Technician was laying under a loader installing a driveshaft. The bearing caps were not secured and one slid off the end and struck technician in the tooth, causing a horizontal fracture.

I was installing a drag friction shoe on a 2400 Lima dragline and during the course of installing the shoe into the drag drum flange the shoe slid to the right and I reached down to catch the shoe to keep it from falling and it pinched my finger between the shoe and the mounting bracket

EMPLOYEE WAS WEIGHING TRUCKS AND GIVING DRIVERS TICKETS AND WENT TO TURN TO GET ANOTHER TICKET AND HIT CORNER OF PRINTER WITH BACK SIDE OF RIGHT HAND.

WHILE HELPING REMOVE A PIN FROM A TRAVEL CHAIN OF A DRAGLINE THE PION CAME OPUT AND HIT H IM JUST ABOVE THE EYE
