WOOD BLOCK TO REMOVE SHAFT FROM THE SHAKER. MEANWHILE, THE SHAFT BREAKS LOOSE FALLING ONTO EMPLOYEE'S (L) LEG PINNING IT BETWEEN THE SHAFT AND THE DIVERTER PLATE.
