EE was on 3 person team changing V-Belts at Impact Crusher. Hand rolled under pulleys while pulling belt. Right thumb fractured from pinch point.
Injured employee was opening hatch lids on top of trucks when they strained their back. Employee was working with case management for remedies when they decided to be seen by a doctor which then elevated case to reportable.
Miner was on top of the rail car opening the lids and strained ligaments of lumbar spine.
Employee worked on multiple jobs this day. One of them involved replacing a gearbox on a screw conveyor. Employee noticed EE's back was sore after carrying the gearbox up the screw conveyor.
Employee was changing brake pads on a haul truck. As the employee lifted the caliper up EE attempted to slide a jackstand under it. EE's hand slipped off of the caliper and it dropped catching right ring finger between the jackstand and caliper. Employee suffered a broken right ring finger.
Employees were reinstalling the belly pan on the undercarriage of the dozer. The rigging on the belly pan slipped allowing it to swing back down and pin the mechanic. The mechanic was diagnosed with three fractured ribs. Mechanic received a restricted work designation to lift no more than 20 lbs. and is due back to the doctor on 8/11/2021 to follow up.
Employee bent down to pick up 50lb weight to calibrate the warehouse scale. They picked the weight up about half way before they felt a sharp pain in groin area and it radiated down the leg. The employee suffered a strained groin and was put on to restricted duty after seeing a doctor.
EE was engaged in the process of rolling out lay-flat hose when they caught the toe of boot on a small rock falling forward landing on right side. EE tried to catch themselves with right hand, however hand slid in wet material that was present. EE had multiple hair line fractures to ribs on right side and was restricted from normal work activity.
Miner works in bagging department and is unsure what may have caused is right forearm to hurt starting on 12/16/2020. The miner informed supervisor on 12/21/2020 and was sent for medical evaluation. Miner was diagnosed with tendinitis on right arm, given anti-inflammatory medication, and put on restricted work duty until 12/30/2020.
Employee was stacking finished material bags onto pallets off the conveyor. As EE picked up a bag and turned to place the bag on the pallet, EE felt a pop and sharp pain in EE's right chest/ribs. EE immediately fell to the floor and called for help. The doctor diagnosed EE with a muscle strain in EE's right thorax wall in the ribs.
EE was lifting a railcar vibrator into place when EE felt a pop on the inside of the elbow. The EE experienced soreness, but did not report the injury to management until December 15th. It was at that time, the employee sought medical treatment and was placed on restricted work duties.
Employee was closing lids on contractor trailer when thumb was caught between latch and dome lid. Employee had applied lubricant to free the latches when right thumb was caught in pinch point fracturing tip.
On 07/03/19, employee was working with broken bags of hydrate lime in the packing department. At the end of EE's shift EE felt a burning irritation on the upper bilateral inner thighs. On 07/10/19, the employee reported the upper thigh injury and an additional heat rash on a small part of EE's top right foot.
A Utility worker was lifting and setting plugs while participating in poke out at K3. Towards the end of the job, the employee felt discomfort in EE's groin. Medical attention was sought and the employee was placed on restricted work duties.
EE was removing a hydraulic line on the crusher rock breaker. When the fitting was on the last thread it whipped and struck the employee on the left calf causing a laceration that required two stitches.
A contractor was completing demolition in the decommissioned bagging area. As the employee was pulling on an electrical local disconnect box to remove, the box fell, landing on the employee's lower leg. The employee sustained fractures to both bones in lower leg and surgery was required to repair one of the fractures.
An employee was shoveling material around the outer wall of the 70x200 building, when the employee felt a pull in the middle of lower back. The employee strained their back.
EE stained neck while lifting air cylinder.

Employee was lowering a ladder at loadout and felt a pulled stomach muscle; medical treatment was declined by employee. On 12/26/17 the employee had non-work related surgery, when the surgeon noted and repaired an unknown hernia. On 2/2/18 the employee notified management the hernia repair was a work-related injury. The injury is under investigation to determine work-relatedness.
Miner was removing a compressed air hose from a wall hook when the hose Chicago coupling hit the miner's upper front teeth.
EE was removing automatic door hinge when one of the arms of the hinge struck ee in the mouth resulting in a laceration that required seven stitches.
Worker was descending stairs; while holding the hand rail and walking down the stairs, left hand, little finger was caught between hand rail and structure steel beam jamming and bending it.
Worker was reaching to pull a 50lb bag of gypsum when lower back began to hurt.
EE was prepping railcar for loading. EE strained back when opening railcar hatchlid. EE declined medical treatment at the time of the incident, however, sought professional medical treatemnt on 8/24/2016 and was assigned restricted work activities.
EE was cleaning off # 3 belt at the primary crusher when EE noted left leg began to cramp. EE sought professional medical attention on 6/19/2016. EE was assigned restricted duty and prescribed meds.
EE was discarding worn out belt idlers when EE turned and felt a twinge in the lower right side of their back. EE did not report to immediate Supervisor until 5/17/2016
EE states on May 9th was working in the bag house stacking 50lb bags onto a pallet. EE noticed a small pain lower back, yet continued to work and finished the shift. EE reproted severe pain on May 12, 2016 to Supervisor.
EE was replacing bags in the r/mill baghouse. Task began upon start time (7:00 AM). EE states lunch break @ 12-12:30 PM. Completed task appr. 2:15 PM and noticed tightness in mid to upper middle portion of back. EE cleaned up used bags and tools, and clocked out @ 3:30 PM.
The kiln pre-heater was clogged. In the process of clearing the clog, a hot pebble bounced of the clearing rod and landed on the neck of the injured worker. The pebble caused a 1st degree burn on the workers neck.
An employee was lifting an end of a screw flight, when employee felt pain in lower back. The employee was placed on restricted duty by a medical provider.

Employee was rolling balls of aluminium foil to calibrate crusher, when the box of aluminium foil fell off the table. The employee went to catch the box, when the blade side of the box cut the employee on the hand. Employee was taken for medical care and received stitches to his hand.
Electrical motor failed on CL-1044m at the 1600 tower. Electricians were called in to replace the motor. Employees were engaged in the process of rigging a come-along from the elevation directly above the motor when material was dislodged entering employees right eye.
Employee shoved troughing idler through gate on aerial lift that was being utilized to transfer material and idlers from 1308 conveyor. Lift was lowered to with 6' to 8' of the ground and idler was shoved out by employee. Employee was struck on the right ankle by troughing idler. Ankle was significantly sprained and employed was placed on restricted duty with medication.
Two employees were removing material from beneath #4 storage, when employee was struck in the mouth with hose from vacuum truck. Hose connection was being taped and was rested against concrete wall, when hose connection slipped striking employee fracturing #9 tooth.
EE was assigned the task of removing tool boxes off of old Maintenance truck that is being taken out of service. EE was using large timber (appr.4' portion of railroad tie) to slide under the tool box (before he began to cut it off of the bed) and it slipped out his hands pinning the tip of his left ring finger on the concrete floor of shop.
EE was assisting in removal of the guard on the W16X conveyor tail pulley. He was positioned on the end of the guard attempting to loosen the guard from the excess material around the inside of the tail pulley. As he and another EE was trying to free the guard the injured EE pinky finger on his left hand was pinched between the corner of the guard and the wall.
Lime dust puffed up when a sample bucket was dropped. The dust got into the eyes of the worker. This caused irritation and discomfort.
EE was assisting supervisor replacing a diesel fuel nozzle (@ the West Shop diesel tank). Upon disconnection of the hose from the nozzle, residual diesel came out of hose and sprayed onto the EEs hardhat, which dripped into the EEs right eye. EEs eye was flushed by co-worker. EE sought medical treatment and given a prescription for eye meds to treat afterwards.
EE was cleaning debris and rocks from the lift cylinder and a large rock fell on right foot.
EE was climbing 8X12 access ladder (fixed) when he struck his right knee cap on a rung. EE was offered medical at the time incident was reported and refused. Appr. 9pm that evening EE sought professional medical treatment. EE was released with no restrictions and returned to work on the next scheduled work day. On 02/17/2015 EE had Rx filled.
The EE started work at 1900 10/3. At 0100 saturday the EE felt a pain in his low back from lifting rail car doors. The EE returned to work at 1900 10/4 with no complaints of pain and while lifting other doors he felt a pain in his upper back. On 10/6 the EE asked to see the Dr and was placed on restricted duty.
An employee was carrying a 72# shaft and bearing set from a screw conveyor up a flight of stairs. He felt a burn in his groin and was found to have a hernia.
On 06/23 the employees was working in the cooler pit below the kilns raking scale at the time he was wearing a tight fitting respirator. While raking he received minor lime burns. On 07/13 he requested medical treatment for the burns on his face that were not healing.
